I moved my mom into Rose Garden and I'm very pleased - the units and common areas are clean, rooms spacious and home-like, and the grounds are beautiful. The staff and administration are caring, family-focused and responsive, they handle medical concerns quickly and truly go above and beyond. Activities are engaging and varied (dancing, crafts, gardening, church, games), residents stay active and social, and the food/nutrition has been good - I would recommend this community.

Summary of reviews
Reviewers describe a facility with clear strengths and notable weaknesses. Positive comments consistently highlight the campus and outdoor spaces: landscaped grounds, a central courtyard and garden-style layout are repeatedly praised and appear to support outdoor activity and resident autonomy. Many families compliment the activities program, citing music, dancing, crafts, memory games and weekly worship services as meaningful daily engagement. Multiple accounts indicate the community operates a dementia-focused program with memory-oriented activities and partnerships (including a PACE relationship) that some families find beneficial.
Staff performance is reported variably. Numerous reviews emphasize compassionate, friendly and helpful direct-care staff, attentive caregivers, and instances of responsive nursing or administrative leadership who provide individualized communication and family-focused service. At the same time, other reviewers describe younger or less experienced staff, long gaps between caregivers, and frequent leadership turnover. A recurring operational weakness is staffing levels: understaffing and high turnover are linked in reviews to delayed assistance, rushed care, and inconsistent follow-through on personal-care tasks.
Care quality and safety impressions are mixed. Positive accounts note attentive care, good nutrition and improvements in resident social engagement. Conversely, several reviews raise concerns about supervision for residents with behavioral risks, inconsistency in personal-care routines (including bathing and clothing changes), and sanitation or odor concerns in specific areas. There are also multiple descriptions that point to gaps in clinical-incident response and family notification following falls or other events, as well as reported problems around medication and personal-item management. These accounts suggest variability in clinical oversight and operational reliability across shifts or units.
Dining and daily life present contrasts. Many families praise the food quality and portion sizes and describe active communal dining and social opportunities. Other reviewers find the dining room large and noisy, with an institutional feel when all residents eat together. Transportation and organized outings are available in some instances but are described as limited or inconsistent by other families.
Facility condition is similarly mixed: reviewers note updated rooms, hardwood flooring and clean common areas, while others point to older building sections undergoing renovation, small shared rooms with limited privacy, uneven walkways and localized sanitation issues. Housekeeping receives both praise and critique, indicating variability across units or time.
Management and administrative themes include both strong, communicative leaders and instability or variable responsiveness. Several families commend administrators and nurses for being proactive and family-oriented; other accounts describe frequent leadership turnover, difficulty resolving billing or discharge questions, and instances where family communication around incidents was inadequate. Billing and discharge coordination are specifically identified as an operational pain point in some reviews.
Overall pattern: the Rose Garden Residential Care environment offers notable strengths in outdoor spaces, activity programming and, for many residents, compassionate direct care and good meals. At the same time, patterns of understaffing, inconsistent cleanliness and supervision, variability in incident communication, and room/privacy limitations create uneven experiences. Prospective families should verify current staffing ratios, observe dining and activity periods, ask about incident-notification protocols and review room configurations and housekeeping practices during a tour to assess suitability for a particular resident's clinical and safety needs.

About Rose Garden Residential Care
Rose Garden Residential Care sits on a quiet two-acre property in Mentone, California, offering both assisted living and memory care, and it's got space for up to 63 residents, so you won't feel crowded but you'll have people around if you want to talk or join in with things, and there's always something going on since the place is known for its active schedule with stretching classes, art groups, movie nights, gardening clubs, music programs, Wii bowling, and even karaoke, and there's an activities director who plans these things out including intergenerational programs with younger folks visiting. The building has indoor and outdoor common areas, so you can relax inside or take a stroll along the secure walking paths in the gardens, and there are amenities you'd expect like a game room, a library that stays stocked with books, a spa, and spaces you can use for visits with family or other residents.
Rooms come in a few styles-semi-private, private, or one-bedroom options with average shared rooms starting at $3,800 a month and private rooms between $4,200 and $4,500, with care fees that go up if you need more help than basic support, and they have aging in place so you can stay if your needs change as you get older, which is something folks appreciate since moving can get hard over the years. The staff is around 24 hours a day with a nurse on staff and a doctor on call, and they can handle most care needs whether that's help with bathing and dressing, medication management, blood sugar checks and insulin shots for diabetes, incontinence care, mechanical lifts for transfers, or reminding you to get to meals or appointments. There's also help for residents who might wander or have more challenging behaviors or need special memory care, since they've got a secured memory care building out back, bracelets that set off alarms at exits if someone is likely to get lost, and staff trained to work with dementia and Alzheimer's-plus, they run memory programs like reminiscence activities and have dementia waiver services.
Meals get cooked by a professional chef and you can eat anytime, and they do special diets like gluten-free, vegan, low salt, or low sugar, and yes, they'll make sure the food is both healthy and tasty, and if you want, your guest can have meals too for an extra fee. Pets are welcome, and the place is non-smoking everywhere indoors. When you need to go somewhere, there's complimentary transportation for some trips, though longer rides may cost extra. You'll see visiting podiatrists, physical, occupational, and speech therapists, along with dentists and nurses who drop by, but there's also a wellness room on site, and they do fitness classes and nutrition talks, so there's help with both body and mind. Hospice and respite care are available, and families often come for visits thanks to the pleasant walking paths and activity rooms.
Residents can get medical equipment as needed, and care works on a light, medium, and heavy level system, making the support pretty customizable-staff can also help with reminders for toileting, grooming, or more complex medicines, and if you have behavioral changes, there are services for that as well. The place is wheelchair accessible, most rooms have full bathroom tubs, and the activities stretch across seasons with birthday celebrations, milestones, and events tailored to what residents actually like, not just what's on a list. The facility is licensed by the state of California, license number 366426422, and has a 3.5 out of 5-star rating from reviews, so it's considered pretty good, and people mention that while it's not the fanciest, it's comfortable, clean, friendly, and feels like home with enough extra touches to make daily living easier and more social, with the goal always being to help residents stay as independent and active as possible for as long as they want.
