    Compassionate professional care and recovery

I'm very pleased with Pleasant View - the staff are loving, compassionate and professional, the facility is clean and well-maintained, the food is good, and rehab services got my family member back on their feet. They hosted events, stepped in like family when needed, and made our loved one feel cared for and happy, so I wholeheartedly recommend them.

    Summary of reviews

Pleasant View Nursing Home presents a mixed but generally positive profile. Many families highlighted compassionate, attentive nursing and support staff who provide hands-on help and emotional support; reviewers repeatedly noted staff stepping in for absent family members and personalized attention during rehabilitation. The facility has a strong focus on short-term rehab and physical therapy, with multiple first-hand accounts of successful recovery after orthopedic procedures. Overall cleanliness and recent facility updates were praised, and the property’s country setting and outdoor space were described as attractive features.

Dining and activities are strengths for many residents. Several reviewers described meals as tasty and varied, and the facility hosts a range of social and recreational programming — line dancing, craft shows, birthday parties and other events were mentioned — contributing to resident engagement and social wellbeing. The community is pet-friendly and appears to offer ample space for activities, which supports a positive day-to-day experience for many occupants.

Despite these positives, a number of operational concerns recur across reviews. Communication with families is described as inconsistent at times, and staffing constraints appear to produce longer wait times for assistance. Some families reported limited physician availability and gaps in off-hours clinical coverage. There are also concerns related to medication-management procedures and consent documentation; one serious family-level allegation regarding opioid administration and subsequent clinical changes has been raised, and some reviewers expressed intent to seek legal counsel. These items suggest families should proactively review medication protocols, consent paperwork, and escalation pathways during admission.

Management and administrative impressions are mixed but contain professional elements: several reviewers praised administrative responsiveness and the facility’s organization, while others noted conditional satisfaction tied to the communication and clinical-response issues above. Prospective residents and families may benefit from in-person inquiries about staffing ratios, physician coverage, medication-consent procedures, and daily communication practices. Observing a therapy session, reviewing recent activity calendars and sample menus, and asking for clarification on visitation policies during infection-control periods can help determine whether Pleasant View’s operational fit matches a prospective resident’s needs.

    About Pleasant View Nursing Home

Pleasant View Nursing Home sits on WI-81 in Monroe and has served seniors as a non-profit care community owned by Green County since 1985, offering 96 certified beds and hosting around 65 residents each day, so you see some rooms are open and some have a person in them, and the staff mix includes a Director of Nursing, Dietician, Social Worker, Business Manager, Billing/Administrative Assistant, Environmental Services Supervisor, Administrator, plus a friendly team focused on comfort and healing, though it's worth pointing out the place has a nurse turnover rate of 47% and logged 42 deficiencies on a recent inspection, with some important ones about infection control and reporting or responding to suspected abuse, neglect, or theft-none listed caused actual harm at the time, but the state noted a real risk.

The nursing staff offers 4.22 hours per resident per day, which means people get a fair bit of staff time, especially with long-term care, short-term rehabilitation, and a range of therapy services including physical, occupational, and speech therapy, and there's a special memory care unit for dementia called the Dementia Stabilization Unit, the first one of its kind in Wisconsin, so folks in crisis with dementia have support and families get help navigating tough spots. The place has been updated in recent years to make things a little brighter and easier to get around, with pleasant dining rooms, big common spaces for family visits and socializing, outdoor gardens, an aviary where little birds sing, private and semi-private rooms, beauty and barber shop services on site, and even a cozy lounge so folks can unwind or visit on quiet days.

Personalized care plans aim to cover each resident's unique needs and encourage independence and dignity, and the facility sets up a Life Enrichment program plus plenty of planned activities, religious services, transportation, chaplaincy, laundry for personal items, daily meals prepared for dietary needs, and an option for hospice and respite care too, if needed. Volunteer help is welcome for all sorts of activities and residents are treated like family-though, again, it's good to remember the place has had issues in the past with infection control and reporting safety concerns, but the focus remains on compassion, steady support, and giving seniors a sense of purpose as they age in a comfortable and supportive space that tries hard to meet a wide range of healthcare and social needs.
