    Renovated facility with caring staff

I toured the recently renovated facility and was very impressed. Amanda gave a great tour and administrator Brandon and the leadership team are caring and professional. Rooms and common areas are spacious, clean, and updated; the large gym, good dining, and many activities (bingo, exercise groups) keep residents engaged. Nursing and rehab staff are attentive with real rehab successes, and the front desk and aides are welcoming. I'd recommend this caring, family-like place for rehab or long-term care and would choose it for myself or a loved one.

    Summary of reviews

Reviews of Arbor Grace Guest Care Center describe a facility with many of the physical and programmatic attributes families often seek: recently renovated spaces, clean and pleasant common areas, a large gym, spacious rooms, and an active calendar of social and therapeutic activities such as bingo, group visits, and exercise classes. Several accounts praise the dining experience and note that staff routinely engage residents in activities and wellness checks. The facility is also frequently characterized as a strong option for short-term rehabilitation and therapy, with multiple positive references to successful rehab outcomes and supportive therapy teams.

Clinical-care perceptions are mixed. Numerous reviewers describe compassionate, attentive nursing and aide staff who provide individualized assistance and good bedside manner. At the same time, other reviewers raised serious clinical and safety concerns, including an allegation of medication mishandling and concerns following a resident's death that prompted a family complaint to state authorities. These accounts suggest variability in clinical performance and highlight the importance of asking about medication-management protocols, incident reporting, and recent survey results when evaluating the facility.

Staffing and management responses are a notable pattern. Many reviews single out named employees and administrators positively for helpfulness and leadership, and some reviewers describe an environment where staff treat residents like family. Conversely, there are consistent comments about high staff turnover, staff moving between units, and uneven performance among nurses and aides. Several reviewers also raised concerns about staff conduct and communication tone, and some described challenges obtaining transparent information from management after adverse events. These points suggest operational weaknesses in staffing stability, conduct oversight, and family communication processes.

On amenities and daily life, the facility receives favorable comments: cleanliness, odor-free common areas, an active social program, and helpful front-desk and administrative personnel. Therapy and rehab are repeatedly cited as strengths, and the center’s local reputation in the Kilgore area is generally positive. However, reviewers also flagged safety and operational controls — for example, expectations around medication handling, staff vetting and oversight, and incident-response procedures — as areas that warrant inquiry.

Overall, the reviews present a polarized picture: many families and residents report positive experiences with clean facilities, engaged staff, and effective rehab services, while a subset of accounts describe significant clinical and communication concerns that affected trust. Prospective residents and families should weigh both sets of observations, tour the facility, meet therapy and nursing leadership, review the facility’s recent inspection/survey documentation, and ask specific questions about staffing ratios, turnover, medication-management protocols, incident reporting, and family communication practices before making a placement decision.

About Arbor Grace Guest Care Center

Arbor Grace Guest Care Center in Kilgore, Texas, provides skilled nursing, long-term care, rehabilitation therapy, memory care, and respite care for older adults, and people find the newly remodeled private and semi-private rooms clean, spacious, and fresh-smelling, with a huge gym that's well-kept and new rehab rooms set up to help people move from hospital back to home. Staff are known for being caring and helpful, and leadership gets good marks for quality management, while the focus always stays on patient-centered care with plans tailored to each person's needs, including special help for those with Alzheimer's and other memory concerns. The facility serves about 70 residents each day, has 127 certified beds, and runs 24/7; guests often mention the easy sign-in process and how thoroughly the office staff answer questions, and the place even has activities like arts and crafts that foster engagement and opportunities for friendship. The facility is affiliated with Hamilton County Hospital District, operates as a for-profit corporation, and is managed by Mpd Texas Resources Of Kilgore LLC and Denny Gamble since June 2023, with direct owners but no specific owner information listed.

Arbor Grace keeps up a strong reputation for community service and aims to create a home-like atmosphere where residents feel respected and cared for, offering both a nurturing environment and support for those who need help with daily living, though it has experienced a higher nurse turnover rate of 64.6% compared to the state average of 51.8%, and it provides 3.47 nurse hours per resident daily, which sits slightly above the state norm. Residents use both English and, in some cases, other languages when talking with clinic staff, while the ombudsman service is there for confidential and free complaint resolution. The facility has guest care services, comprehensive long-term services, rehab, a credentialing process for providers, and office hours which remain open but without set times posted.

On the other hand, inspection reports show 46 total deficiencies, with a standard inspection from April 2, 2025, highlighting four recent problems, and three of those relate to federal infection control, like not fully preventing or managing infections as required, and there were also citations for lacking proper supervision, accident hazard prevention, and not meeting care requirements for bowel and bladder needs, though these got seriousness scores that mean no actual harm happened but potential for more than minimal harm exists; most of these issues affected few or some residents but didn't result in immediate danger. The facility's areas remain clean, the environment has been updated and fresh, and the community strives to provide safety and support, although currently it isn't accepting new patients. Activities and amenities aim to offer daily comfort and engagement, and the private rehab-to-home rooms reflect a recently renovated look. Residents benefit from services such as dietary support, skilled nursing, memory care, personalized care plans, physical, occupational, and speech therapies, and enjoy frequent organized opportunities to stay active and make friends.
