    Clean, compassionate care; excellent rehab

I had a very positive experience at this home-like, clean and odor-free facility - admissions were smooth and my loved one stayed comfortably in a private, spacious room. The nursing staff, CNAs and administration were attentive, compassionate and professional, and the PT/OT team was effective at meeting mobility and daily-living goals; meals, housekeeping and activities were all helpful. I'm grateful for the supportive, well-organized care team and would confidently recommend this place for short-term rehab or long-term care.

    Summary of reviews

Reviewers describe a facility with clear strengths in rehabilitation and therapy services alongside persistent operational and staffing challenges. The rehabilitation program, particularly physical and occupational therapy, is frequently highlighted as a major asset: therapists are described as skilled, goal-oriented, and effective at progressing mobility and daily-living goals. Organized therapy spaces, in-room therapy options, and a generally accommodating rehab workflow make the short-term post-acute program a good fit for many recovering patients.

Clinical nursing and direct-care staff receive mixed but often positive assessments. Many families praised attentive nurses and CNAs, timely medication administration, and compassionate bedside care; several accounts single out individual staff for exceptional service and note the value of social workers and specific admissions staff in smoothing transitions. At the same time, there are recurrent reports of inconsistent responsiveness to call lights, delays assisting with toileting or transfers, and variability in night-shift conduct and privacy practices. These patterns suggest that care quality may fluctuate by unit and shift.

Dining and basic care services are areas of notable variability. Some reviewers described appealing meals and adequate housekeeping, while others raised concerns about meal quality, limited heart-healthy or alternate-diet accommodations, and weekend meal-service gaps. Sanitation and incontinence-care concerns were reported for particular units or stays; these accounts point to uneven cleanliness standards across the facility rather than a universal condition. Infection-control practices were also described as inconsistent in some reports, including breakthrough infections and related operational disruptions.

Management and operations present a mixed picture. Several families complimented admissions leadership and specific administrative staff for being communicative and supportive. Conversely, other reviewers described strained interactions with administration, questions about billing (bed-hold charges), and operational lapses such as transportation refusals or mishandled personal belongings. Reliance on agency or contract staff and high staff turnover are recurring themes that appear linked to variability in daily-care consistency and resident experience.

Notable patterns for prospective residents and families: the facility appears well suited for goal-driven short-term rehab stays where therapy intensity and facility resources are a priority. Long-term care experiences seem more variable, with outcomes heavily influenced by staffing stability and the particular unit or shift. When evaluating placement, families should assess current staffing levels, observe mealtime offerings and diet flexibility, ask about infection-control protocols and night-shift practices, and confirm logistical policies (transportation, bed-hold billing, personal property procedures). Overall, the facility demonstrates clear strengths in rehabilitation and several high-performing staff members, but consistent oversight of staffing, sanitation, dietary accommodations, and administrative communication would be necessary to address the recurring operational concerns described by reviewers.

About Lakeside Nursing and Rehabilitation Center

Lakeside Nursing and Rehabilitation Center sits in North Kingstown, Rhode Island, in a single-story building with easy wheelchair access and customer parking out front, and you'll find it close to all the local hospitals, which is something families usually want to know because that way residents can get quick help if needed. With a 120-bed capacity, this skilled nursing facility provides care for older adults needing long-term support, and folks can pick from different room layouts, including private rooms, so they can feel comfortable. Care teams there offer skilled nursing and intermediate care for very frail people who need a lot of help, plus they provide short-term rehabilitation for those working their way back home after a hospital stay, with therapists on hand for physical, occupational, or speech therapy, and a state-of-the-art rehab gym where recovery work happens. They have assisted living and independent living options, besides home care, and for people living with Alzheimer's or dementia there's a memory care program with safety features and routines tailored to each person's needs, so families with loved ones who need that support will see there's focus on personal attention. The community also lets folks stay temporarily through respite care or move in long-term, and they welcome hospice residents as well. Care is well-rounded, so nurses and aides, along with visiting specialists like dentists and podiatrists, work together and keep different kinds of special care in mind-including mental health services-while a dedicated activities director organizes social and recreational programs to help people stay active. They accept credit cards for onsite payments, which can be handy for some families. Emergency care is available if required, and access to hospital specialists is close by, covering nearly every medical need. The nursing home itself gets a 1-star rating out of 5 from the Centers for Medicare & Medicaid Services, though it has a 6.9 out of 10 average resident rating, making it the third highest rated in the city. Lakeside is privately operated, has 11-50 staff members, and offers a sensitive and nurturing environment without a lot of fuss, just focused care for seniors who need different levels of help.
