    Responsive caring staff and therapy

I'm very pleased with the new Director of Nursing and the whole team - responsive, kind, and genuinely caring; my mom's communication has improved and they're fast to answer questions. The rehab/therapy staff are excellent, activities are engaging, and the facility is clean and professional - residents clearly receive loving, attentive care.

    Summary of reviews

Greenfield Healthcare and Rehab elicits mixed impressions. Several reviewers describe compassionate, attentive caregivers and therapy teams that produced measurable functional gains and improved communication for some residents. The activities program is frequently mentioned as engaging, with organized events (for example, bingo) and therapy-led social opportunities that families view positively. A number of comments also describe a clean environment in certain units and note improvements in leadership responsiveness since a new Director of Nursing took over.

At the same time, a pattern of operational weaknesses emerges. Multiple accounts indicate inconsistent staff responsiveness, including long call-bell wait times and delays in attending to basic needs. There are also concerns about clinical oversight: some families reported limited physician rounds, prolonged periods without direct medical assessment, and gaps in management of oxygen or other clinical needs. These items point to variability in clinical monitoring and handoff practices across shifts or units.

Care coordination and administrative processes appear disorganized in some cases. Families described delays and confusion around intake, discharge, and the setup of home health services, and at least one account referenced a privacy-management concern involving administrative outreach to a relative. Communication breakdowns extend to billing and insurance coordination, with at least one report alleging services were billed despite not being received.

Facility conditions are another mixed area. While some parts of the building and rooms are described as clean and orderly, other accounts raise sanitation and waste-management gaps, maintenance issues such as water damage and mold in bathroom areas, and pest-control concerns. These indicate uneven upkeep and infrastructure reliability across the campus.

Staff conduct and consistency vary: several reviewers praise specific staff members and therapy teams as compassionate and proactive, while others describe rudeness, unprofessional interactions, and difficulty locating staff when needed. Personal-care delivery also appears inconsistent, with references to missed or delayed bathing and related care tasks.

Overall, the facility shows strengths in therapy, activities, and pockets of compassionate caregiving, but prospective residents and families should evaluate operational reliability before placement. Recommended areas to discuss with administration during a tour or intake include staffing levels and call response expectations, physician coverage and clinical monitoring routines, sanitation and maintenance protocols, coordination of discharge and home-health services, and how privacy and billing concerns are handled.

    About Greenfield Healthcare and Rehab

Greenfield Healthcare and Rehab sits over in Erie, PA, where they offer a place for seniors who need skilled nursing, rehab, and long-term care, and folks living there get care 24 hours a day from the nurses and staff, and you'll notice there's all sorts of meals that get planned to fit special diets like for those living with diabetes or heart problems, so if you need certain foods, they'll do their best to help with that, and besides meals, there's a focus on making the place feel comfortable, almost homelike, with cozy rooms, spaces for recreation, and social events or activities so residents can interact if they want or just spend time however they like. The building's got about 133 certified beds, and on an average day, around 79 people call it home, which means there's usually room to get to know one another without feeling too crowded or too empty, and if someone's been in the hospital and needs rehab, or maybe long-term help, Greenfield offers services aimed at improving health, such as physical therapy, speech therapy, and occupational therapy, and therapists and nurses are available for those specific therapies as needed, and Greenfield provides specialized medical care for people dealing with serious issues like pain, wounds, stroke recovery, cardiac or respiratory care, tracheostomy, diabetes management, and orthopedic recovery.

You'll find advanced care available, including help for folks coming in for short-term rehabilitation after surgery, or those who might need hospice care, memory care, or respite care, and along with the staff's efforts to look after residents' well-being, they have programs and social gatherings to keep everyone involved if that's wanted, so being in a community where something's always going on is part of their model. They accept both Medicare and Medicaid, which matters to many families, and Greenfield is a for-profit corporation that's managed by Elizabeth Kachel, Charles Rohrbach, and Abraham Smilow, with links to Ghc Snf Opco Holding Company LLC and other entities like Hillel Tropper 2016 Irrevocable Trust, and Abraham Smilow also has a connection in both management and ownership. Even with all these services, you'll want to be aware that inspection reports from CMS show Greenfield has had some problems over time-there have been forty deficiencies found in their reports, and three of those were infection-related, meaning the inspectors found concerns about infection control that did not meet federal standards, and there were other issues like not always having proper doctor reviews, not documenting everything as required, and not honoring every resident's rights all the time, so families should know this part too.

On staffing, Greenfield reports nurse hours per resident each day at 3.21, which falls below the Pennsylvania state average of 3.9, meaning nurses might be stretched a little thinner, and the nurse turnover rate stands at 61.4%, higher than the state's average of 47%, so some residents might notice staff changing more often than in other places. Still, the care team keeps focused on providing support, especially for those who need help regaining mobility or living skills, and they aim for recovery with respect and a positive outlook, but it's good to remember the facts about staffing and inspection history when comparing options. The place offers regular activities, good food that tries to meet everyone's health needs, and a setup that tries to make people feel respected as individuals-so for someone in Erie or nearby needing skilled care and a range of rehab options, Greenfield Healthcare and Rehab provides many services under one roof, with a clear effort to deliver both medical and daily support, though it's wise to consider all inspection and staffing details before making a choice.
