    Dementia-aware staff, excellent care, recommended

I'm very pleased with the kind, professional, dementia-aware staff and strong rehab/nursing care in a clean, attractive facility. Family communication is good, visits are allowed by appointment, and the activities director runs engaging puzzles and holiday/birthday events-I'd recommend this pleasant, well-cared-for community.

    Summary of reviews

Overall impression: Feedback for Walnut Grove Living Center is mixed. Several families praised the facility for compassionate clinical staff, active programming, and an attractive, clean environment; others raised substantive concerns about operational consistency, particularly around responsiveness, property controls, and infection/pest management. The result is a polarized set of experiencesโ€”some consistent positives alongside recurring operational weaknesses that prospective families should evaluate during a visit.

Care and clinical services: Many accounts highlight attentive, dementia-aware nursing and effective rehabilitation/therapy services, with some families stating that clinicians were intuitive to residents' needs and provided clear health updates. At the same time, there are recurring concerns about delays in assistance (including slow responses to call lights and long waits for toileting or transfers) and inconsistent bathing schedules. Additionally, reviewers described gaps in medication handling and tracking that suggest the facility could strengthen medication-administration controls and audit practices.

Staff and management: Staff behavior descriptions vary considerably. Numerous comments describe staff as kind, welcoming, and professional, with a helpful front desk and engaged caregivers. Conversely, other accounts note indifferent or unprofessional interactions and specific concerns about administrative communication and professionalism. There are also serious allegations concerning financial or property mishandling; these represent high-severity issues that families should probe directly with management and through official channels.

Dining and activities: The activities program is consistently cited as a strengthโ€”an active director, puzzles, parties, birthday and holiday celebrations, and a mix of group and individualized options contribute to social engagement. Food quality is described as decent by some reviewers, though assistance and monitoring at meal times appear variable, and families are advised to observe mealtime staffing and resident support during a visit.

Facilities and environment: The facility is frequently described as clean and pleasant with an attractive interior and a generally welcoming ambience. However, there are pest-control concerns noted for some units and complaints that outdoor common areas are dominated by smoking activity, which can limit their use. Some residents are in shared rooms; prospective families should confirm room arrangements and housekeeping standards during a tour.

Operational patterns and recommendations: The main operational themes to explore further are staffing consistency and training, call-response times, medication and personal-property controls, availability of transfer-assist equipment and safe-lift procedures, pest-control protocols, and how the facility handles visitation during infectious outbreaks. Given the polarized feedback, visit in person at different times of day to observe call-light response, mealtime assistance, and activity engagement; ask for staffing ratios, lift-equipment availability, medication-administration policies, incident/complaint resolution processes, and recent inspection or remediation records. These checks will help determine whether the facilityโ€™s strengths align with a particular residentโ€™s needs and safety expectations.

    About Walnut Grove Living Center

Walnut Grove Living Center offers skilled nursing care and several senior living options, including nursing home services, assisted living, and memory care, and the place provides a peaceful environment with views of gentle landscapes that help folks feel at ease and maybe a little less anxious. The staff includes registered nurses like Stacy Gideon as well as a full team of medical, therapy, and support professionals, and they're known for working closely with people to make sure everyone's needs get attention, whether someone needs round-the-clock help or a bit more independence. Administrators Peggy Bennett and Waynette Bennett oversee the facility's operations, and Walnut Grove runs as a for-profit partnership, tying in with Walnut Grove Care & Rehab and a reputable healthcare organization that's been steadily serving southeastern Oklahoma for a long time.

Residents get access to physical, occupational, speech, and aquatic therapy, and there are hospice services, rehab, help with medication, and special programs for folks with dementia. There's a strong focus on person-centered care through things like customized diet and nutrition plans, help with homemaking, and plenty of everyday support. Meals are cooked on-site with attention to nutrition, and staff are always around to offer a hand with daily tasks, making sure needs don't go overlooked. The long-term care quality rating stands at a C, meaning it meets standard requirements, but the facility inspection grade sits at an A, which means the state found few problems or deficiencies during checks. Walnut Grove Living Center accepts both Medicare and Medicaid.

Amenities and activities aim to help everyone stay comfortable and busy, whether it's social events, outings, or quiet relaxation. Residents have a council that gives them a voice, but family members don't have their own council. The center has won different awards and holds accreditations for its services over the years, showing a steady dedication to healthcare. The facility uses online platforms for communication and information, and keeps privacy choices clear, with features for activity logging and account access. Walnut Grove Living Center highlights a history of caring for seniors, building a local reputation over generations, and tries to provide a warm, welcoming place where people feel supported and respected.
