    Moved mom compassionate organized care

I moved my mom here and have been very pleased - the staff are friendly, helpful, and clearly compassionate, providing professional, family-like care and excellent communication with us. The facility is clean, secure, welcoming and well-organized with activities that helped her settle in; she seems happier and more energetic, and I would recommend it.

    Summary of reviews

Reviewer feedback for Wayne Healthcare Center shows a mixed picture with clear strengths as well as recurring operational concerns. Positively, many families and residents describe a warm, family-like atmosphere and respectful, compassionate interactions from front-line staff. The facility is frequently described as clean and welcoming, with organized admissions processes, helpful transition assistance, and on-site activities including an activity room. Several accounts note professional therapy services and timely updates to families, contributing to overall satisfaction for a number of residents.

Care quality and staff performance are areas of polarization. Numerous reviewers praise individual staff members for going above and beyond, providing patient, respectful, and affectionate care that makes residents feel at home. At the same time, other reviewers describe variability in day-to-day caregiving: inconsistent staffing levels (notably on weekends), delays in responding to call lights and toileting needs, and lapses in meal assistance. These patterns suggest that resident experience can depend significantly on shift staffing and personnel allocation.

Clinical services and safety show both strengths and weaknesses. Therapy and rehabilitation services are available and appreciated by some families, but others report variable therapy effectiveness and a constrained therapy space. Multiple reviews raise concerns about fall-prevention practices and transfer safety, and there are descriptions of delayed incontinence care and delays in attending to basic needs; collectively these indicate opportunities to strengthen safety protocols, response times, and hands-on care consistency.

Dining, activities, and facilities feedback is similarly mixed. The facility environment and cleanliness receive positive comments, and activities/engagement resources are in place. Conversely, meal quality and meal-assistance reliability are noted as inconsistent, and therapy-area size and transportation resources (a facility van) have been called out as limited or unreliable by some families.

Management, communication, and cost issues appear as recurrent themes. While many families praise clear, timely communication from staff, others report gaps in management responsiveness, limited or unsatisfactory follow-up after incidents, and concerns about post-incident family notification. A small number of accounts describe serious injury-related concerns; these accounts highlight the need for clear incident response, documentation, and family outreach. Additionally, some families note high out-of-pocket costs and restricted transportation options as financial and logistical considerations.

Overall, Wayne Healthcare Center exhibits palpable strengths in staff compassion, a home-like environment, cleanliness, and family communication for many residents. However, the facility also shows operational vulnerabilities: staffing variability, responsiveness delays, safety and incontinence-care gaps, and inconsistent therapy and dining experiences. Prospective residents and families should weigh these mixed signals, ask specific questions about staffing ratios (including weekend coverage), fall-prevention protocols, incident-notification procedures, meal services, and the scope of therapy and transportation resources before making placement decisions.

    About Wayne Healthcare Center

Wayne Healthcare Center, also called Allegria Village and Independence Village of Plymouth, sits at 6999 WV-152 in Wayne, WV, and is run by CommuniCare Health under Health Care Facility Management, Llc since April 2023, and while folks say there are some problems with cleanliness and maintenance and inspection reports show 34 total deficiencies-including 2 infection-related-there's a focus on supporting residents and families while offering a wide range of senior care services right from independent living to assisted living and memory care, with a skilled nursing team, aides, experienced physicians, med team, administration staff, and physical and occupational therapists doing their best to meet everyone's changing needs, and there's 24-hour nursing care and a continuum of care setup so folks can stay in their rooms and still get more support as they need it which often helps with memory care especially, including special programs for dementia and Alzheimer's, and there's a Christian atmosphere and supportive neighbors, and you'll find lots of resident and family involvement in planning care which means people can stay connected with both staff and their loved ones. The Home has some nice features like furnished two-bedroom apartments, shared rooms, air-conditioning, kitchenettes, cable, telephones, and private bathrooms, plus common spaces like a dining room-even though sometimes the dining area closes and residents have to help out-alongside a garden, outdoor areas, fitness and wellness rooms, gaming and computer areas, beauty salon, small library, and the apartments themselves have amenities for comfort. They've got scheduled transportation and arrangements so people can get to appointments or outings, and support with daily needs, medication, mobility help, and help for folks with complex medical conditions, and even though staff shortages can make aides seem impatient at times, and nurse turnover is 43.5% which is lower than the state's 48.5% with 3.54 nurse hours per resident per day slightly below average, people find a supportive environment and staff do what they can with limited hands. The Department of Social Workers helps residents and their families adjust, while a med team handles medical management and records, and there are modern rehab facilities with therapy focused on physical, occupational, speech, and emotional health, aiming for both short-term and long-term rehab, wound care, hospice, palliative, and neurorehabilitation, always personalizing the plans. There are meal services with registered dieticians handling nutritional needs and kosher diets and they try for healthy options, and concierge and move-in coordination lend some comfort, and the center puts a priority on keeping people safe with secured areas for memory care, 24-hour security, and emergency call systems in place. People can take part in social activities such as bingo, ice cream socials, holiday parties, themed events, religious services, special games, planned day trips, and sometimes even things like fishing or shopping excursions, and a lot of activities are resident-run which adds to the community feel. Even though past inspections pointed out a handful of infection control issues and upkeep challenges, Wayne Healthcare Center accepts Medicare and Medicaid and handles all kinds of skilled nursing, home care, respite care, and hospice services with a focus on making the experience comfortable and dignified for everyone, and families stay involved and updated if anything affects their loved one, and there's a general atmosphere of friendly care for seniors with many different needs.
