    Consistently caring, professional, responsive staff

I placed my loved one here and am grateful for the consistently caring, professional, and responsive staff - nurses, therapists, aides, housekeeping and maintenance provided attentive 24/7 medical and rehab care in a clean, safe, home-like setting. Management stayed engaged, issues were handled promptly, the food and atmosphere kept residents happy. I highly recommend this facility; it's excellent overall with only minor room for improvement.

    Summary of reviews

Gallaway Health and Rehab elicits a mixed picture: many accounts emphasize compassionate, skilled bedside care and effective rehabilitation, while others raise significant operational and clinical concerns. Positive comments center on staff who are described as caring, attentive, and accessible; strong therapy programs that support good short-term rehabilitation outcomes; and a home-like, social atmosphere with activities and on-site spiritual services. Several families specifically praised admissions staff, individual nurses, CNAs, and therapists for being professional, communicative, and available around the clock.

Clinical quality appears uneven. On the positive side, reviewers described medically skilled nurses, timely therapy, and cases in which staff provided life-saving or highly effective care. Conversely, a number of accounts describe breakdowns in medication administration, inconsistent documentation, and delays in clinical assessment or escalation for emergent needs. Those operational gaps are associated in some reports with subsequent hospitalizations and concerns following resident deaths; reviewers also referenced public oversight actions and a low Medicare rating as context for their worries. Infection-control practices and vaccination handling were described inconsistently across accounts.

Staffing and culture are similarly mixed. Many reviewers report a family-like atmosphere, respectful interactions, and staff who go above and beyond. At the same time, other reviewers described communication challenges, rudeness from certain office or payroll staff, and variability in social-work responsiveness. High administrative turnover and episodes of interim leadership were recurrent themes, and they contributed to perceptions of management instability and inconsistent follow-through on corrective actions.

Facility and operational issues also showed patterning. Positive notes include generally well-kept common areas, enjoyable dining experiences for some visitors, and active recreational programming that contributes to resident well-being. Critical accounts identify sanitation and pest-control concerns in resident rooms, unreliable laundry and personal-property processes, and limitations in the rehabilitation space and equipment. Families mentioned difficulties reaching staff by phone and restrictive or poorly communicated visitation procedures during certain periods.

For prospective residents and families: there are clear strengths to considerโ€”compassionate direct-care staff, effective therapy services, and a social, home-like environment in many units. At the same time, the facility exhibits operational variability in medication safety, clinical escalation, documentation, infection control, and management stability. When evaluating this facility, visitors should request current information on medication-administration protocols, staffing ratios and turnover, emergency-response procedures, infection-prevention policies, laundry/property handling, and recent regulatory or quality-reporting details. An in-person tour, direct conversations with frontline clinical staff and the director of nursing, and review of up-to-date public reporting (e.g., Medicare/Nursing Home Compare) will help clarify whether the facilityโ€™s strengths align with a specific residentโ€™s care needs.

    About Gallaway Health and Rehab

Gallaway Health and Rehab has 104 certified beds and an average of 88 residents daily, and it's a nursing home facility with assisted living, memory care, and skilled nursing care. The building has private and semi-private rooms, each with an emergency call system and private bathrooms in some units, and there are open social areas, a dining room, a residents' lounge, outdoor living spaces, WiFi, and a hair salon and barber area, so folks have different places to relax or spend time together. The staff offers help with daily activities, medication management, and personal care, and they run housekeeping and laundry services, too. Specialized medical services include wound care, IV therapy, trach care, PEG tubes, CPAP and BiPAP support, wound vacs, and complex condition management like cardiac rehab, pulmonary rehab, hemodialysis, stroke care, and sepsis care, so residents with more complex needs can still get care on site. Rehabilitation programs are handled by physical, occupational, and speech therapists, using advanced equipment for people who need to rebuild strength or function after illness or surgery, and short-stay and transitional care are available for conditions like fractures or hospital recovery.

Gallaway Health and Rehab provides respite care, comfort care, and palliative care for people who need short-term or end-of-life support, and home healthcare and caregiving services are listed among their long-term care options. Residents can join in recreational activities, entertainment events, light physical exercise, and educational health programs, and the place also has a computer center and community-run transportation. Staff helps manage medications, pain, and chronic conditions around the clock, and there are options if someone needs pharmacy or restorative nursing services. Average nurse staffing is about 3.17 hours per resident per day, and the reported nurse turnover rate stands at 66.7%.

The community is managed by Gbd LLC and Mission Health of Georgia, LLC, both since January 2015, and it operates as a for-profit limited liability company affiliated with Mission Health Communities. Reports show past deficiencies, including infection control failures and issues with quality assurance, which resulted in a large fine and payment suspension, and there hasn't been a standard inspection in the last two years. Gallaway Health and Rehab has a 4.4 rating from 58 online reviews, and residents get 24-hour care, access to a range of rehabilitation therapies, and daily support services, but it's important to know the facility's history with inspection problems if considering it for care.
