    Kind caregivers, excellent rehab, communication

I had a very positive experience here. The staff were kind and professional-caregivers like LaKenya and Punam kept my mom safe, cheered her up and made her laugh. The rehab team was outstanding: quick recovery, regained mobility with a walker, and clear daily updates. The facility is clean, bright and welcoming, and communication throughout the stay was excellent.

    Summary of reviews

Forum Parkway presents a clear split between its physical environment and its operational performance. Many reviewers praised the facility’s appearance: the building, public areas and resident rooms are frequently described as clean, spacious, and well maintained. The therapy space and equipment are consistently highlighted as strengths, and the physical, occupational and speech therapy teams receive repeated positive comments for producing measurable rehab gains, especially in short-term orthopedic and pulmonary pathways.

Direct caregiving experiences are mixed. Numerous accounts identify individual CNAs, aides and therapists who were attentive, compassionate, and effective; these staff members are often credited with daily improvements and positive morale for residents. At the same time, there is a recurring pattern of inconsistent nursing responsiveness, long waits for basic assistance, and variability in bedside practices. Reviewers describe delays in medication delivery and clinical attention, lapses in incontinence-care routines, and uneven wound or device management. These patterns appear most pronounced during nights, weekends, and shift changes.

Operational and administrative issues emerge as a second major theme. Families reported frequent breakdowns in communication — between nursing, therapy and administration, and between staff and families — resulting in missed updates, incomplete care plans, and poorly coordinated discharges. Several reviewers described discharge transitions that lacked essential equipment or medication continuity, and others cited delayed or absent follow-up communications. There are also repeated concerns about staff professionalism and conduct, including examples that reviewers considered unprofessional; more serious are allegations of missing belongings and medication diversion, which warrant further inquiry.

Dining and housekeeping are inconsistent. Many reviewers appreciated a pleasant dining environment when service was timely, but numerous reports describe cold or poor-quality meals, late tray distribution, and failure to accommodate specific medical diets. Housekeeping performance is similarly variable: some areas and rooms are well maintained while others experienced service gaps or sanitation concerns that families found troubling.

Activities and ancillary services are generally viewed positively, with reports of engaging programming, social events, and an active therapy-driven schedule. However, activity staffing and the ability to reinforce therapy goals can be limited by staffing shortages, which also affects supervision and response times. Management impressions are polarized: some families encountered responsive, helpful administrators, whereas others described uncooperative or dismissive interactions with admissions or social-work personnel.

Overall pattern: Forum Parkway can offer high-quality rehabilitation outcomes and a welcoming environment when therapy and caregiving staff are fully engaged and staffing levels are adequate. The recurring operational weaknesses — staffing variability, medication and discharge-process gaps, communication breakdowns, and inconsistent housekeeping/dining — create risk for residents who are medically complex or require tight clinical oversight. Prospective residents and families should weigh the facility’s strong therapy resources and environment against the potential for inconsistent nursing and administrative performance, and consider asking specific questions about weekend/night staffing, medication controls, discharge processes, and individualized care plans during tours and admissions.

Inspection reports show an A-plus grade, meaning few or no severe problems came up during checks, and the facility's always focused on keeping care quality high, even though it isn't currently BBB accredited. From sub-acute care to full rehabilitation, from respite care to hospice, the team gives flexible support, draws up individualized plans, uses advanced technology, and stays clear in its communication with families. Families and patients get regular updates, and there's even a newsletter to keep everyone in the loop about community happenings.
