    Compassionate, outstanding care and rehab

I moved my mom to Silver Tree and have been very pleased - leadership and administration are responsive, staff are caring and attentive, and the nursing and therapy teams (PT/OT/ST) were outstanding in her rehab. The building is clean, well-run with engaging activities and good meals, staff feel like family, and I would happily recommend it for compassionate, high-quality care.

    Summary of reviews

Silver Tree Nursing and Rehabilitation elicits strongly mixed impressions. Many families and former residents described high-quality rehabilitation outcomes, praising the physical, occupational and speech therapy teams for measurable functional gains and successful discharges home. Nursing care is frequently characterized as compassionate and attentive; several accounts highlight caregivers who provide personalized attention and create a family-like environment. The facility's amenities — including a beauty salon, game and craft rooms, courtyard with seating, and live entertainment — are commonly cited as strengths that contribute to resident engagement and a welcoming atmosphere.

At the same time, a recurring operational pattern undermines otherwise positive aspects of the facility. Staffing shortages and slow response times were described across multiple accounts, producing delays in assistance, medication administration, and timely attention to clinical needs. Communication and documentation gaps were also prominent: families described unclear coordination during admissions and discharges, transport errors, paperwork delays, and inconsistent follow-up after discharge. A subset of accounts raises serious clinical concerns such as wound-care issues, dehydration or post-discharge deterioration; these items point to weaknesses in clinical oversight, discharge planning and continuity of care rather than a universal condition across the facility.

Dining and activities present a mixed picture. Many reviewers praised high-quality meals, themed programming, and an active activities schedule (bingo, veterans meetings, holiday events, live performers). Others described inconsistent meal timing, staffing shortages in dietary services, or meals that sat before service. Facility maintenance and cleanliness are often lauded — several families noted an odor-free, well-kept building and pleasant grounds — yet a number of comments indicate inconsistent sanitation practices in specific areas (bathrooms, trash handling) that merit attention.

Management and leadership are described inconsistently. Some families named administrators and staff as responsive and proactive, crediting specific employees with strong communication and problem resolution. Conversely, other accounts cite management instability, staff turnover, and concerns about administrative handling of funds and records; there are even allegations of financial and administrative mismanagement that would warrant verification through regulatory records. Overall, the pattern is one of strong clinical and social programming potential combined with uneven operational reliability.

For prospective residents and families, the central takeaway is that Silver Tree offers substantial strengths in rehabilitation, therapy, amenities and community activities, but operational variability — particularly around staffing, communication, discharge planning and some clinical processes — can materially affect an individual’s experience. Those considering placement should tour the facility, ask specific questions about staffing levels, medication and wound-care procedures, discharge and home-health coordination, and review recent state inspection and incident history to assess whether the facility’s operational practices meet the level of clinical reliability required for their situation.
