    Attentive staff, clean welcoming environment

I'm very pleased with this facility - the staff are consistently kind, helpful and professional, and my mom is clearly well cared for in a clean, welcoming environment. Nurses, CNAs and therapy teams (shout-outs to Bayley, Priscilla and Maggie) are attentive and communicative, activities and holiday events brighten the days, and the new digital sign-in/security updates make visits easy. I feel grateful for the care and hopeful the new ownership keeps this dedicated team in place.

    Summary of reviews

Overall impression: Reviews present a mixed but largely favorable picture of care at Selmer Post Acute. Many families emphasize compassionate, attentive caregiving from nursing and nurse-assistant staff, and reviewers repeatedly cite strong rehabilitation services (physical and occupational therapy). The facility is frequently described as clean, well maintained, and welcoming, with accessible therapy spaces, a decorated cafeteria, and seasonal activities that engage residents.

Care and clinical communication: Clinical care is often praised—staff are described as responsive, respectful, and willing to provide condition updates to families. Several reviewers called out excellent follow-through and individualized attention. At the same time, there are recurring operational concerns: staffing levels appear variable at times, producing delays for routine assistance, and some families described inconsistent communication after clinical changes or adverse events. These patterns suggest the facility delivers strong baseline clinical services but may have gaps in surge staffing and standardized post-event family outreach.

Staff and management: Staff interpersonal skills are a consistent strength; many reviews name specific caregivers and praise a positive, friendly culture among nursing, therapy, and front-desk teams. There is appreciation for infection-control practices and hygiene emphasis. Conversely, some reviewers express concern about administrative priorities and one set of reviews raises serious, individual allegations that go beyond routine quality concerns; such claims merit careful, independent review by regulators or facility leadership. New ownership and management transitions were noted, along with requests to retain key personnel who families view as central to good outcomes.

Activities and dining: The facility offers active programming, with examples such as holiday parties, themed cafeteria events, and social refreshment gatherings that residents enjoy. These activities and the decorated common areas contribute to a warm, inviting atmosphere. Dining experiences are presented positively in the context of social events, though there is less detail about everyday meal quality across reviews.

Facilities and logistics: Physical spaces are described as clean, spacious, and well-equipped. Recent investments in digital sign-in and security updates were noted positively, though visitor access practices vary between accounts—some families report easy kiosk check-in while others report entry challenges. Property-management issues also emerge as an operational weakness: at least one family reported missing personal items, suggesting the facility may benefit from clearer protocols around resident belongings and inventory controls.

Notable patterns and recommendations for families: Strengths cluster around person-centered caregiving, rehabilitation services, cleanliness, and an active events program. Recurrent operational weaknesses include inconsistent staffing that can delay assistance, variable family communication after changes in condition, visitor-access variability, and property-management gaps. There are isolated but serious allegations that should be investigated separately. Prospective residents and family members would be well served to tour the facility, ask about current staffing ratios and contingency plans for high-demand periods, review protocols for belongings and incident reporting, confirm visiting procedures, and inquire how the facility communicates clinical changes and investigates serious complaints.

    About Selmer Post Acute

Selmer Post Acute sits over on East Poplar Avenue in Selmer, Tennessee, serving folks who need post-acute nursing care, some short-term help after a hospital stay, or longer-term support when the time comes, and it has Medicare and Medicaid certification, so you know they take part in those programs. The large building features both single-family residence-style and apartment-style accommodations, so there's some choice if a person prefers a cozier spot or something a bit more independent, and each room usually has air-conditioning, cable, a phone, Wi-Fi, private bathrooms, kitchenettes, and furniture already inside, making moving in easier. Licensed nurses stay on duty every hour of the day and night, while certified nursing assistants and skilled staff help with wound care, pain control, medication management, dialysis support, and daily needs like bathing, getting dressed, and transferring from bed to chair, with extra options for help with memory care for people who have Alzheimer's or dementia, meaning constant staff supervision when needed.

The place has a big rehab gym staffed by licensed experts, so if someone needs to regain strength or movement, they can get physical, occupational, speech, or respiratory therapies right there, and social services are around to work with patients and families, helping them plan for their return home or connect with outside help and community resources, while a mental wellness program supports folks' overall health. Meals are available in a restaurant-style dining room and each person's special diet, like for diabetes, can usually be taken into account, and there's a beauty salon, a fitness room, small library, garden and outside spaces, a wellness center, activity rooms, and gaming rooms for hobbies and relaxation. The community covers housekeeping and laundry, and there's a 24-hour emergency call system for added safety.

Daily activities aim to keep people active and included, and supportive services like transportation for medical and non-medical appointments, concierge assistance, and community events help residents stay connected. Selmer Post Acute also offers short-term respite stays for temporary relief and has arrangements in place to adapt services if needs change, making for a flexible environment. Staff and leadership have a strong focus on training and education, running seminars for long-term care professionals, a gerontological nurse training program, and a CNA training path, and the annual healthcare convention keeps the staff current. All in all, Selmer Post Acute provides rehabilitation, skilled nursing, assisted living, and memory care in a setting that tries to be safe, comfortable, and supportive for people who need different levels of care.
