    Attentive staff and excellent rehab

I placed my mother here and have been very pleased - the facility is clean and welcoming, the nurses, CNAs and therapists are attentive and go above and beyond, meals are provided regularly, and rehab/therapy is excellent. Plenty of activities, comfortable rooms and a warm, family-like atmosphere; I would recommend this community.

    Summary of reviews

The reviews of Reunion Plaza Senior Care and Rehabilitation Center present a mixed but distinct pattern: many families and residents praise the day-to-day caregiving, therapy services, and social environment, while others describe operational and clinical gaps that affected safety and satisfaction. Strengths most frequently highlighted include compassionate CNAs and nursing staff, a robust therapy/rehab program with a modern therapy wing, an active activities schedule (bingo, live music, church services and seasonal events), and amenities such as an on-site beauty shop. Multiple accounts describe the facility as clean and welcoming in parts, with attractive landscaping and communal spaces that support socialization and family interaction.

Care quality and staffing present contrasting experiences. Positive feedback emphasizes attentive nursing, effective physical therapy, and staff who go above and beyond for residents. However, reviewers also describe inconsistent staffing levels, high turnover, and delayed responses to call bells and requests for assistance. These operational patterns are associated with concerns about medication administration accuracy, timeliness of care, and gaps in chronic-condition management including wound care and diabetes monitoring. Several reviewers described situations where clinical escalation and timely follow-up did not occur as expected, indicating inconsistent safety monitoring processes.

Dining and housekeeping feedback is similarly mixed. Many residents enjoyed meals, occasional standout entrees, and regular meal service; others noted variability in food temperature, portioning, and limited snack availability. Housekeeping and room-cleaning receive positive mentions for daily upkeep in many comments, yet sanitation and odor concerns in some areas and inconsistent linen/room-turn practices were also reported. These contrasting observations suggest variability in day-to-day operational consistency across units and shifts.

Activities, amenities, and facilities are often called out as strengths. The facility offers an engaging calendar, social events, and spaces conducive to visiting and therapy. The therapy unit and some communal areas are described as modern and well-maintained, and private-room options are available for short-term stays. Positive interactions with admissions and certain administrative staff, as well as helpful business-office personnel, were reported and appreciated by families.

Management, communication, and administrative processes show uneven performance. Several reviewers praised specific administrators and department leaders for responsiveness and leadership, while others experienced unsatisfactory follow-through on complaints, coordination issues with caseworkers, delays in equipment provision, and billing disputes — including aggressive collection communications in some accounts. These patterns point to variability in operational oversight and consistency in customer-service processes.

Notable patterns for prospective residents and families to consider: ask about current staffing ratios and turnover on the unit where placement is planned; review medication administration policies and how clinical changes are escalated; clarify wound-care and chronic-condition protocols; inspect meal service routines and snack availability; and verify billing practices and insurance coordination processes. A tour focused on the specific unit, a meeting with therapy and nursing leadership, and a review of recent state inspection or deficiency history can help balance the facility’s strong rehabilitation and social programming against the operational concerns some families experienced.

    About Reunion Plaza

Reunion Plaza Senior Care And Rehabilitation Center provides a range of care for seniors, from skilled nursing and assisted living to memory care and independent living, along with both long-term and respite care options, so you'll find different types of support under one roof for people who need help with daily life, memory loss, or even if someone's recovering from an illness or surgery, because they're set up to take care of seniors with a variety of needs. The center has 129 certified beds and usually cares for about 88 residents each day, and you'll find bedrooms tailored for comfort along with living spaces that look out onto outdoor areas, while trained staff are there around the clock to help with medical needs, including medication, wound care, and therapies for seniors who are working to recover from cardiac, pulmonary, or orthopedic issues. People get help with meals if needed, choosing from menu options with their "Dining Your Way" program, which feels more like a restaurant than an ordinary cafeteria. Reunion Plaza also offers specialized services like podiatry, optometry, and dentistry on-site, along with pharmacy and lab services, so you won't need to travel far for those things, and residents can visit the on-site beauty and barber salon too. They have a LifeWorks wellness program with activities to keep minds and bodies active, and they offer rehabilitation-both for short stays and as outpatient therapy-including physical, speech, and occupational therapy. Oxygen therapy, restorative nursing, IV, enteral, and parenteral therapies are available, and staff help seniors manage respiratory issues as well as other medical conditions, along with providing hospice when needed. The center provides social services and family involvement programs to support emotional health, and there's a long-term care ombudsman to help resolve complaints. Run as a non-profit, the center's been managed by Fannin County Hospital Authority since 2017 and under Pf Reunion Snf Ops, LLC since 2021, plus it's tied to Stonegate Senior Living, while staff work to create a welcoming place that feels like home. Nurse staffing hours average 2.87 per resident each day, which is lower than the state average, and they've seen a higher nurse turnover than the Texas average-about 68.4%. There have been 85 deficiencies according to inspection reports, including some serious issues involving infection control, resident safety, and accident hazards, with a few matters called immediate jeopardy to residents, so families may want to check for more details about those reports and see how concerns get handled. Reunion Plaza has experience with rehabilitation and memory care, so they'll try to match care and programs to what each senior needs, aiming to offer medical attention, social support, and daily help in one senior care center where staff try to be friendly and ready to answer questions for families or those considering a stay.
