Warren Park Health & Living Center at 6700 N Damen Ave, Chicago, IL 60645, opened in 2016, runs day and night to care for up to 127 residents, and most days sees about 124 people living there, so it's nearly always full, and the staff is there all the time, including registered nurses, LPNs, CNAs, therapists, and physicians, who work shifts of 12-16 hours and help with everything from wound care, medication, and bathing to moving around and eating, and though the nurse turnover rate is lower than most places in Illinois, the total nursing hours per resident per day are lower than average, and the staff help with skilled nursing, assisted living, memory care, home health, independent living, and even short-stay respite or hospice-plus there's post-acute and inpatient rehab, and a big focus on stroke recovery, orthopedic, cardiac, oncology, speech, respiratory, pain management, wound care, and palliative needs, with special programs like the Interventional Behavior Medical Complex Care Program that tracks several conditions and aims for medication management and psychosocial rehabilitation to keep up recovery, and while it tries to make folks feel at home with things like a spa, fitness room, game and activity rooms, library, gardens, walking paths, and comfortable rooms with their own bathrooms, cable TV, Wi-Fi, air conditioning, and personal kitchenettes, guests also get recreational, music, and art programs, movie nights, and different activities and outings organized by dedicated staff, and the center does transportation-including medical trips-housekeeping, laundry, move-in help, emergency alerts and even parking, so there's a lot to cover each day. Besides all that, the facility's run by Citadel Healthcare and managed by Damen Healthcare Group LLC, is a for-profit business, takes Medicare and Medicaid, and doesn't have its own resident or family councils, and although the staff are known to be empathetic and focus on personalized care, the place has had documented issues in the last year, including an abuse flag, infection-related and environmental pest control deficiencies, and citations for nutrition and dietary problems like not always getting and storing food the right way-so health inspections gave it a B-minus, which isn't top marks but not the worst either, and despite these concerns, folks who need help managing serious health concerns after surgery, illness, or a hospital stay, including non-ambulatory needs, often go here because of the range of therapies and medical help, and the team includes a social worker to help meet everyone's preferences and needs, always aiming to get people feeling better and back to as much independence as possible, even though the staff hours are on the low side, and everything is mostly in English, with special support for those needing help with transfers or everyday tasks.
