    Reassured by compassionate, professional care

I placed a loved one here and feel very reassured - the staff are warm, friendly and professional, truly going above and beyond with compassionate, family-like care and excellent rehab/therapy after a stroke. The campus is clean and homey with pleasant, tree-lined grounds, good meals and engaging activities; communication is clear and the team treats residents like people. I'm grateful for the peace of mind this well-run, caring community provides.

    Summary of reviews

Accounts of care at Autumn Winds Living & Rehabilitation are mixed, with a clear pattern of strong, compassionate caregiving described alongside operational weaknesses that affect safety and consistency. Many families praise the nursing and therapy teams for individualized attention, bedside meal options, and effective rehabilitation services (including speech therapy and Parkinsonโ€™s/stroke management). Reviewers frequently highlight a warm, family-like culture among staff, an energetic activities program with live entertainment and volunteer engagement, and a pleasant, tree-lined campus with a restaurant-style dining room. Several comments also note helpful owner- or manager-level involvement and good hospice coordination, which can provide families with additional reassurance.

At the same time, there are recurring operational concerns that prospective families should evaluate. Staffing and coverage appear inconsistent, and multiple accounts describe delays in responding to calls for assistance and in handling medical emergencies; this suggests potential gaps in on-shift staffing levels and emergency protocols. Relatedly, there are concerns about medication and dietary-order adherence and about dementia-specific care practices, which may indicate weaknesses in clinical oversight or training in certain care areas. Cleanliness and odor-control are praised in many descriptions but criticized in others, pointing to variability in housekeeping performance across the facility.

Communication and staff conduct emerge as another area of variability. While many families describe staff who are kind, professional, and communicative, other accounts describe poor responsiveness, concerning interactions, and breakdowns in family communication. There are also allegations of serious staff misconduct and involvement by protective services mentioned in some accounts; such claims merit careful inquiry and verification during a facility tour and through regulatory records. Management activity is described both positively (hands-on owners, new management energizing therapy) and as uneven (inconsistent follow-through), which helps explain the divergence in experiences.

For prospective residents and families: visit at different times of day, observe staffing levels and response times, ask about recent staffing ratios and emergency protocols, request documentation on medication/dietary-order compliance and dementia-care training, and review any recent regulatory or protective-services actions. Confirm the therapy services you expect (speech, physical, occupational) and ask about continuity of housekeeping and odor-control practices. Overall, Autumn Winds shows clear strengths in person-centered caregiving, therapy, activities, and campus amenities, but those positives coexist with operational inconsistencies that should be explored before a placement decision.

About Autumn Winds Living & Rehabilitation

Autumn Winds Living & Rehabilitation is a medium-sized nursing facility offering a range of services for seniors requiring short-term rehabilitation or long-term care. Licensed for 96 beds, the facility operates with a for-profit, LLC ownership structure and participates in both Medicare and Medicaid programs. Autumn Winds Living & Rehabilitation is designed for those seeking skilled nursing care and assistance with daily living, but it is not a part of a larger continuing care retirement community. Its skilled care services are available to individuals recovering from hospitalizations for conditions such as stroke, heart attack, infection, or injury, providing them with the opportunity to regain their strength and abilities before returning home.

Residents at Autumn Winds Living & Rehabilitation have access to daily nursing support, with each resident typically receiving around 2 hours and 54 minutes of nurse staffing per day. The facility is rated as average for its short-term rehabilitation services, with approximately 52.4% of residents able to return home following their stay. Measures are in place to manage residentsโ€™ health and safety, including maintaining high vaccination rates; nearly all residents recently received the annual influenza vaccination.

Autumn Winds Living & Rehabilitation focuses on supporting residentsโ€™ independence as much as possible. A significant majority, about 90.4%, have maintained the ability to manage daily activities such as moving, eating, and personal hygiene without the need for assistance. The facility also tracks health outcomes for its residents, such as fall injuries, infection rates requiring hospitalization, and emergency room visits, to help continually assess and improve its care practices.

In addition to medical and rehabilitative care, Autumn Winds Living & Rehabilitation places an emphasis on maintaining a safe and comfortable environment for residents. Regular state and federal inspections are conducted to monitor health, safety, and quality standards. Recent reports have identified various areas for improvement, such as honoring residentsโ€™ rights to dignity, personal possessions, privacy, and the organization of resident or family groups. The facility also continues to evaluate its practices in medication management, food preparation, infection control, and the provision of a homelike atmosphere to meet the needs of its residents.

The care provided extends to specialized needs, such as appropriate pressure ulcer prevention, respiratory care, pharmaceutical oversight, and safeguarding of medical records. Autumn Winds Living & Rehabilitation strives to foster a respectful and supportive community atmosphere, where residentsโ€™ medical, social, and personal needs are addressed with compassion. While it is not affiliated with a hospital or a larger retirement community network, the facility remains focused on delivering individualized care to seniors requiring either short or extended stays.
