    Caring staff and excellent rehabilitation

I'm very pleased with this facility - the staff are caring, attentive and professional, rehab/PT/OT were thorough, and the clean, bright spaces with engaging activities and an outdoor dining deck made my loved one feel at home quickly; admission was smooth and administration responsive, so I highly recommend.

    Summary of reviews

Complete Care at Berkshire elicits a mixed set of impressions. Many families praise individual caregivers, nurses, and therapists for compassion, attentiveness, and effective rehabilitation; reception and social-work staff are frequently described as helpful and reassuring, and some long-term residents and their families report stable, positive experiences. The facility offers an activities program and outdoor dining/deck that families find appealing, and parts of the building are described as clean and well-maintained.

At the same time, a consistent pattern emerges around variability in operational performance. Staffing appears uneven: reviewers describe high turnover, understaffing at times, and inconsistent responsiveness to call lights and care requests. These operational gaps are associated with concerns about hygiene and incontinence-care consistency, delays in assistance for transfers and mobility, and lapses in clinical oversight—several accounts reference delayed recognition of infection or other clinical deterioration that required escalation to higher-level care.

Dining and ancillary services are another area of mixed performance. While some residents experienced appropriate therapeutic and appetizing meals, others reported inconsistent meal temperature, presentation, and adherence to dietary preferences. Laundry and personal-belongings management also show recurring problems: missing clothing, lost personal items, and delays or errors in laundering were described by multiple families. Cleanliness and odor control receive praise in some areas and criticism in others, suggesting uneven housekeeping or maintenance coverage.

Communications and management processes present further concerns. Families report gaps in timely updates from nursing leadership, variable responsiveness from supervisors and administration, and occasional billing or discharge coordination problems. COVID-era visitation policies and quarantine procedures were noted as limiting family contact in some stays; reviewers recommended confirming current visitation and infection-control practices during a visit.

For prospective residents and families: the facility demonstrates clear strengths in individual caregiver dedication and therapy services, but operational variability can affect day-to-day experience. During a tour or pre-admission conversation, ask for current staffing ratios by shift, observe call-response times, review infection-control and laundry/belongings policies, ask to meet therapy staff, and request examples of recent clinical-incident communication and discharge coordination. Those steps can help clarify whether the facility’s strengths align with the candidate’s care priorities and risk tolerance.

    About Complete Care at Berkshire

Complete Care at Berkshire is a for-profit skilled nursing facility located at 5501 Perkiomen Ave in Reading, PA, and it's part of the Complete Care network, though it operates independently and is licensed to use the Complete Care name. The facility has 130 certified beds and usually has about 116 residents each day, with staff providing 3.60 nurse hours per resident daily, but staff turnover is fairly high, with about half of the nurses leaving each year. English is commonly spoken among staff, and clinic staff may know other languages. Folks there get a clean environment with healthy air, modern rooms, and plenty of space around the building, plus food that's known to be good.

Long-term care, assisted living, and memory care services are available, including round-the-clock nursing, and the team's known for being accommodating and respectful, especially in the memory care unit which provides support for people living with Alzheimer's or other dementia types, using personalized plans and secure areas for care. There's a state-of-the-art therapy gym and specialized programs for orthopedic rehabilitation, cardiac care, stroke recovery, and wound care, plus in-house dialysis, subacute rehab, and innovative therapies tailored to each resident. The staff work together to meet individual needs and help residents work toward their health and recovery goals, and the nursing and therapy teams include staff nurses and a doctor on call when needed.

Residents can take advantage of daily living support, shared spaces, full-time staff assistance, and an on-site beauty salon and barbershop. There's support for both male-only and female-only living options as well as pet allowances, and folks gather for activities, both onsite and offsite, with programs for recreation, mental stimulation, and education. Specialty care in respiratory therapy and pulmonary care is available for people with breathing challenges like COPD. Hospice care and respite care can be arranged, and people get support with discharge planning and case management when it's time to move on.

The setting aims to feel home-like and friendly, with a receptionist who's helpful at the front desk, staff who pay attention to cleanliness and detail, and frequent leadership visits to build trust. People living at Complete Care at Berkshire get personalized treatment plans and the support they need to feel as independent as possible, but the facility isn't currently accepting new patients, so you'd need to check in about when spots might open up.
