    Welcoming, compassionate, clean, communicative care

I toured the facility and felt welcomed-staff greeted us, introduced themselves, answered questions, and were consistently friendly and responsive. My family has seen attentive, compassionate nursing and rehab, good memory-care support, frequent communication, a clean, lively, family-like atmosphere with activities and pets, and helpful administrative guidance on medical/Medicaid matters; overall I'm very pleased and would recommend it.

    Summary of reviews

Overview: Reviews of Nesbit Living & Recovery Center show a mixed picture. Many families and visitors describe strong clinical and rehabilitative outcomes, an active activities program, and a compassionate, family-like culture among direct-care staff. At the same time, others highlight operational weaknesses in housekeeping, maintenance, medication handling, and administrative responsiveness. Prospective families should weigh both the clinical strengths and the operational inconsistencies when evaluating placement.

Care quality and clinical concerns: Positive accounts emphasize attentive nursing, successful therapy and rehabilitation services, and supportive memory-care and hospice resources. Several reviewers specifically praised rehabilitation teams that aided functional recovery and nursing staff who were caring with dementia residents. However, other accounts describe medication delays, gaps in medication-approval workflows, and more serious clinical monitoring concerns; a small number of reviews described severe infection-related complications. There are also patterns suggesting weaknesses in transfer and fall-prevention practices. Families should ask about medication administration protocols, clinical oversight, staff-to-resident ratios, and incident-response procedures during a tour.

Staff and management: Many comments reflect friendly, helpful frontline staff and social-work support that communicates with families. Conversely, a significant subset of feedback indicates inconsistent communication from leadership, rude or unhelpful administrative interactions, and high staff turnover. Reported problems include poor responsiveness to family concerns, difficulty reaching staff via phone, and breakdowns in the admission/liaison process. Prospective families may want to request current staffing information, escalation contacts, and examples of how management addresses complaints.

Dining, activities, and resident life: The facility receives positive remarks for an active calendar of activities, a social atmosphere, on-site amenities such as a beauty salon, and a pet-friendly environment. These elements contribute to social engagement and resident satisfaction in many reports. At the same time, some reviewers noted inconsistent meal-service timing and at least occasional interruptions to expected dining schedules. It can be useful to observe a meal service and activity period during a visit to assess consistency and resident engagement.

Facilities, housekeeping, and maintenance: Several visitors praised attractive decor and clean common areas, while other reviews document variability in room readiness, housekeeping follow-through, sanitation and odor concerns, pest-control issues, and delayed maintenance responses (including plumbing and window leaks). Instances of TVs or rooms not being set up at admission were also mentioned. These mixed reports suggest that physical upkeep and housekeeping consistency may vary by unit or shift; families should inspect rooms and ask about housekeeping schedules, pest-control contracts, and maintenance response times.

Notable patterns and recommendations: The overall pattern is one of operational variability: strong clinical and relational strengths exist alongside notable administrative, housekeeping, and communication inconsistencies. When considering Nesbit, arrange an in-person tour, meet nursing and administrative leadership, observe activities and meals, and request written policies on medication management, infection control, staffing ratios, fall prevention, and transfer protocols. Ask for references from recent families and clarify how the facility handles complaints, phone communications, and emergency hospital returns. Those steps will help determine whether the facility’s strengths align with a prospective resident’s clinical and quality-of-life needs.

About Nesbit Living & Recovery Center

Nesbit Living & Recovery Center is a large, for-profit nursing facility with 134 beds, providing both short-term rehabilitation and long-term care in the Seguin, Texas area. The center participates in both Medicare and Medicaid programs, offering an accessible option for individuals requiring skilled nursing care after a hospital stay or for those who need ongoing assistance due to chronic medical needs. Nesbit Living & Recovery Center is not part of a larger continuing care retirement community and serves a diverse population, emphasizing care for individuals who need daily medical or non-medical support.

The facility’s short-term rehabilitation program is designed for patients recovering from surgeries, hospitalization for acute illnesses, or injuries such as stroke, heart attack, or fractures. Clinical data indicates that approximately 42.3% of these residents are able to return home after their discharge. During their stay, an average of 2 hours and 27 minutes of nurse staffing is provided per resident per day. The incidence of falls with major injury is low, and infection rates requiring hospitalization during short-term rehabilitation are monitored, with about 7.3% experiencing serious infections necessitating a hospital visit. Additionally, emergency services are on hand, as indicated by the 16.3% of short-term residents who required ER visits.

For long-term residents at Nesbit Living & Recovery Center, the goal is to maintain independence while ensuring safety and health. Roughly 80.9% of long-term residents maintained self-care abilities, such as moving, eating, and using the bathroom without help. Vaccination rates at the center are high, with 99.7% of residents receiving annual flu shots, which reflects an emphasis on preventative health. Staffing for long-term care matches that of the short-term program, and the center is recognized for providing consistent staffing levels through weekends as well as weekdays.

Nesbit Living & Recovery Center is subject to regular inspections covering health, fire, and safety standards, as well as the effectiveness of training programs and adequacy of facility staffing. In recent reviews, areas of focus included proper staff licensure, developing effective resident-centered care plans, managing personal funds security, providing proper training on dementia care, medication management, and maintaining comprehensive medical records. The center ensures that residents’ rights are respected, including their participation in care planning, their right to request or refuse treatment, and making informed decisions about care directives.

The nursing facility places an emphasis on a safe and comfortable environment for residents, staff, and visitors, and strives to maintain cleanliness and accessibility throughout its premises. Behavioral health training and education on resident rights and facility responsibilities are incorporated into ongoing staff development to support the well-being of all individuals in their care. The commitment to quality improvement is further underscored by mandatory quality assurance and performance improvement training for all staff members. With a combination of skilled nursing care, supportive services, and a focus on resident autonomy, Nesbit Living & Recovery Center aims to provide a supportive living and recovery environment for seniors and individuals in need of rehabilitative or long-term care.
