    Caring staff and responsive maintenance

I'm a family member of a long-term resident and have had a very positive experience - staff are consistently kind, helpful and truly care, maintenance (Dan) is responsive, and the therapy team helped with a quick recovery. The facility is clean, accepts Medicare, has outdoor access and a nearby pharmacy/church, and the renovations underway are looking great. I'd recommend it for short- or long-term stays - leadership is strong and the community feels caring and well-run.

    Summary of reviews

Reviewer accounts of Focused Care at Waxahachie describe a facility with substantial variation in experience. Positive feedback highlights compassionate caregivers, an effective therapy department, responsive maintenance, and a welcoming resident community with outdoor access and nearby amenities. Several reviewers specifically praised short-term rehabilitation outcomes and noted that the facility accepts Medicare and provides both short- and long-term care options.

Clinical care quality appears mixed. Many reviewers praised individual caregivers for attentive, kind care, while other accounts indicate lapses in clinical processes such as medication charting, delayed medication administration, and limited care coordination that some families associated with hospital readmissions. There are also recurring concerns about delayed responses to call lights and other requests for assistance; these delays are described as intermittent but potentially clinically significant when they occur.

Staffing and conduct present a clear pattern of inconsistency. Reports reflect both hardworking, long-tenured staff and instances of rude or confrontational interactions, poor phone responsiveness, and uneven application of policies. High staff turnover and perceptions of favoritism were frequently mentioned as drivers of variable service quality. Families and reviewers also noted that communication and transparency around procedures and incidents are uneven, contributing to frustration and mistrust in some cases.

Dining, activities, and daily life were described as uneven. The therapy program and some activity offerings received positive comments, but meal quality and presentation were cited as inconsistent, and several reviewers found resident rooms to be bland or lacking stimulating engagement. Personal-care routines were described as inconsistent, with specific references to delayed bathing and clothing assistance in some accounts.

Facility condition and cleanliness are areas of concern and contrast. While some communal areas were described as clean and attractively decorated, multiple accounts raised sanitation and pest-control concerns, odor concerns, and issues with room cleanliness. Infrastructure issues were also noted, including uneven progress on renovations, HVAC problems, and episodic maintenance needs. On the positive side, some reviewers praised a responsive maintenance team and management staff who address physical-plant issues when engaged.

Management practices and incident handling show mixed signals. Several reviewers commended mission-focused leadership and administrators who were receptive and engaged, while others criticized inconsistent policy enforcement, poor complaint resolution, and inadequate transparency following incidents, including property loss. A few accounts escalated to police involvement or described serious family concerns following adverse outcomes; these were framed by reviewers as reasons to scrutinize clinical oversight and incident-resolution processes.

For prospective residents and families: the facility demonstrates clear strengths in therapy, individual caregivers, and certain operational areas, but consistent patterns of variability suggest the need for active inquiry during tours. Recommended questions include staffing stability and ratios, medication-administration and charting protocols, sanitation and pest-control measures, incident-reporting and resolution procedures, recent renovation timelines, and examples of family communication practices. A direct, in-person assessment and conversations with current families can help determine whether the facility’s strengths align with a prospective resident’s priorities.
