    Compassionate staff, engaging activities, recommended

I'm very pleased with the compassionate, patient staff - nurses Sam and Clarissa and social services director Crystal Gage especially stood out, treating my mom like family and truly going above and beyond. Dedicated physical therapy improved mobility, activities (bingo, exercise) and personalized meals kept her engaged and comfortable. Friendly, attentive team (Betty, Jamie, Jessica, Michelle, Nadia, Sally, Ryan) made our stay reassuring and supportive; I'd recommend this facility.

    Summary of reviews

Reviews of Avamere Rehabilitation of Clackamas present a mixed picture in which strong direct-care relationships and therapy outcomes coexist with systemic operational weaknesses. Many families and residents praised individual staff—nurses, CNAs, and therapists—who provided compassionate, attentive bedside care and helped residents regain mobility. The facility’s therapy programs and proactive activities director receive particular commendation for rehabilitation results and meaningful engagement. Social services support and availability of private-room options are additional positives cited by families, and the facility accepts Medicare/insurance for rehab admissions.

At the same time, reviewers describe recurring clinical and operational concerns. Several accounts point to inconsistent clinical oversight related to wound and post-operative care, difficulties coordinating follow-up appointments, and lapses in timely wound management. Medication administration and pain control were described as uneven across stays, with some families observing reliable medication management and others reporting missed or delayed doses. These clinical inconsistencies are often tied to staffing patterns: many reviews mention chronic understaffing, heavy reliance on agency or traveling staff, and particular strain on night shifts, all of which reviewers link to delays in assistance and uneven care continuity.

Staff-related feedback is polarized. Numerous comments highlight compassionate, patient-centered interactions—staff who treat residents like family, accommodate dietary preferences, and go above and beyond routine duties. Named staff and therapists drew positive mention for bedside manner and clinical skill. Conversely, other reviewers described communication breakdowns, limited inter-shift handoffs, and occasional unprofessional conduct or poor responsiveness from specific staff members. The presence of traveling/agency nurses is associated with variability in care experience between shifts.

Dining and activities present mixed impressions. The activities program and participation opportunities are frequently praised, with organized daily events and individualized engagement. Meal experiences vary: some reviewers report well-seasoned, accommodating meals, while others describe cold or poorly timed service and mechanically textured diets that did not meet expectations. Call-light responsiveness and timely assistance with bathing or shower schedules are also inconsistent and sometimes linked to the same staffing shortfalls.

Facility and management issues are another recurrent theme. The building is described as aging, with maintenance needs such as leaking pipes, window problems, trim deterioration, and stained rooms; some reviews reference odor concerns in certain areas. Administrative responsiveness drew both praise (supportive social-services leadership) and criticism (billing questions, perceived indifference from management). There are also serious, though not uniformly documented, claims including allegations of theft and billing irregularities; prospective residents should consider these claims when evaluating financial transparency and security protocols.

For families considering this facility, it is advisable to perform an in-person visit and ask specific questions about staffing ratios, on-site clinical oversight (physician and nursing availability), wound-care and post-operative protocols, call-light response metrics, meal service procedures, and billing practices. Clarify visitation rules tied to vaccination status if that may affect access. Overall, Avamere Rehabilitation of Clackamas may offer strong, compassionate hands-on care and effective therapy for some residents, but visitors should weigh those strengths against reported variability in clinical consistency, staffing, facility maintenance, and administrative practices.
