    Attentive therapy in welcoming community

I stayed short-term for rehab after a fall and was very pleased. The nurses, CNAs and therapists (Stephanie and team) were attentive, professional, and kept my family informed while helping me regain independence. The community felt clean, home-like and welcoming with activities and weekly church - I'd recommend it for therapy and respite care.

    Summary of reviews

Overall impression: Reviews describe a facility with many physical and programmatic strengths alongside notable operational inconsistencies. The building and common areas are frequently described as recently updated, clean, and well maintained; many families appreciated the neat dining area, routine room checks, and a generally home-like environment. The therapy department and short-term rehab services receive consistent praise for helping residents regain mobility and independence, and several reviewers singled out therapists and specific clinicians as particularly effective.

Care and staff: Staff performance is mixed across accounts. Numerous reviews highlight respectful, caring, and professional interactions from nurses, CNAs, and therapy staff; readers commonly cite attentive caregiving, good communication from certain staff members, and clinicians who go beyond expectations for families. At the same time, other reviews describe operational weaknesses that directly affect care delivery: delays in answering call lights, missed medications, rushed dressing or bathing, and inconsistent personal-care scheduling. These items suggest variable staffing levels and uneven competence among frontline staff โ€” strengths appear concentrated in therapy and some nursing personnel, while day-to-day personal care and responsiveness can be inconsistent.

Dining and nutrition: The dining area and presentation are frequently noted as neat and orderly. However, several accounts point to interruptions in meal service, delayed lunches, and concerns about diet provision. These indicate uneven meal-service continuity rather than a uniformly managed dining program.

Activities and rehabilitation: Activity offerings and spiritual programming (including weekly church services) are positive features for social engagement. Therapy and rehabilitation are among the facility's more consistent strengths; reviewers repeatedly report effective mobility therapy, skilled therapists, and successful short-term rehab outcomes that helped residents return home.

Facilities and safety: The physical environment is often praised for being clean and freshly updated. Conversely, there are operational maintenance and safety concerns: one example cited a malfunctioning door lock posing an exit-safety risk, and other notes raise pest-control concerns. These items point to gaps in building maintenance and environmental services rather than fundamental design issues.

Management and communication: Family experience with leadership and administration is mixed. Some families report responsive, informative staff and managers who keep them apprised; others describe unresponsive management, poor communication about incidents or care changes, and limited follow-through on complaints. Together with staffing variability, these management issues appear to contribute to inconsistent care continuity, particularly for residents with complex needs.

Notable patterns and guidance: The reviews cluster into two broad patterns โ€” clear strengths around facility appearance, therapy/rehab services, and several compassionate staff members, and recurring operational weaknesses related to staffing consistency, responsiveness, medication processes, meal continuity, pest control, and maintenance. Prospective residents and family members would benefit from asking targeted questions about staffing ratios, medication-administration checks, bathing schedules, meal-service continuity, pest-control protocols, therapy outcome metrics, and how management communicates incidents or care-plan changes. These topics can help clarify whether the facilityโ€™s strong therapy and environmental features are paired with reliable day-to-day care practices for a particular residentโ€™s needs.

    About Diversicare of Martin

Diversicare of Martin sits at 158 Mount Pelia Rd in Martin, Tennessee, and cares for people with a mix of health needs, since the place covers short-term rehabilitation, long-term care, skilled nursing, assisted living, memory care, and hospice care, so folks with complex medical issues or specialized needs, like those dealing with memory loss, can get what they need right there, and the staff includes English speakers along with people who know other languages, which helps folks feel understood, and since the care team handles both typical aging health problems and mental health or substance use challenges, you get a place that brings together a lot of knowledge under one roof. People who need therapy after an illness or surgery can go for short stays to work on recovery, and for anybody who can't go home safely, long-term care is right there, and some stay in the assisted living side where there's support for daily tasks but still a push for independence, since the staff tries hard to help people keep their dignity and relearn lost skills or pick up new ones for daily living. The whole goal is to lift spirits, give people hope, and encourage confidence, treating every resident as an individual with a personal story to be honored. CNA training happens onsite, which helps new caregivers learn what to do and keeps the staff freshly trained, and the facility keeps a comprehensive team for high-quality care, but right now they're not taking new patients. Amenities and features aim for comfort and make daily life a little easier or more pleasant, so people can feel welcome and supported every day, even though hours and some info aren't currently clear, and while there are connected services like Facebook access, the main thing is a safe and caring environment where medical needs, daily support, and personal stories matter.
