    Compassionate skilled staff; excellent care

I felt my loved one received compassionate, highly skilled care - nurses, CNAs and therapists helped him make real rehab progress and kept him comfortable. The staff went above and beyond, treated us like family, were responsive to every need, and the meals and facility were excellent. I'm grateful for their support and would recommend this place.

    Summary of reviews

Reviews of this facility present a mixed picture: there are many accounts of attentive, compassionate frontline caregivers and successful rehabilitation outcomes alongside multiple reports of operational and clinical consistency problems. Strengths most frequently cited include caring nurses and CNAs, effective therapy that produced measurable functional improvement for some residents, and an engaged social-work presence that advocates for families. Several reviewers also praised cooperative hospice coordination, pleasant common areas with activities and Wi‑Fi, and assistance from staff on administrative and Medicare-related questions.

Care quality and staffing appear uneven. Positive experiences emphasize staff who go above and beyond, gentle personal care, and therapists who delivered strong progress. Conversely, other accounts describe variability in staff conduct and communication tone, delayed responsiveness—especially on later shifts—and episodic delays in medication administration or pain control. The social-work and case-management function is consistently noted as a strength, with specific staff singled out for effective advocacy and family communication.

Dining and activity programming receive mixed feedback. A number of families describe enjoyable meals and a cooperative kitchen team, and communal spaces are viewed as pleasant and activity-friendly. However, a subset of residents experienced weight loss during stays and some families described dissatisfaction with meal quality or portioning during rehabilitation, suggesting inconsistent dining outcomes across stays.

Facility and direct-care processes show notable inconsistency. Several reviewers raised sanitation concerns, inconsistent linen and bathing routines, and shared-bathroom arrangements that contributed to incontinence-care concerns and odor issues in areas near resident rooms. These are contrasted by other accounts that described outstanding housekeeping, indicating variability between units or shifts rather than uniform facility condition.

Administrative and clinical oversight issues also recur. Families reported documentation gaps, clinical-monitoring inconsistencies (including concerns that clinical red flags were not consistently trended or escalated), lost or mishandled medications, and billing errors or disputes. Some reviewers described difficulty reaching management or slow follow-up on complaints. There were also mentions of regulatory citations, probation, and concerns following a resident's death, which suggest prospective families should verify current inspection and enforcement status with regulators before placement.

In summary, the facility demonstrates clear strengths in compassionate bedside care, effective individual rehab episodes, and strong social-work support, but these are offset by recurring operational weaknesses: inconsistent cleanliness and personal-care routines, variable staff professionalism, scheduling and coverage gaps for therapy, and administrative controls around medications, billing, and documentation. Prospective residents and families should meet key staff (nursing leadership and social work), review therapy schedules and weekend coverage, ask for current inspection reports and medication/billing policies, and clarify linen, bathing, and incontinence-care protocols to assess whether the facility’s current performance aligns with their priorities.

    About Sheboygan Progressive Health Services

Sheboygan Progressive Health Services sits in Sheboygan and has provided care since 1965, offering both short-term and long-term help for people who need assisted living, rehabilitation, skilled nursing, or memory care. The team here is made up of dedicated nurses and professionals who follow set systems and rules to make sure residents get steady care, though their nurse staffing hours are 3.10 per resident each day, which falls below the average for the state, but nurse turnover is at 29.3%, so staff tends to stay longer than at most other places. This place has 50 certified beds and sees about 43 residents each day, and it's affiliated with North Shore Healthcare. It holds a Bronze Commitment to Quality Award from 2019. They run a Guardian Angel program aimed at building trust and getting residents more involved in their care.

The range of health services is broad, including daily housekeeping, giving out and checking medicine, building upkeep, and helping families through support services. Residents can receive physical, occupational, and speech therapies, plus there are specialty services like wound care, IV fluids, infection control with isolation as needed, dialysis transportation, psychiatric consultations, and a comprehensive dining program. The facility uses tools like a life vest, automatic defibrillator, an emergency kit, in-house x-ray, EKG, venous doppler, and bladder ultrasound, and there's a full pharmacy service right there. There are also activity services, discharge planning, daily weights, glucose checks, respiratory therapy, swallow studies, swallow incentives, and in-house lab testing. A licensed social worker is part of the staff.

Inspection records show a mix of feedback. The Centers for Medicare and Medicaid Services rates this place highly, giving it five stars, and at one point it had a deficiency-free rating, but recent official inspections reported a total of 28 deficiencies, including infection-related issues and problems with abuse prevention policies and pharmacy oversight. So, while there are strong efforts on quality, it's clear no place is perfect and inspection findings show there's room for improvement.

All in all, Sheboygan Progressive Health Services stands as a long-serving healthcare spot with a broad mix of care and rehabilitation services, unique programs like the Guardian Angel, and a dedicated team, but it's also had some serious care issues documented by state inspectors, so families might want to look at the reports when considering care options.
