    Excellent rehab, attentive staff, spotless

I stayed here after surgery and was very pleased - the staff genuinely goes above and beyond and it felt like family. The on-site rehab with full-time therapists helped me regain independence, nurses were attentive, the building was spotless, and leaders like Nicole were incredibly helpful and responsive; very good overall and I'd recommend it.

    Summary of reviews

This set of reviews presents a mixed but instructive picture of Floresville Nursing Center. Strengths cluster around caregiving and rehabilitation: many accounts describe compassionate, attentive nurses and therapists, long-tenured staff, and strong unit-level leadership. The facility's on-site rehabilitation program and presence of full-time therapists are frequently cited as effective for post-operative recovery and regaining independence. Administrative and office staff are also characterized as helpful and professional, helping with paperwork and transitions. Several reviewers emphasized a home-like environment, cleanliness, engaging activities (including a community garden), and a family-oriented culture that contributes positively to resident experience.

At the same time, recurring operational concerns appear. There are consistent indications of inconsistent clinical oversight and irregular implementation of care plans; a few accounts described instances where therapy was not delivered as expected despite on-site capacity. Staffing shortages and reliance on agency personnel are referenced alongside variable communication from management, producing uneven perceptions of leadership and workplace culture. Administrative systems show weaknesses as well: reviewers pointed to billing and invoicing problems, privacy and personal-property handling lapses (for example, mail and laundry), and the facility's CMS 1-star rating is noted as a regulatory signal to investigate further.

Safety and clinical governance are additional areas to scrutinize. Several reviews raised issues suggesting gaps in safety and incident-response protocols, including an event involving swallowing-risk management and an elopement/wandering response that took an extended period to resolve. There were also comments about variable physician oversight and a lack of consistent dietary oversight in the kitchen. One reviewer referenced criminal charges involving a staff member, which highlights the importance of robust background-screening and staff-conduct safeguards. Taken together, these items suggest the need for strengthened incident prevention, monitoring, and family communication after events.

Facility infrastructure and cost are other patterns that emerged. While many reviewers praised cleanliness and a welcoming environment, the building itself was described as aging and in need of updates. Several reviewers also expressed concern about the facility's private-pay cost relative to the quality and consistency of services, and about billing administration. Prospective families should weigh the strong rehabilitation and caregiving strengths against these operational and governance issues.

Recommendations for prospective residents and families: visit multiple times, ask specifically about care-plan development and adherence, review therapy schedules and staffing models (including agency use), request documentation of recent regulatory reports and corrective actions, inquire about billing and privacy practices, and observe mealtime supervision and mobility/safety protocols. For families already using the facility, maintaining frequent communication with unit leadership and establishing a point person in administration may help address the operational gaps highlighted in these reviews.
