    Kind staff, clean facility, recommended

I placed my mom here and I'm very pleased - the staff are kind, friendly and knowledgeable, and the nursing and rehab care have been excellent. The facility is very clean and smells fresh, it's conveniently close to the hospital/rehab center, and rooms have a Jack-and-Jill shared bathroom; the food is just average, but overall I would recommend it.

    Summary of reviews

Reviews indicate a facility with a mix of positive individual caregiving experiences and significant operational challenges. Several families and visitors describe certain CNAs, nurses, and therapy staff as caring, knowledgeable, and effectiveโ€”particularly in short-term rehabilitation contextsโ€”and the facilityโ€™s proximity to a hospital and outpatient rehab services is viewed as an asset. Some reviewers also characterize interior areas as clean and find rehabilitation programming helpful for recovery goals.

At the same time, a recurring theme is staffing instability. Reviewers describe frequent short staffing, last-minute nurse call-offs, and staff who appear overextended; these conditions are associated with long wait times for assistance, slow responses to call lights, and inconsistent bathing and toileting support. There are repeated claims of delays or lapses in medication administration and of insufficient clinical oversight, including instances that resulted in emergency transport or family concern over physician availability.

Dining and basic supplies are another area of concern. Multiple comments reference inconsistent meal quality, cold or inadequate portions, and restricted access to snacks or beverages tied to budget practices. Some reviewers said staff personally supplied snacks when facility provisions were limited. These reports suggest gaps in meal-service continuity and resident hydration/snack routines.

Environmental and safety issues are reported as well. Reviewers raise sanitation and incontinence-care concerns in resident areas, questions about water quality and plumbing, and instances they perceive as inadequate infection-control measures. Room layouts such as shared "Jack-and-Jill" bathrooms are noted; while not inherently problematic, these configurations can complicate privacy and staffing workflows when combined with staffing shortages.

Management and culture are also recurring focal points. Several reviewers describe limited transparency from administration, budget-driven decisions that appear to affect resident services, and a perception that financial priorities outweigh care needs. A subset of reviews includes serious allegations of severe misconduct and other safety-related incidents; these claims are extreme in nature and warrant direct follow-up with facility management and, if appropriate, regulatory bodies to verify facts and corrective actions.

Overall, the facility presents a mixed picture: there are evidently competent and compassionate staff members and effective rehabilitation services for some residents, but persistent operational weaknessesโ€”most notably staffing instability, delays in assistance and medication administration, inconsistent dining and supply provisions, and management communication gapsโ€”create risks to consistent care quality. Prospective residents and families should seek up-to-date staffing metrics, infection-control policies, meal-service procedures, and documentation of any incident investigations when evaluating this facility.

    About Clarksburg Healthcare Center

Clarksburg Healthcare Center, located at 2096 Davisson Run Road in Clarksburg, West Virginia, is a skilled nursing facility offering assisted living, memory care, skilled nursing, respite care, and rehabilitation services. The place focuses on creating a supportive and nurturing environment, where staff and residents often get to know each other pretty well and family members take part in care planning. With an average daily census of about 93 residents and 110 certified beds, the center provides care for both short-term rehab and long-term stays, including services for those with Alzheimer's or dementia, as well as palliative and hospice care. Residents get assistance with daily needs like bathing, dressing, and transferring, and support includes medication management, mental wellness programs, and physical, occupational, and speech therapy, including some therapy from Lifetree Therapy Company. They handle more complex health needs too, including ventilator rehabilitation, in-house dialysis, and substance abuse treatment, and provide specialty medical and wellness services, plus emotional support for both residents and their families.

The center's social life revolves around organized group and individual activities like bingo, animal visits, music, dance performances, and staff dress-up days, alongside day trips and other outings in the nearby area, which includes historic downtown, Veteran's Memorial Park, and Minard's Spaghetti Inn. Amenities span a beauty salon, computer center, dining room, fitness area, game room, small library, garden, outdoor space, and a wellness center. Rooms come fully furnished with private bathrooms, and living areas have air-conditioning, cable TV, Wi-Fi, and telephone services-meal service is offered in restaurant-style dining with options for special diets.

Clarksburg Healthcare Center uses a 24-hour call system for emergencies and features 24-hour support and supervision, with a nurse on site 12 to 16 hours a day and a staff ratio of about 3.36 nurse hours per resident per day. Care services are delivered by skilled nurses and doctors, and staff can help arrange or provide community transportation, with non-medical trips possible. Residents benefit from a bunch of memory activities and programs designed for those facing Alzheimer's or similar conditions. Families and residents alike often play a role in decisions about care and treatment, with the management and staff trying to keep open lines of communication.

The facility is part of CommuniCare Health and has been under Health Care Facility Management, LLC since April 2023. Inspection reports note the center submitted documentation with 31 deficiencies, including issues in timely care planning, food handling, and infection control-one of the deficiencies involved infection-related concerns. The nurse turnover rate is about 52.7%. All staff speak English. Clarksburg Healthcare Center isn't currently taking new residents, but for those who live there, the staff tries to support physical, mental, and social health in a way that fits each person's needs, while involving families every step of the way.
