    Warm compassionate care, excellent rehab

I placed my mom here for short-term rehab and have been very pleased. The staff are warm, compassionate and highly skilled - nurses, therapists and CNAs gave individualized, top-quality rehab that helped her walk again; the facility is immaculate, well-equipped, and the meals are delicious. She's happier, healthier, and well cared for medically, emotionally and physically - I'm grateful and would highly recommend this team.

    Summary of reviews

The reviews for this facility present a polarized picture. Many families and residents praise the clinical and rehabilitation services: nursing staff and therapists are frequently described as compassionate, skilled, and focused on individualized treatment plans. Short-term rehabilitation outcomes are commonly noted as very good, and the facility’s therapy gym and rehabilitation amenities receive positive mentions. Several reviewers also highlighted specific staff members and leaders who provided clear clinical direction and infection-control attention.

Activities and dining receive substantial positive feedback. The activities program appears broad and engaging, with regular outings, seasonal events, craft programs, and unusual offerings that contribute to resident engagement. Dining quality is often described as a strength, with varied menus and meals that residents enjoy. When admissions and administrative processes work well, families report helpful guidance with paperwork and Medicaid assistance.

At the same time, reviews identify repeated operational concerns related to the physical environment and day-to-day care. Cleanliness and sanitation issues, along with odor concerns in particular units and corridors, are recurring themes. Housekeeping and environmental maintenance are described as inconsistent, and there are specific mentions suggesting pest-control needs. These environment-related concerns are frequently tied to perceptions of reduced comfort and dignity for residents.

Clinical-safety and staffing patterns are another area of divergence. Multiple reviewers noted inconsistent responsiveness from staff—particularly during overnight or weekend shifts—and described gaps in fall-prevention, transfer safety, and wound or catheter management. These observations point to uneven direct-care staffing, variable supervision, and the need for clearer protocols and training in higher-acuity care. Families also described shortcomings in communication and coordination after clinical incidents, including billing and discharge-planning problems and occasional transport or admissions miscoordination.

Management and leadership earn mixed assessments. Several reviews commend particular administrators and project leaders for responsiveness and positive engagement, while others cite inconsistent communication, slow follow-through, and concerns about operational oversight. A small number of reviews raised serious claims, including allegations of dishonest conduct and questions about post-incident handling; these accounts suggest the value of external investigation or quality-audit attention where warranted.

For prospective residents and families, key takeaways are: the facility appears capable of delivering strong rehabilitation and compassionate day-to-day care from many staff members, and it offers an active activities program and generally well-regarded dining. At the same time, prospective families should assess unit-level cleanliness and odor control, inquire about staffing patterns for nights and weekends, confirm fall-prevention and wound/catheter-care protocols for higher-acuity needs, and ask for clear communication and discharge-planning procedures. A focused conversation with leadership about remediation plans for environmental and safety concerns is advisable before placement.

    About Jewel Health and Rehab

Jewel Health and Rehab sits at 535 North 17th Street in Allentown, Pennsylvania, where it works as a skilled nursing facility and also as a healthcare and rehabilitation center, and folks find both short-term rehabilitation and long-term care here, especially for those who are very frail and need a lot of help each day. The facility's got private suites and rooms you can share, all with private bathrooms, and every room has air-conditioning, a telephone, internet, cable, and WiFi, so residents can stay cool and connected without any trouble. There's help with meal preparation, daily meals served, and the place takes on housekeeping and linens for the folks living there, making sure things stay clean and comfortable, and you'll find 24-hour supervision with staff always close by.

Nurses, certified nursing assistants, physical therapists, and licensed medical professionals all work here, providing medication management, assistance with daily activities, coordination with doctors, and transporting residents to appointments or arranging transportation when needed, and if you need personal care or skilled nursing around the clock, they've got that too. There's a 24-hour call system for emergencies, help with mobility, and regular engagement programs for residents who might need special care, including a memory care unit for those living with dementia or Alzheimer's-plus protected areas so folks don't wander off. The staff pays close attention to personalized care plans, and they really keep to a team approach, even though the nurse turnover rate sits at 42.5%, with nurse staffing around 3.45 hours per resident per day, and on average 134 residents stay here with a total of 146 certified beds.

Rehabilitation services at Jewel Health and Rehab focus on helping people regain independence and work towards recovery, and the facility says it values empathy, respect, and excellence in its approach, which is part of what they call a holistic and transformative way of caring for people. There are therapeutic activities, personalized plans, and an emphasis on dignity and comfort in both daily life and medical care, and while folks who manage the place-Diamond Healthcare At West Allen Mgmt LLC and Avon Pa LLC-took over in September 2023, the indirect owners include Chaim Brecher, Yosef Frankel, and Benjamin Kurland, with direct ownership under Diamond Healthcare At West Allen Holdco LLC. Now, while the facility aims to offer luxury and comprehensive health support, state inspections have found some issues-like trouble offering trauma-informed and culturally sensitive services, as well as problems meeting some food preferences-so it's good to know both the positives and things they need to work on. All in all, the place tries to create a safe and supportive environment with both short and long-term skilled nursing, specialized memory care programs, and a range of amenities meant to help residents feel comfortable, cared for, and part of a community.
