    Modern facility, caring staff, independence

I had a very positive stay - the remodeled, clean facility felt modern (new TVs, updated call system) and check-in was smooth. The staff were consistently caring, attentive, and proactive - nurses, CNAs and therapy (led by Mr. Konn) helped me go from bedbound to walking with a walker and regain independence. The family-like atmosphere, plentiful activities, pleasant garden and personalized care made me comfortable. I felt well cared for and would return.

    Summary of reviews

Overall impression: Reviews collectively present Pacific Care Center as a facility with consistently praised caregiving and rehabilitation services, complemented by recent facility upgrades and an active social program. Families and residents commonly describe staff as compassionate, attentive, and willing to go beyond baseline duties; several accounts emphasize personalized, proactive assistance and strong pain-management practices. Physical therapy is a recurring highlight, with multiple reports of significant functional gains and named staff and leadership receiving positive recognition.

Care and clinical oversight: Clinical care is characterized by attentive nursing and coordinated, multidisciplinary team planning. Reviewers note effective pain control, timely therapy interventions, and structured team meetings that support goal-directed rehabilitation and discharge planning. Rehabilitation outcomes are frequently described as strong, with examples of residents improving from limited mobility to walking and returning to community activities. Management involvement in clinical processes and an emphasis on safety and infection-control precautions are also noted.

Staff and culture: The workforce is described as warm, family-oriented, and hardworking. Reviewers use terms such as compassionate, helpful, and professional to describe CNAs, nurses, therapists, and administrative staff. Several comments highlight staff willingness to accommodate families and to remain engaged beyond scheduled hours, which contributes to a perception of high-quality, person-centered care. That dedication is framed positively, though it also underlies a potential operational pressure point (see cons).

Dining and activities: The activity program is robust, with multiple daily offerings that include bingo, singing, dancing, themed celebrations, and grooming services such as haircuts and manicures. Indoor and outdoor communal spaces — including a day room, pleasant window views, and a landscaped garden with bird feeders — are seen as valuable for resident engagement. Dining impressions are mixed: some reviewers praise the dining environment and food, while others characterize meal quality as unacceptable; this suggests variability in culinary delivery or menu execution.

Facilities and operations: The center has undergone visible remodeling with modern finishes, a contemporary call system, and new in-room televisions. Bathrooms on every hall and spacious rooms are noted as amenities. The sign-in and visitor-screening process is described as efficient and user-friendly, with printed badges that aid identification. Infection-control measures have led to activity cancellations at times, consistent with standard outbreak precautions.

Notable patterns and considerations: The dominant theme is strong, resident-focused caregiving and effective rehabilitation supported by recent capital improvements. Primary operational concerns to weigh are inconsistent meal quality, the presence of shared accommodations and an overall nursing-home atmosphere that may not suit those seeking a more private setting, occasional activity interruptions tied to infection-control events, and an implied reliance on staff working extended hours. Prospective residents and families should weigh the facility’s clear strengths in therapy and staff responsiveness against these trait-level considerations, and may wish to ask leadership about meal planning processes, room configurations, staffing models, and how activities are maintained during infection-control periods.

    About Pacific Care Center

Pacific Care Center, located at 3035 Cherry St. in Hoquiam, WA, offers skilled nursing, rehabilitation, and memory care, all under one roof, and the place's run by Hoquiam Healthcare, Inc., doing business as Pacific Care Center, owned by Ensign Services, Inc. with Medicare certification and 72 licensed beds, so you'll find both private and semi-private rooms that're well set up for comfort. The center has nurses and other healthcare staff around all day and night, who help with ongoing medical needs, daily life tasks like bathing and dressing, and other personal care. Memory care's available for folks living with dementia or Alzheimer's, and the staff tailor care plans as the disease changes, while the cost for this kind of service does depend on the level of assistance needed and how far the disease has gone, which can be about three times higher than regular healthcare for seniors over 65, but many people get help through programs like Medicaid, Medicare, and social security disability.

Short-term rehabilitation helps with recovery after surgery, illness, or injury, and there're state-of-the-art in-house therapy services for physical, occupational, and speech therapy, so people can work on getting stronger and regaining skills, and there are specific programs, such as post-op and neurological rehabilitation, arranged to match what each person needs. On the long-term side, residents who need continuing medical attention and support can stay as needed, and the interdisciplinary treatment team works together to keep residents comfortable and promote recovery and good quality of life, which is important when you need both caring nurses and knowledgeable therapists who set up clear and effective plans. You'll also find dietary and nutritional services with meals planned according to health needs and nutrition counseling, and the meal program in assisted living offers one to two prepared meals every day, while laundry and housekeeping services are routine.

The center's grounds have beautifully landscaped areas, with two large indoor gardens and an outdoor courtyard where people can visit their families in a safer way, especially when health conditions might be a concern. The activities program includes daily options for socializing, exercise, and other things people enjoy, because staying active is important, and you'll find everything from small gatherings to excursions out and about, so there's a choice for everyone. They offer a friendly, caring staff and aim for an environment that feels welcoming and home-like, no matter if it's a short or long stay, and the reviews seem to average about 4.7 stars based on what folks have posted, with many liking the compassionate care given.

The place accepts Medicare, most major insurances, and works with different funding sources. There's also an option to claim the community listing for administrative purposes and the location shows up clearly on available maps. Pacific Care Center's main service is nursing and rehab, but assisted living, respite care, and memory care are also offered, alongside therapy and activity programs, and the goal throughout the center is to help every resident feel as comfortable, active, and cared for as possible within the support and healthcare levels offered.
