    Spotless facility, caring staff, excellent

I'm very pleased with this senior living facility - spotless, beautifully kept grounds, welcoming and genuinely caring staff (nurses, CNAs, PT/OT, admissions, activities), excellent meals, and engaging programs that kept my mom happy and involved. Her transition was smooth and she was treated like family; a few small improvements would make it exceptional, but I'd confidently recommend it.

    Summary of reviews

Overall impression: Westerly Rehabilitation & Healthcare Center elicits strongly mixed feedback. Many families and visitors describe consistently positive experiences — notably compassionate nursing and CNA care, strong rehabilitation outcomes, an active activities program, and a clean, recently updated physical environment. At the same time, a cluster of serious operational concerns appears in a subset of accounts; these concerns concentrate on clinical oversight, unit-level supervision, medication handling, and administrative responsiveness.

Care and staff: A recurring strength is the direct care team. Nurses, CNAs, therapists, and social services receive frequent praise for attentiveness, professionalism, and patient-focused rehabilitation that supports safe returns home. Admissions and rehab intake processes are described as welcoming and well organized. However, several reviewers raised clinical-safety issues including avoidable hospital transfers, infections, and traumatic injuries; these reports point to inconsistent clinical oversight, variable incident-response processes, and occasional lapses in treatment timing. Families should clarify supervision ratios, fall-prevention protocols, medication administration safeguards, and communication expectations before placement.

Dining, activities, and facilities: The facility scores well for its activities program and social offerings — residents are described as engaged through outings, holiday events, bingo, and specialized groups. Meals are often praised for quality and portioning, with some menu items singled out positively, though other comments describe variable food satisfaction. Physical plant feedback is generally favorable: renovated rooms, tidy common areas, and attractive grounds are noted repeatedly. Nonetheless, there are isolated sanitation and incontinence-care concerns, and at least one comment referenced pest-control issues; prospective families may wish to tour the unit(s) where the resident will live and inquire about housekeeping and pest-control schedules.

Management and operations: Human-resources practices and employee development are highlighted as strengths (sign-on incentives, continuing education, and recognition programs). Several staff-centered comments describe a supportive workplace that can benefit resident care. Conversely, a number of families report administrative unresponsiveness, inconsistent family communication, and personnel-culture issues (favoritism/nepotism and cliques) that can undermine trust. Operational deficiencies were also described around security systems (door codes and alarms), room/property handling after transitions or deaths, and prescription-control processes — including serious allegations of medication loss. These are procedural concerns that should be clarified with leadership during a tour.

Bottom line: Westerly demonstrates clear strengths in hands-on care, rehabilitation, activities, and physical upkeep, but the facility also shows operational variability that has had significant consequences for some residents. Prospective residents and their families would be well served by an in-person visit, targeted questions about supervision and medication safeguards, review of incident-communication procedures, and confirmation of documentation and consent practices before placement.

About Westerly Rehabilitation & Healthcare Center

Westerly Rehabilitation & Healthcare Center sits in a scenic part of Westerly, Rhode Island, and serves older adults who need special care, with 106 beds available for different levels of support and attention, and you'll see it's a place where the staff really keep safety in mind, with secure areas that stop residents from wandering and a team of certified nurses, licensed medical staff, qualified nursing assistants, and physical therapists ready to help, day or night, with personal care, medication, and mobility. For people who need constant monitoring, especially after hospital stays or who have medical disabilities, the center provides a safe and controlled setting and uses its programs like Rehabbing Care™, Care Navigation, and Urgent SNF™ to try to make recovery and daily life as smooth as possible, and if someone needs short-term relief, the center also has respite care. Folks with memory loss can find a secure and supportive space in the dedicated Alzheimer's care unit, with staff who know how to work with memory impairment. The nursing home part of the facility gives around-the-clock help with daily activities, and the team supports daily living needs, from meals to dressing, always focusing on each person's unique requirements. The rehabilitation program addresses many issues, including problems from orthopedic surgery, strokes, cardiac or lung conditions, and aims to restore independence and help each resident reach their best level of function, and they call this approach "holistic" and "person-centered," because they try to look after the physical, mental, and emotional needs of everybody there. Therapy services cover physical, occupational, and speech needs, with specialized programs like OrthoWIN Rehab for orthopedic recovery. If a person requires post-acute care, Westerly Rehabilitation & Healthcare Center follows a progressive way of supporting patients and families, helping them adjust from hospital to home, using a mix of clinical services, subacute rehab, long-term care, and programs designed to promote both comfort and recovery. Safety and security stay uppermost, with features and routines meant to give families peace of mind. The facility comes with transportation help when needed and provides resources about its own programs and care models, including Care Navigation and Urgent SNF™, and MarquisHealth Consulting Services oversees the approach, always trying to offer personalized attention, compassion, and guidance at every step. The environment has a warm and homey feeling, and while it stays focused on rehabilitation and healthcare, it also tries to make every day feel comfortable for the people who call it home.
