    Warm caring community, excellent rehab

I'm very pleased with this facility - the staff and nurses are caring, communicative, and treat residents like family. My loved one enjoyed a private, homelike room, plentiful activities (concerts, events, walks) and excellent rehab/therapy. The building is clean and welcoming, the food is good, and leadership often goes above and beyond. Overall a warm, well-run community I'd recommend.

    Summary of reviews

Reviewers portray Signature HealthCARE of Monteagle as a facility with strong strengths in person-centered caregiving and rehabilitative services, alongside several operational weaknesses families should evaluate. Praise centers on a consistent theme of caring staff: CNAs, nurses, therapists, and front-office personnel are frequently described as compassionate, attentive, and willing to go beyond routine duties. Therapy and rehabilitation are repeatedly highlighted as a major asset; renovated therapy spaces and a patient-first therapy model were noted to support successful transitions to lower levels of care.

Clinical care impressions are generally positive with many comments about engaged nurses who communicate with families and an attentive direct-care workforce. At the same time, there are concerns about staff conduct and boundary management in isolated instances and indications of uneven accountability and team coordination that can affect efficiency. A serious allegation involving a resident's death was raised in one review and accompanied by complaints about delays in providing medical records; this highlights the importance of clarifying the facility's documentation and compliance practices during intake. Families should ask specific questions about incident reporting, record-request timelines, and disciplinary processes.

Dining and daily living receive mixed feedback. Several families appreciated a homelike dining room redesign and staff willingness to accommodate extra food items, birthday celebrations, and personal comforts. Conversely, some reviewers described inconsistent food quality and basic-room provisioning that felt minimal in certain areas. There are also sanitation- and odor-related concerns reported in some resident rooms, and at least one account referenced missing personal items — both operational areas prospective families may want to probe (cleaning schedules, laundry handling, personal-item safeguards, and pest-control or odor-management protocols).

The facility's culture and activities are frequently cited as strengths. Reviewers describe an active program of concerts and evening events, a family-style atmosphere, regular resident engagement, and visible staff morale — many staff observers described the center as a good place to work. Leadership visibility is another recurring positive: a new administrator and director of nursing were credited with organizing teams, improving appearance and flow (clear hallways, renovated spaces), and raising staff engagement. Still, some comments reference variability in communication tone and occasional rudeness; families may want to observe interactions and ask about customer-service expectations for staff.

In summary, Monteagle appears to offer strong rehabilitative care, a compassionate workforce, and an improving physical environment under engaged leadership. Prospective residents and families should balance those strengths against operational issues flagged in reviews: inconsistent meal quality, localized sanitation and personal-item security concerns, variability in room furnishings, and questions around record responsiveness and staff accountability. Recommended due diligence includes touring multiple resident rooms, observing a mealtime and activities period, asking about complaint and record-request procedures, and clarifying how the facility handles staffing coverage and personal belongings.

    About Signature HealthCARE of Monteagle

Signature HealthCARE of Monteagle Rehab & Wellness Center sits at 26 2nd Street in Monteagle, Tennessee, and has room for up to 150 residents, offering skilled nursing, assisted living, independent living, memory care, and rehabilitation all under one roof, and you'll find studio room layouts and options for furnished or private rooms that include cable TV, air conditioning, Wi-Fi, kitchenettes, and private bathrooms, with some rooms for more advanced needs and others for longer stays, while there's always help with things like bathing, dressing, medication, and moving around for those who need it. The nursing staff gives direct care for 12 to 16 hours a day, but a 24-hour call system lets someone get help day or night, and the team provides both short-term rehab and long-term care, including special support for residents with memory loss or trouble moving.

Staff at the center speak English and focus on providing care in a nurturing way, and the setting encourages recovery and health, with a wellness program built right into the daily routine, and residents can access physical therapy, occupational therapy, speech therapy, and respiratory therapy as needed, getting care that fits each person's plan. Meals come from a professional chef and you can expect a dining room with restaurant-style service, all-day menus, options for allergies or diabetes, and even special diets if needed, so everyone can find something to eat safely and in good company.

The building itself has activity rooms, a library, a game room, an arts room, a fitness center, spa and wellness rooms, and even space for outdoor activity with walking paths, a garden, and green areas where residents can spend time. There's a schedule of events with games, arts, music, movie nights, and activities run by both staff and residents themselves, plus family support and community-sponsored gatherings, which seems to help keep things lively, and the staff offers help with housekeeping, laundry, and transportation, making life easier for folks living there. Safety gets some attention, too, as the center has emergency alert systems and a secure environment, while care navigation teams help coordinate a resident's needs and family support.

Residents use Medicare or Medicaid for payment, and the center also offers counseling to help families understand their options, and the facility gets a 3-star rating from the Centers for Medicare & Medicaid Services and a community rating of 9.8 out of 10, which stands out in the area, though no place is perfect. Signature HealthCARE of Monteagle focuses on dignity and a resident-centered approach, providing programs meant to help improve life for people of different backgrounds and needs, and they try to include spiritual and emotional care for those who want it. There's a memory care unit for people with memory or cognitive problems, and extra attention gets paid to those with trouble moving around, so they don't feel left out or put at risk. The center's part of the larger Signature HealthCARE network, promising a level of consistency and access to other services as needed, and staff try to keep the atmosphere welcoming and supportive, designing most programs to fit a range of cultural backgrounds and lifestyles. There are also special features like a family messaging system, move-in help, and discounts or coupons for some new members, aiming to make the move as smooth as possible for residents and their families, and while every resident's experience is unique, overall the center works to combine healthcare, daily care, recovery opportunities, and community life in one place.
