    Friendly staff, smooth, comfortable transition

I had a very positive experience - the staff were friendly, caring and went above and beyond, management and caseworkers were responsive, and admissions were smooth. The therapy team, clean building, good meals and engaging activities made my transition comfortable, and I'd highly recommend this community.

    Summary of reviews

Reviewer feedback for First Coast Health & Rehab is polarized, with families describing both reliable, compassionate care and a range of operational concerns. Positive comments emphasize caring direct-care staff, experienced CNAs, an active therapy department, and an engaging activities program. Several families praised responsive managers, helpful business-office or casework staff, smooth admissions transitions, and meal quality. These strengths suggest the facility can and does provide supportive social, rehabilitative, and daily-living services for many residents.

At the level of clinical care, experiences vary. Some reviewers described effective medication management and care plans that supported safe stays, while others described missed medication timing, delays in feeding assistance, and situations that led to hospitalization or respiratory complications. There are also reports of delayed or limited emergency response and concerns about clinical knowledge or care continuity in certain shifts. Overall, the pattern indicates that clinical reliability may depend on staffing and shift-level consistency.

Staff behavior and management impressions are mixed. Multiple accounts highlight long-tenured, compassionate CNAs, an engaged activities director, and managers who communicate promptly and provide supportive follow-up. Conversely, other accounts describe disengaged or dismissive staff, poor interpersonal tone, and difficulty obtaining timely or transparent updates. High turnover and reported staffing shortages appear to contribute to uneven responsiveness and variable resident experience across days and units.

Dining and rehabilitative services were frequently mentioned. Many families enjoyed the food and noted that social and recreational programming is active and well-run. At the same time, reviewers raised concerns about promised therapy not being delivered on schedule, delays in assistance with transfers or meals, and policies that can create late in-room meals. These indicate variability in adherence to therapy schedules and staffing impact on mealtime assistance.

Facility condition and safety present contrasting impressions. Several reviewers described clean, attractive interiors and smooth admissions areas; others raised sanitation concerns, odor issues in some locations, and pest-control questions. Physical-safety concerns were also noted, including missing bed alarms or rails, broken equipment, crowded rooms, and transfer or fall-prevention gaps. Additionally, multiple accounts mentioned missing personal items and cash, suggesting weaknesses in property controls and asset security.

Management and communication show a split pattern: some families experienced prompt, compassionate management and practical assistance (including help with end-of-life logistics), while others described unresponsive administrators, poor discharge or transport coordination, and opaque communication about clinical events. There are also serious claims about missing belongings and cash attributed to staff, which families raised as a major trust issue.

In sum, First Coast Health & Rehab demonstrates clear strengths in caregiving, therapy, and activities for many residents, but reviewers also describe significant variability in staffing, communication, sanitation, safety practices, and asset controls. Prospective residents and families would benefit from an on-site assessment focused on staffing levels by shift, fall-prevention measures, property-security procedures, therapy schedules, and current sanitation and pest-control practices to determine whether the facilityโ€™s strengths align with their expectations and clinical needs.

About First Coast Health & Rehab

First Coast Health & Rehab sits in Jacksonville, FL, and provides skilled nursing care for people who need medical help or rehabilitation, and the place usually has about 91 residents each day, with 100 certified beds, and it's open all day and night, every day of the week. The facility offers long-term care, short-term rehabilitation, memory care for people with cognitive issues, and post-acute services, so people can get different kinds of help depending on what they need. Residents can join programs that aim to keep them active and healthy while supporting their emotional well-being, and there are outdoor spaces set up for relaxation and meeting with others, plus activities and social programs going on regularly.

They give care around the clock and have Registered Nurses (RN) and Licensed Practical Nurses (LPN) on staff, although the nurse staff turnover rate is high at 56.8%, which is more than the state average, and nurses spend an average of 3.37 hours per resident per day, which is less than the state average of 3.9 hours. Inspection reports have found issues in the past, mostly about not having enough nurses on each shift and some problems with the building's safety, cleanliness, and comfort for everyone inside, which means sometimes things have fallen short in these areas.

The facility offers physical, occupational, and speech therapy, along with restorative and palliative care, respite care, physician services, and help with daily living. You'll also find specialized care programs, social services, some nutritional guidance, housekeeping, and laundry. Modern amenities try to make residents comfortable, and the place is set up to give care that's personalized and coordinated for each person. The direct owner is Senior Health First Coast LLC, and the facility is connected to Senior Health South. Leadership says they're dedicated to the community's needs and want to promote dignity, compassion, communication, and respect, aiming for an environment where people can keep learning and feel included.
