    Clean, well-maintained rehab; caring staff

I placed my grandmother at Rex Rehab and was generally pleased. The facility is clean, spacious, well-maintained and feels new, with a nice courtyard and secure entry. Nursing and Spanish-speaking staff were professional, kind, attentive and communicative, and physical therapy was effective. Residents stayed in good spirits - I would recommend Rex Rehab.

    Summary of reviews

Overall impression: Reviewers present a mix of strong clinical rehabilitation services and operational concerns. The facility is generally described as clean, modern, and pleasant to visit, with a physical environment that includes a well-kept courtyard, secure entry, and welcoming common spaces. Many families praised the rehabilitation program and individual therapists for effective therapy and functional gains, and several accounts characterize the nursing staff as professional and hardworking. Bilingual (Spanish-speaking) staff and staff who assist with care coordination and access to services are noted as meaningful strengths.

Care quality and clinical patterns: Physical and occupational therapy are consistently highlighted as effective, and some families described positive rehab outcomes. However, reviewers also described inconsistencies in clinical care and follow-through. One reviewer raised concerns following a resident's death that related to catheter management, infection monitoring, and antibiotic effectiveness; that account also cited a lack of laboratory documentation to confirm infection clearance. These comments point to potential gaps in infection-management documentation and in consistency of clinical monitoring. Separately, some reviewers questioned the clinical justification for extended rehabilitation stays, suggesting unclear discharge criteria or utilization oversight.

Staff and family communication: Multiple reviews praise attentive, communicative caregivers and staff who engage positively with families. At the same time, there is notable variability in staff conduct and tone; comments describe unkind or dismissive interactions and periods when staff appear inattentive. Delays in responding to call signals and in assisting with toileting or transfers were described, indicating responsiveness and staffing consistency are areas for improvement. A few reviewers described perceived lapses in frontline managerial oversight tied to visible staff behavior.

Dining, activities, and resident experience: The physical dining space and communal areas receive positive mentions, and the resident community is often described as upbeat or socially engaged. However, food quality and meal satisfaction are recurring negatives; reviewers described the menu or meal preparation as bland or unsatisfactory. There is less detailed feedback about structured activities, though the social mood among residents was often reported as positive.

Facilities and operations: The building and grounds are generally seen as an asset โ€” clean, well-maintained, and modern in appearance, with a secure entry and pleasant outdoor space. Operational concerns center on inconsistent staffing quality across shifts, variable bedside manner, and gaps in clinical documentation and utilization processes. These operational patterns suggest opportunities for stronger clinical oversight, clearer discharge and utilization policies, and focused staff training on responsiveness and family communication.

Bottom line: Prospective residents and families can expect a facility with strong rehabilitative services, a clean and modern physical environment, and many compassionate caregivers. At the same time, they should inquire specifically about clinical monitoring protocols (particularly infection-management and catheter care), staffing ratios and responsiveness, discharge-planning criteria, and meal programs. Asking to speak with therapy leadership, nursing leadership, and recent family references may help assess whether the facilityโ€™s operational consistency matches its clinical strengths.
