    Renovated facility with caring staff

I've lived here since 2014 and I'm very happy with the new ownership, management and renovations. The staff-from attentive CNAs to phenomenal respiratory, PT/OT and nursing teams-are caring and quick to respond; the food and daily activities are excellent and residents feel like family. Management is supportive and involves families; I highly recommend this facility for rehab and long-term care.

    Summary of reviews

Reviews describe a facility with a mix of clear strengths and notable operational challenges. Clinical rehabilitation services and therapy are frequently praised; families and former residents credit the therapy teams with measurable mobility improvements and enhanced quality of life. Several reviewers highlight respiratory therapy capability and effective care navigation for residents with complex needs.

Frontline caregiving receives generally positive comments: many reviewers identify CNAs and night-shift staff as attentive and compassionate, and note long-tenured kitchen and care personnel who create continuity for residents. Families also report positive reception interactions and instances of constructive family involvement. The activities program is commonly described as engaging, offering daily social opportunities that residents enjoy.

Conversely, there are recurrent concerns about facility operations. Cleanliness and sanitation issues, along with pest-control concerns, are raised often enough to indicate inconsistent environmental maintenance in some areas. Meal quality and consistency are variable: comments range from praise for good food to complaints about overcooked or inadequate meals and interrupted meal service. Supply management problems such as linen shortages were also mentioned.

Staffing and clinical operations show mixed performance. Staffing stability and weekend coverage are identified as weaknesses, and reviewers describe delays in medication administration, laboratory follow-up, and assistance with feeding and other ADLs. Several comments point to inconsistent staff conduct and responsiveness at times, particularly on weekends, which can affect day-to-day care experience. There are also gaps reported in communication and coordination with families and attending physicians, and reviewers cite delays or non-answers from clinical leadership.

Management, oversight, and compliance emerge as meaningful themes. Some families praised strong frontline leadership, while others described generic administrative responses to concerns and slow follow-up. Allegations of theft and mention of regulatory citations or fines indicate serious issues that prospective residents and families should investigate further with state inspection records. Infection-control and outbreak communication were identified as areas needing clearer, more timely disclosure and management.

In sum, Old Southwest Rehabilitation and Nursing presents a combination of solid rehabilitation and caring frontline staff alongside operational weaknesses in environmental maintenance, staffing consistency, clinical follow-through, and administrative responsiveness. Prospective residents and their families should weigh the facility’s therapeutic strengths and engaged caregiving against the reported variability in housekeeping, supply management, staffing reliability, and management responsiveness; verifying current inspection reports and arranging an in-person tour at different times (including weekends and nights) is advisable to assess current conditions and leadership engagement.

About Old Southwest Rehabilitation and Nursing

Old Southwest Rehabilitation and Nursing sits at 324 King George Avenue, SW, Roanoke, VA 24016-5213, managed by Mr. Trent K. Clugston, and it's owned by The Portopiccolo Group, which folks might find interesting since it's part of both the Virginia Health Care Association and the Virginia Center for Assisted Living. The place has 130 licensed beds and can serve people needing skilled nursing, long-term care, and rehabilitation, with 18 of those beds set aside for vent patients, and they even take more than one family member if families prefer to stay together. People will find private rooms at a daily rate of $240 and semi-private rooms for $215, and there are full-time staff-about 68.3 full-time equivalents as of 2023-to help meet residents' needs. There's 24-hour clinical nursing care with licensed nurses, certified nursing assistants, an on-site medical director, and on-call area doctors, all using advanced medical equipment and a nurse call system for safety and quick response.

The care at Old Southwest includes skilled nursing, pain management, restorative nursing, trach management and support, stroke recovery, wound care, IV infusion therapy, post-op rehab, post-illness, and injury rehabilitation, plus hospice and palliative care for when that's needed, along with personalized nutrition plans made by a registered dietitian, which people often say brings comfort because meals are so important. The community takes Alzheimer's and dementia patients on a case-by-case basis, though there's no special Alzheimer's unit, but the nursing team works on individual memory care plans when needed, and they'll offer rehabilitation and long-term care for people with all sorts of backgrounds-some even coming in for short stays like respite care if family needs a break, since that's one thing families worry about a lot. Social activities, educational programming, and events happen on a regular basis to keep everyone involved, and people say the dining experience stands out, often helping residents feel they're more at home than in a hospital.

You'll find Old Southwest connected to groups like VirginiaNavigator, SeniorNavigator, and similar directories, with links to a directory of over 26,000 programs and services across local, state, and national groups, which helps families and residents figure out next steps or look for support. Counseling and discharge planning takes some worry away when it comes time to move or transition after a rehab stay, because these things can get complicated otherwise. Mr. Trent K. Clugston as the administrator runs the facility, and there's focus on efficiency with VHI efficiency indicators to keep things running smooth, which folks who've stayed here or had family members here say helps keep standards up and avoids things falling through the cracks. Old Southwest can be found online at both www.avantecenters.com and www.oldsouthwesthealth.com if people want to check more details, but what stands out is that this isn't a place that just says it cares but tries to support real needs with 24/7 care, family involvement, case-by-case assessment for memory care, and accessible resources for veterans, seniors, and people with disabilities.
