    Attentive, clean, home-like rehab experience

I'm very pleased with the attentive, professional, and friendly staff - they personalize care, keep my family informed, and welcomed visitors. I've had rehab here twice and found the clean, home-like facility, pleasant dining, many activities, and beautiful garden much better than other area nursing homes. Overall comfortable and well run; a few small improvements would make it perfect.

    Summary of reviews

Reviews for Concordia Nursing and Rehabilitation – Northhaven show a mix of clearly positive experiences and notable operational concerns. Positive descriptions emphasize friendly, attentive staff members, effective short-term rehabilitation outcomes, an active activities program, family involvement, and well-maintained common areas and dining rooms. Several families specifically cited personalized attention to resident preferences, timely communication during admissions and care transitions, and the presence of an appealing outdoor garden that supports family visits and outdoor time.

Care quality appears variable. Many accounts describe competent, attentive nursing and therapy staff who support successful rehab stays and individualized care plans. Conversely, a number of accounts document delays in assistance, inconsistent personal-care routines (including missed or delayed bathing, turning, and toileting assistance), and examples of pressure-injury concerns. There are also repeated statements indicating lapses in medical-equipment management — for example, issues with oxygen supply, respiratory-device maintenance, and timely attention to device-related needs. These patterns suggest uneven adherence to basic clinical and safety protocols in some shifts or units.

Staff and communication present a mixed picture. Some reviewers praised specific caregivers by name and described organized, welcoming teams that kept families informed. Other accounts raise concerns about staff professionalism, tone in family interactions, and responsiveness to inquiries; examples include abrupt phone interactions and perceived dismissiveness. There are also references to privacy and documentation process gaps, including a HIPAA/privacy concern and instances described as improper handling of advance-directive paperwork; in one combined concern reviewers used the term "allegations" regarding financial and advance-directive matters. These are serious family-level concerns that warrant direct clarification from management.

Dining and daily living services are similarly inconsistent. Several residents and families reported enjoyable meals and a comfortable dining environment, while others described unpalatable food textures, incorrect trays, and interrupted dining-room access when staffing was limited. Facility condition reports are mixed as well: common areas and some rooms are described as clean and comfortable, but other comments mention outdated rooms, limited storage space, cold temperatures, sticky floors, and pest-control concerns. These differences suggest variability between units or over time.

Management and operational issues recur in the feedback. Reviewers noted perceived staffing shortages, policy changes associated with ownership or branding, and variable follow-through on therapy schedules and care plans. Security and property-control weaknesses are another recurring theme, with several families reporting missing personal items or concerns about storage and device security. At the same time, some families expressed overall trust in the facility and satisfaction with placement, indicating that quality is uneven rather than uniformly poor.

For prospective residents and families: arrange an in-person tour across different units and meal times; ask about current staffing ratios and how they manage peak demands; review infection-control, pest-control, and equipment-maintenance protocols; confirm processes for advance directives, documentation, and family communication; and request recent inspection or quality metrics if available. These steps will help identify whether the areas of concern reflected in some reviews have been addressed and whether the facility’s strengths align with your priorities.

About Concordia Nursing and Rehabilitation – Northhaven

Concordia Nursing and Rehabilitation - Northhaven Under New Management sits in Knoxville, Tennessee and is a skilled nursing and rehabilitation facility now led by a new management team. This place takes in people using Medicaid and Medicare, and it's run as a for-profit facility by a Limited Liability Company. It's not a Continuing Care Retirement Community, but it still has a full range of care and support services, helping folks after things like surgery, illness, or a hospital stay with 12-16 hour nursing care, a 24-hour call system, and staff ready to help with daily living tasks like bathing, dressing, transfers, medication management, and more. The community also has special attention for memory care, as there are 49 memory care units and programs for seniors living with dementia or Alzheimer's, plus amenities like private rooms, private bathrooms, kitchenettes, cable TV, air conditioning, and high-speed Wi-Fi. Residents can take part in community events, resident-run activities, and scheduled programs covering music, arts, religious services, and movie nights, and you see people using the business room, fitness room, library, outdoor walking paths, and gardens pretty often. Meals are served restaurant-style in a shared dining room with all-day options, and staff make sure to handle special needs like food allergies or diabetes. There are safety features like an emergency alert system in rooms, and housekeeping covers laundry and even dry cleaning, which can be a relief. Caregivers here keep a close eye, and there's a higher staff-to-resident ratio in the memory care unit. Other parts of the facility include a spa, wellness room, enclosed courtyards, and arts rooms, so there's always something to do, and there are programs for transportation, outings, and assistance getting to medical appointments or therapy sessions. The staff are trained to help with Alzheimer's, and the community tries to keep life active and structured with activities, support, and supervision for those who need it. Concordia Nursing and Rehabilitation - Northhaven Under New Management is one of the many nursing facilities serving the Knoxville area, offering basic comforts and reliable care for seniors needing short-term rehabilitation or longer-term support.
