    Caring, visible staff; excellent care

I moved my mom to Greene and have been very pleased - the staff are genuinely caring, friendly, and visible (nurses, aides, front office, maintenance, housekeeping and activities). Excellent PT, delicious meals, a home-like, peaceful atmosphere, and strong leadership from Colleen and Kari (with attentive providers like Randy and Dr. Shipley) gave us real peace of mind and compassionate end-of-life care. Communication was clear, laundry and rooms were well kept, and the team treated us like family - I recommend them.

    Summary of reviews

Greensburg Care Center elicits strongly mixed impressions. Many families and residents praise the clinical and support teams for compassionate, attentive bedside care, visible leadership, and accessible medical providers. Physical therapy and rehabilitation services are highlighted repeatedly as effective for short-term recovery, and social services and activities staff are credited with keeping residents engaged and implementing care plans quickly. Dining and housekeeping receive frequent positive remarks: meals described as palatable and adaptable to preferences, housekeeping and maintenance noted as supportive, and admissions orientation (Your Path) framed as informative and reassuring.

At the same time, a substantive subset of reviews describe operational and safety concerns that affect confidence in day-to-day care. Common themes include inconsistent staffing levels and delays in responsiveness (for example, slow call-bell response times), lapses in family communication, and uneven cleanliness or sanitation practices across units. Several accounts point to problems managing resident belongings and inventory control, including missing or mixed items, which has eroded trust for some families. There are also concerns about medication-management transparency and documentation, and a small number of serious allegations regarding staff handling of property.

Infection-control and visitation policies appear inconsistent in their implementation: some families found testing and restrictions disruptive or unevenly applied, and there are reports of conflicting signage and check-in practices. A few reviews describe perceived safety lapses during transfers and transports and at least one instance of a resident requiring emergency care after discharge, underscoring the importance of clear transfer protocols and discharge planning that align with family concerns.

Leadership and administrative staff receive both praise and critique. Several reviewers explicitly commend named leaders and administrative responsiveness, noting visible involvement and coordinated care plans. Conversely, others describe front-desk interactions or communication breakdowns that were dismissive or unclear. This contrast suggests strengths in governance and team cohesion in many areas alongside opportunities to standardize frontline procedures and family outreach.

For prospective residents and families: the facility demonstrates clear strengths in clinical skill, rehabilitation, activities, and an environment that many find home-like and comforting. However, weigh those positives against the pattern-level operational issues some families experienced — staffing consistency, responsiveness, belongings management, infection-control practice uniformity, and medication transparency. Visiting in person, meeting leadership and the care team, reviewing transfer and discharge protocols, and asking about inventory controls for personal items and medication documentation would help assess whether the facility’s current practices match your expectations and needs.

    About Greensburg Care Center

Greensburg Care Center is a skilled nursing and rehab community that sits on nearly ten acres over on Tollgate Hill Road in Greensburg, Pennsylvania, and whether someone needs short-term rehab or longer-term nursing care, they try to meet a wide range of needs. The place has 120 certified beds in the main Skilled Nursing Center and a separate 14-bed Center for the Memory Impaired, and you'll find walking areas and two courtyards where residents can get outside a bit, which does help with daily life. Saber Healthcare Group's managed the facility since July 2023, with Saber Governance, LLC and Dolores Nasri giving leadership, and they take care of residents by providing about 3.49 nurse hours per patient per day, though the nurse turnover rate has been 48.7% and there's been a total of 58 deficiencies from inspection reports, with 3 infection-related deficiencies and some problems in pharmaceutical services and keeping medical records secure, so that's something to keep in mind. The staff works alongside physicians to create individual care plans, covering everything from physical, speech, and occupational therapy, to cardiac care, IV therapy, pain management, pulmonary care, stroke and neurological care, and even wound care, plus they offer hospice care and respite stays for those who need it.

The community has programs such as the Progressive Approach to Home (PATH) which helps people move back home after a hospital stay, and a Pre-registration Program so folks can plan their rehab even before surgery, trying to make transitions smoother. There's a Memory Care Neighborhood with a focus on safety, structure, appropriate stimulation, and lowering anxiety for residents with dementia or memory issues, and the restorative program works to help people keep up habits like walking or social dining, which is good for both body and mind. The center provides 24-hour nursing coverage, assisted living and personal care, and tries to keep a home-like setting with seasonal activities, special meals, a social services program, and a full calendar of engagement.

There are programs for families and friends who need support, with both outdoor and indoor spaces that try to feel comfortable and private, and they make sure to follow federal civil rights standards. Management includes James Woodley for employee relations, Sue Fox as the community liaison, and Geri Yacobucci as the main contact, working to answer questions and help with tours. Even though there have been deficiencies, the staff does aim for quality services and compassion, and residents and their families often mention a nurturing and supportive environment, especially with the many therapy services, medical care, and activities available. Greensburg Care Center works to provide a peaceful place for recovery, long-term living, and memory support, with an emphasis on dignity, comfort, and professional medical care, but like anywhere, it's best to visit and ask about all the details before making decisions.
