    Compassionate skilled care, clean facility

I was very pleased with the care my loved one received - the nursing, CNA and therapy teams were skilled, attentive, and genuinely caring. The facility is clean, well-decorated, with engaging activities and accommodating dining; staff treated us like family and communicated promptly. I highly recommend this facility for rehab and long-term care.

    Summary of reviews

Reviews for this facility are mixed, with a clear pattern: many families praise the clinical rehabilitation and supportive staff, while others describe operational inconsistencies that affected safety, comfort, and communication. The facility receives consistent positive feedback for its therapy services, many instances of skillful nursing and attentive aides, and an overall environment that is attractive and well maintained. At the same time, several families reported instances where routine care processes did not meet expectations.

Care and staff: The therapy department (PT/OT) is frequently highlighted as a strength—therapists are described as skilled, encouraging, and effective in helping residents return home. Numerous comments also recognize compassionate CNAs and nurses who provide attentive bedside care. However, reviewers described inconsistent responsiveness from nursing staff at times: delayed responses to call buttons, missed or delayed medication administration, and lapses in monitoring basic needs (for example, incontinence-care checks and linen changes). There are also repeated concerns about transfer safety and mobility oversight, including wheelchair battery/maintenance checks and the timeliness and safety of transfers and ambulation assistance.

Dining and nutrition: Dining experiences are variable. Positive notes include accommodating menu planning for dietary needs, assistance with dentures, family meal tickets, and several accounts of tasty, hot meals. Conversely, others described cold meals, missed meal deliveries, intermittent kitchen equipment issues, and the need to supplement nutrition with commercial supplements or outside food. Families should verify dining continuity and special-diet handling as part of their evaluation.

Activities and amenities: The facility’s activities program, attractive common areas, patios, and home-like décor are frequently praised. Many reviewers appreciated the availability of engagement options and the pleasant grounds. Private rooms and comfortable common spaces were also noted as a positive environment for recovery or long-term residence.

Facilities and sanitation: The building and grounds are commonly described as clean, well maintained, and inviting. Nonetheless, some reviews raised sanitation and linen-management concerns, including episodes of missing linens, bedding issues, and inconsistent housekeeping in specific situations. Prospective families may want to ask about laundry turnaround times, linen protocols, and routine housekeeping checks.

Management and communication: Opinions about administration are polarized. Several families reported administrators and admission staff who were professional, sympathetic, and effective at resolving issues. Other reviews describe lapses in administrative accountability, poor phone access or unreturned calls, billing or follow-up delays, and access-control shortcomings. There are also accounts of regulatory involvement and family complaints in individual situations. Communication style and follow-through appear uneven across shifts and teams.

Notable patterns and recommendations: The strongest and most consistent positives are the rehabilitation services, the therapeutic staff, and the facility’s physical environment. The consistent negatives are operational: staffing responsiveness, medication and clinical-tracking reliability, sanitation/linen management, and some safety practices around transfers and access control. Prospective residents and families should assess current staffing levels, medication administration protocols, fall-prevention and transfer procedures, linen/housekeeping policies, and direct lines of communication with nursing leadership during a tour or admission discussion to determine whether the facility’s strengths align with their care priorities.

About Senior Care Health & Rehabilitation

Senior Care Health & Rehabilitation of Wichita Falls, TX has been serving the community since 1969 and operates as a skilled nursing home for seniors who need serious medical help or long-term care, and while it's not a BBB-accredited business, it's known locally for its thorough care and strong team. The facility, managed by Ms. Karen Harvick since new ownership took over in 2007, offers both short term and long term care, memory care, and therapy services including physical, occupational, and speech therapy, with the Rapid Recovery Rehab Program for those needing to regain strength and independence after a hospital stay. Residents get help with daily activities like bathing, dressing, and medicine, with 12 to 16 hours of nursing care and supervision available around the clock, along with a 24-hour call system.

There are 16 certified beds available as of June 2025, with rooms that come furnished and have private bathrooms, cable TV, Wi-Fi, and air conditioning, making stays more comfortable, and they accept both Medicare and Medicaid. The staff, including nurses, therapists, and clinicians, use a personalized approach to care with input from on-site physicians and therapists, so each resident has a care plan fitted to their specific needs, whether it's dealing with chronic illnesses, post-surgery recovery, or memory loss.

Meals are prepared to meet a range of dietary needs, including those for diabetes and allergies, and the facility offers both communal dining and private arrangements. Residents can use amenities like the library, salon, fitness and wellness center, spa and wellness room, and garden areas with outdoor walking paths, plus shared indoor spaces like an arts room, game room, and relaxation spots. The community runs a full day of scheduled activities, movie nights, music programs, group exercise, hobbies, spiritual activities, and family support services, making sure people can stay active and engaged. Housekeeping, meal prep, move-in coordination, and concierge services are available to make life a bit easier, and transportation is provided for shopping and medical appointments, with parking for visitors.

Social work services come through the Texas State Board of Social Worker Examiners, adding another layer of support for residents and their families, and the staff is trained in medication management to keep safety high. The facility puts effort into keeping a warm environment that welcomes families, focusing on dignity, respect, and quality of life, and many people in the area speak positively about it. Spiritual programs, entertainment, and group events help residents build connections, and relaxation spaces inside and in the gardens give room to enjoy quiet moments. The facility has held a solid place in Wichita Falls' healthcare since 2008, and stays open every day, always ready to provide skilled nursing and rehabilitation for those who need it.
