    Exceptional care, friendly staff, welcoming

I'm very pleased with my experience here. Staff are friendly, professional and attentive - therapy and rehab are excellent, activities are engaging, and residents genuinely seem at home. The building is bright, clean, and being nicely updated, and ownership/management are responsive. I would recommend this facility.

    Summary of reviews

Reviews for Azria Health at Montclair are mixed, with clear positive impressions of the physical environment and certain clinical services alongside serious operational concerns. Several reviewers highlighted friendly, welcoming frontline staff, a generally clean and odor-free facility, private-room availability, and rehabilitation/therapy services that families found beneficial. At the same time, recurring themes raise questions about the facilityโ€™s consistency in delivering safe, reliable care.

Care quality shows a split pattern. On the positive side, therapy and some nursing encounters were described as effective and supportive. However, a set of operational weaknesses was noted that directly affect clinical reliability: inconsistent medication administration and gaps in medication documentation, missed or incorrect treatments, and failures in executing prescribed respiratory therapies. There are also concerns about clinical monitoring for higher-risk residents, including fall-risk oversight and infection-control processes. A small number of accounts referenced serious clinical consequences, hospitalizations, and legal action; these appear to be individual but significant events that prospective families should investigate further through regulatory records and direct questions to management.

Staffing and communication present contrasting impressions. Multiple reviewers described staff who were friendly and greeting residents with smiles, yet others reported unresponsiveness to call systems, abrupt or discourteous interactions, and difficulty reaching staff when needed. Reviewers also raised staffing-level concerns consistent with high turnover and short-staffing, which can amplify delays in care and weaken continuity. Families should ask about current staffing ratios, turnover rates, and the facilityโ€™s processes for shift handoffs and after-hours response when evaluating placement.

Dining and ancillary services receive mixed feedback. Some residents found clinical care acceptable but disliked the food; others indicated meal quality and satisfaction were inconsistent. Therapy services and rehabilitation received comparatively positive remarks, with some families indicating they would return for therapy. There is limited specific feedback about activities programming in the provided summaries, so activity availability and engagement should be confirmed directly.

Facilities and management are similarly mixed. The physical environment and cleanliness were cited positively by several commenters, but management stability and organizational health raised concernsโ€”reviews referenced facility restructuring, potential financial instability, and legal exposure. These items suggest that operational oversight and long-term continuity may be variable. Prospective families should request information on the facilityโ€™s recent inspection history, any active complaints or legal proceedings, and plans for operational improvements.

Recommendations for prospective residents and families: conduct an in-person tour, observe staffing levels at different times of day, ask for written policies on medication administration and infection control, request recent inspection reports and staffing ratios, and speak with current families about responsiveness to calls and clinical monitoring. The facility offers strengths in environment and therapy services, but the operational issues identified warrant careful vetting before placement.

    About Azria Health at Montclair

Azria Health At Montclair sits in Omaha, Nebraska, at 2525 S 135th Ave, and serves as a skilled nursing and rehabilitation center with a focus on senior care, and while folks will find it large and busy, the heart of the place is really about helping people stay as well and safe as can be. It's got memory care services for people with Alzheimer's and dementia, with plenty of safety features and personalized routines, and the nursing home care offerings support long-term needs along with rehabilitation services for those just out of the hospital or needing extra recovery, and the staff covers everything from help with transfers, bathing, dressing, and other daily activities to managing medicines, so residents get support twenty-four hours a day.

There are 175 beds in all, though only 56 are certified for Medicare and Medicaid as of June 2025, and the staff includes people who speak English and sometimes other languages, which can make things smooth when someone needs to explain complicated care details. Residents can stay in studio room layouts that are furnished and offer private bathrooms, cable TV, kitchenettes, phones, and air conditioning, with Wi-Fi and high-speed internet everywhere in the building. Azria Health At Montclair also has transportation and parking services for outings and appointments, plus laundry and housekeeping help to lighten daily chores, and residents even get move-in coordination to help adjust to life here.

For food, the community provides restaurant-style dining with meals cooked by a professional chef, and dietary needs get attention, with meal options for allergies, diabetes, or just different schedules thanks to all-day dining. Amenities include a community kitchen, common areas with outdoor spaces for folks who enjoy fresh air and visiting, a spa and wellness room, a fitness room with planned exercise programs, and activity and arts rooms, a library, walking paths, and an emergency alert system for safety. There are both community-planned and resident-led activities like movie nights and music, so social days are common.

Medical care here covers skilled nursing, rehabilitation, and advanced recovery needs, with therapy services like physical, occupational, and speech therapy, and specialized care programs for stroke, neurology, orthopedic needs, prosthetic training, pulmonary rehab, cardiac recovery, and wound care, all set up with detailed, personalized care plans. Discharge planning and family caregiver resources aim to keep families involved and support ongoing health and wellbeing.

Azria Health At Montclair doesn't accept new patients right now. It holds a community rating of 6.3 out of 10 and a nursing home rating of 1 star from the Centers for Medicare & Medicaid Services, while online it gets a rating of 3.4 out of 72 reviews, which shows the experience can vary. The environment combines medical care with programs for engagement, but like anywhere, it's important for families to consider ratings and offerings when weighing options for their loved ones.
