    Clean compassionate facility; excellent care

I'm very happy with this clean, well-kept facility - remodeled rooms with private baths and walk-in showers, accessible entrance, pleasant grounds, and a varied, delicious menu. The staff are compassionate, patient and attentive, with very good PT/OT and medical teams, responsive 24/7 care, frequent engaging activities, and accessible administration; I'd recommend it to families looking for dignified, reliable care close to home.

    Summary of reviews

Reviews present a mixed but consistent picture: many families praise the facility's clinical strengths and activity programming while others raise operational and maintenance concerns. On the positive side, reviewers frequently highlight compassionate, professional nursing staff and available clinical leadership (APRN/physician on site). Rehabilitation services receive commendation in multiple accounts, with specific praise for PT/OT teams and an active recreation director. Several reviewers describe clean, recently remodeled common areas and private rooms with walk-in showers, an accessible entrance, and well-kept grounds. Dining and programming are also commonly cited strengths — a varied menu, accommodation for special dietary needs, frequent activities, music, games, and social events contribute to positive resident experience for many families.

At the facility level, recurring operational weaknesses emerge. Staffing levels and workload pressures are described as an ongoing issue, which reviewers connect to delayed assistance for personal care, long waits for bathroom help, and inconsistent bathing or grooming schedules. Relatedly, reviewers report gaps in care coordination and medical-record documentation, including missing orders for appointments and difficulties reaching staff, which can disrupt planned care and appointments. Several accounts raise medication-management concerns including inconsistent medication administration and polypharmacy issues; these constitute a notable safety-related theme.

Facility maintenance and sanitation are another area of divergence. While some wings and public areas are described as well maintained, other reviews describe sanitation concerns and odor issues in specific rooms or bathrooms, intermittent hot-water or plumbing problems, and missing or damaged fixtures. These issues point to uneven upkeep across the facility rather than uniform conditions. Billing and financial transparency is a separate and substantive pattern: reviewers describe undisclosed charges, billing disputes, and dissatisfaction with how financial questions were handled, which is especially salient given the high monthly cost cited by multiple families.

Prospective residents and families will find both strengths and risks in these reviews. The facility appears capable of delivering high-quality nursing care, meaningful rehabilitation, and an active social program, and several reviewers report dignified end-of-life care and responsive clinical personnel. However, the persistent themes of staffing shortages, care-coordination lapses, medication-management concerns, inconsistent maintenance/cleanliness, and billing transparency issues suggest operational variability that could affect individual experiences. When evaluating this facility, families may wish to confirm current staffing ratios, medication-safety policies, recent inspection or complaint records, the consistency of housekeeping and maintenance in the specific unit under consideration, and written billing policies to assess whether the aspects important to them are reliably delivered.

    About Apple Rehab Saybrook

Apple Rehab Saybrook sits on Boston Post Road in Old Saybrook, Connecticut, with 120 beds for short-term rehab, long-term care, respite care, dementia, hospice, and even outpatient therapy, and you'll find plenty of hotel-like comforts, like renovated rooms, flat screen TVs, wireless internet, and fine dining, all mixed in with caring staff, but if you want to know about how they operate, you'll see a team made up of doctors, dietitians, case managers, social workers, certified nursing assistants, a therapeutic recreation director, spiritual support staff, licensed nurses around the clock, and consulting specialists. There's an expansive rehab gym with new equipment and therapeutic tools, and the place is known for short-term rehab, helping folks after things like surgeries, strokes, and injuries, and specialists provide physical therapy for walking, balance, and exercise, occupational therapy for daily skills and splinting, and speech language therapy for swallowing and communication, including memory programs, and verbal and nonverbal techniques. Rooms are set up with electric beds, private options, and meals with assessment and training, plus family support groups and help for both residents and families, but there's a bit more to the picture, as inspection reports list 44 deficiencies, with citations for infection control, antibiotic monitoring, and foot care issues, so while the nurse turnover rate is below state average and there's a full team, nursing hours per resident are a bit lower than most in the state. Management has been steady with Ryan Vess since 2013, and Brian Foley owns all of it, plus the place follows physician-driven care, has discharge planning, respite for caregivers, and supports people with complex and chronic care needs, acting as part of the Connecticut Association of Health Care Facilities, sitting in warm, gracious settings that aim to feel comforting and supportive right here in Old Saybrook.
