    Compassionate staff, excellent secure care

I'm very pleased with the care here - compassionate, attentive CNAs, nurses and therapists who know residents by name and go above and beyond. Meals are excellent, the building is spotless with a pleasant smell, and the secure memory-care wing gave me real peace of mind. Admissions, rehab and social-work teams were proactive and communicative, and staff make the place feel warm and home-like. Overall I feel my loved one is safe, well cared for, and valued.

    Summary of reviews

Reviews for Five Points of Pflugerville are highly mixed, with a clear pattern of strong frontline care staffed by dedicated individuals alongside systemic operational weaknesses. Many families and residents praised specific caregivers, CNAs, nurses, and therapists for compassionate, attentive care; the therapy department, social work/case managers, and admissions team were repeatedly described as proactive and helpful. Several comments highlighted positive resident engagement through activities and themed events, prompt maintenance work, a secure memory-care wing, and generally clean common areas.

At the same time, recurrent operational issues emerge across reviews. Staffing consistency appears to be a central concern: reviewers described understaffed shifts, high turnover, and particular responsiveness problems on certain shifts that result in delayed answers to call lights and slow assistance. Those staffing gaps are linked to care-process issues such as delayed bathing and incontinence-care, inconsistent medication and paperwork readiness, and limited availability for showers and personal-care routines. Safety practices also drew concern — reviewers cited weaknesses in transfer procedures, fall-prevention measures, and variability in how safety equipment (rails, mats) is used.

Dining and activities present a mixed picture. Many residents find meals acceptable and praise kitchen and food-service staff, but others note inconsistent food quality and limited beverage options. Activity programming and cognitive-stimulation offerings are strong for some residents but are described as sparse or under-resourced in other areas, and crowded common rooms or limited staff availability can reduce participation.

Facility maintenance and environment likewise show both strengths and weaknesses. Maintenance and quick repairs receive positive mention, and some visitors note a clean, fresh-smelling environment and a welcoming lobby. Conversely, there are intermittent sanitation and odor concerns, comments about pest-control issues, and repeated accounts of missing personal items and problematic laundry controls, including serious allegations of theft and mishandling of belongings. These issues, along with reported problems reaching administrative staff and delays in authorizations/referrals and refund/payment processing, point to gaps in administrative systems and oversight.

Management and communication appear uneven. Multiple reviewers praise individual leaders and staff who communicate well and arrange scheduled care meetings; others describe difficulty contacting administrators, poor transparency around significant events, and inconsistent follow-up. A subset of reviews references regulatory concerns and state inspection issues. Improvements since an ownership or management change were noted by some families, suggesting recent efforts to address problems, while other reviews indicate persistent operational and cultural challenges.

In summary, Five Points of Pflugerville shows strengths in individualized caregiving, therapy services, admissions, and certain facility operations, but also evidence of systemic weaknesses: inconsistent staffing and responsiveness, care-process and safety gaps, variable cleanliness and pest-control, administrative delays, and concerns about property controls. Prospective residents and families would benefit from direct inquiry about current staffing patterns, specific safety protocols (transfer and call-response), laundry/item-tracking procedures, and administrative workflows for authorizations and communication when evaluating the community.

    About Five Points of Pflugerville

Pflugerville Care Center is a medium-sized skilled nursing facility with 111 beds, located in the Pflugerville area. The center offers both short-term rehabilitation and long-term care services to its residents, focusing on the needs of older adults who require daily assistance, medical support, or recovery after a hospital stay. The facility operates under hospital district ownership, and it is not part of a larger continuing care retirement community. Pflugerville Care Center participates in both Medicare and Medicaid, allowing for a range of payment options for those seeking care and support.

The care center has received below average ratings for both its short-term rehabilitation and long-term care services. The short-term rehabilitation rating evaluates how the center helps patients recover after events such as a stroke, heart attack, infection, or injury. Staff at Pflugerville Care Center provide an average of 3 hours and 6 minutes of nurse staffing per resident per day, a metric that factors into the center’s performance assessments. Approximately 43.6% of short-term residents are able to return home after discharge, and 12.9% of residents required trips to the emergency room during their stays. The facility shows a strong influenza vaccination rate among residents, with 98% receiving the annual flu shot. Despite this, consistent staffing on weekends is rated as poor, which can affect the quality and continuity of care that residents receive.

Long-term care at Pflugerville Care Center involves providing daily medical and non-medical assistance, with attention to essential activities such as eating, dressing, and using the bathroom. About 78.4% of residents are reported to maintain the ability to perform these activities without help. Emergency room visitation rates for long-term care residents stand at 1.8 visits per 1,000 resident days, indicating occasional acute medical needs despite ongoing support within the facility. The center’s efforts in infection prevention, pain management, activity planning, and nutrition have been areas of focused improvement following regulatory inspections and feedback.

Over the past three years, Pflugerville Care Center has addressed a number of care challenges identified during routine government health and safety inspections. These inspections highlighted issues such as accident prevention, accurate resident assessments, development and implementation of personalized care plans, timely assistance with daily activities, proper food sourcing and preparation, and effective pest control programs. There have been instances where the center worked to enhance protocols for infection control and pain management. Significant concerns have included ensuring resident safety from all forms of abuse and making certain that basic life support measures are administered when needed, always adhering to resident preferences and physician directives.

Pflugerville Care Center has undergone periods of additional oversight by federal authorities due to patterns of care that required close monitoring, particularly between August 2023 and July 2024. As part of its ongoing operations, the facility strives to address these concerns and provide a safe, supportive, and healing environment for its residents. The cumulative effect of governmental oversight, fines, and quality ratings demonstrates both the challenges inherent in skilled nursing care and the continuing efforts at Pflugerville Care Center to improve the well-being of the individuals it serves.
