    Warm clean caring faith-based community

I visited with my grandma and would be comfortable having her live there long-term. The place is warm, meticulously clean, faith-based and church-owned, with encouraging scripture on the walls and staff who truly minister to residents. Staff are caring, proactive and available 24/7; residents are happy and social with good food, camaraderie at meals, games, music and church service. Bright, beautiful apartments (full kitchens), an on-site hairdresser, attentive maintenance, varied housing options and a great location make it affordable, welcoming, and well worth recommending - I'd feel privileged to work there.

    Summary of reviews

Pine Haven Christian Communities — Giddings Avenue Campus presents a mixture of strengths and operational inconsistencies. Many descriptions emphasize a warm, faith-centered atmosphere with active spiritual programming, welcoming common spaces, and well-kept landscaping. The community offers a wide range of housing choices (including duplexes, apartments, assisted living, and memory care), private apartments with full kitchens, and on-site conveniences such as a hairdresser. Several families praised short-term rehabilitation services, the social life around meals, and a general sense of camaraderie among residents.

Care quality is mixed. Certified nursing assistants are frequently described as compassionate, attentive, and a clear asset to daily resident care. In contrast, concerns center on variability among licensed nursing staff and gaps in clinical oversight. Specific operational weaknesses include inconsistent adherence to medication orders and problems with following prescribed comfort or end-of-life regimens. Several accounts also indicate delays in responding to care requests and call bells, which contributes to perceptions of uneven responsiveness.

Dining and activities are generally seen as positive elements. Reviewers noted good food, communal dining experiences that foster social interaction, and a variety of activities — including games, music, and regular church services — that support resident engagement and independence. The campus layout and proximity to local amenities (restaurants and parks) were highlighted as conveniences for families and residents.

Facility condition and services show a divide. Many reviewers describe bright, clean apartments and proactive maintenance staff, but others raised sanitation concerns in certain rooms and common areas, including unreliable laundry and housekeeping coverage. These inconsistencies appear to vary by unit or time period rather than reflect uniform conditions across the campus. The landscaping and exterior upkeep are consistently noted as strengths.

Management and communication present another area of mixed feedback. Some families describe proactive, helpful staff with prompt updates on unusual events and 24/7 availability, while others experienced unprofessional or uncompassionate administrative interactions, denied visitation or rigid enforcement of policies, and poor follow-through on family concerns. A few accounts raised concerns about hospice coordination and how end-of-life care was managed.

Notable patterns: the campus tends to polarize experiences — strong praise for direct-care staff, community life, amenities, and affordability sits alongside recurring operational issues in clinical oversight, medication processes, housekeeping, and family communication. Prospective residents and families would benefit from an on-site tour that specifically reviews current nursing oversight, medication-administration protocols, housekeeping and laundry schedules, call-bell response metrics, hospice coordination, and the facility's visitor policies to understand how those operational areas are managed today.

About Pine Haven - Giddings Avenue Campus

Pine Haven Christian Communities Giddings Avenue Campus sits on Giddings Avenue in Sheboygan Falls, Wisconsin, and belongs to the larger Pine Haven Christian Communities network, which means the place usually gets known for its focus on Christian values and a sense of community, and when you visit, you'll notice it's pretty big, with space for as many as 140 residents in various studio apartments, so there's always someone around but it doesn't feel too crowded either. The community offers different care types, including independent living, supported independent living, assisted living, memory care, long-term skilled nursing, respite care, palliative care, hospice, and rehabilitation, so people can find the care that fits their needs, from folks getting by on their own to those needing more help or even specialized memory care for early and mid-stage memory problems, and they've got daily activities, exercise rooms, walking trails, and an aviary you can visit, and then there are raised garden beds where residents can get their hands dirty, a walk-in spa bath for relaxation, and even a hair salon and barber, just like back in the day, and dining facilities with flexible meal times, plus The Corner Store. You might see residents heading to the chapel, which is important to many in a Christian community, or taking the campus-provided transportation for outings and errands. The place has a care questionnaire to help figure out what someone might need, and if you're looking at services, they offer everything from therapy and rehabilitation to dental, eye, podiatry, audiology, and they work with community groups and have their own events, plus they share news and updates so people can stay engaged. Out of the three senior living communities in the city, this campus stands out with an average rating of 7.9 out of 10, which is the highest in Sheboygan Falls, and that's worth noticing if you're comparing options, and really, the campus just tries to fit a lot of different needs under one roof, focusing on a resident-centered approach and supporting seniors through changes with as much dignity and comfort as they can.
