    Attractive spotless community, caring staff

I'm very pleased with this senior living community - attractive, spotless facility and grounds, caring and attentive staff (Tasha, Becca, Melissa, Ariane), and excellent food with good vegetarian choices. Rehab and physical therapists are outstanding, and there are plenty of activities (bingo, singalongs, church, movies, outings), plus a beauty shop and thoughtful touches like birthday decorations. My family feels our loved one is comfortable and well cared for, and I'd recommend this place.

    Summary of reviews

Reviewer feedback for this facility describes a mixed performance profile with clear strengths in rehabilitation, activities and certain aspects of hospitality, and recurring operational weaknesses centered on nursing, staffing and communication.

Care and clinical operations: Physical therapy and rehabilitation services are frequently praisedโ€”therapists are described as skilled, compassionate and effective at producing measurable progress. By contrast, nursing-level care is characterized as inconsistent. Concerns include medication-administration errors or delays (including insulin and other medication timing), slow responses to call lights, inconsistent bathing schedules, and instances that reviewers interpreted as inadequate pressure-injury prevention and wound care. These items suggest variability in clinical oversight and uneven adherence to resident-care protocols across units.

Staffing and staff conduct: Multiple accounts point to chronic understaffing and consequential long wait times for assistance. Family communication and responsiveness from admissions and unit leadership are also identified as weak points; some families described difficulty reaching staff and limited proactive outreach. At the same time, many reviewers singled out individual CNAs, therapists and activity staff as caring and attentive, and several noted positive, family-like interactions in hallways and communal spaces.

Dining, activities and environment: Dining quality is a clear strength for many reviewersโ€”meals are often described as good, with variety and vegetarian options, and a nutritionist presence noted. The activity program is robust, offering regular group activities, religious services, outings and salon services. The building and grounds receive compliments for cleanliness, attractive landscaping and well-maintained common areas. However, there are also specific food-safety and sanitation concerns cited (including reports of mold and pest-control issues) that indicate inconsistent food-handling and environmental services practices in some areas.

Management and operational patterns: Patterns in the feedback point to operational gaps rather than uniform performance failures. Key themes are inconsistent housekeeping and laundry follow-through, instances of missing personal items or supply controls, occasional safety-protocol lapses (for example, bed-rail and transfer-support practices), and mixed experiences with unit management and social work responsiveness. There are also serious, isolated allegationsโ€”such as theft or supply mismanagementโ€”that warrant discrete investigation by prospective families and regulators.

Recommendation for prospective families: This facility may offer excellent rehabilitation, engaging activities and high-quality dining for many residents, but families should be diligent in evaluating nursing consistency and operational safeguards. During a tour, ask about staffing ratios by shift, medication-administration and insulin protocols, infection-control and pest-management policies, pressure-injury prevention programs, and procedures for handling personal property. Request to observe a mealtime and a therapy session and speak directly with nursing leadership about how they address call-response times and family communication.

    About Nhc Healthcare - Lexington

NHC HealthCare - Lexington sits at 100 East Vine Street in Murfreesboro, Tennessee, and serves as a 170-bed post-acute center offering a mix of senior care including skilled nursing, rehabilitation, and hospice, and you'll find there that they try to support residents with a focus on comfort and respect, with around-the-clock nursing staff and a team that includes doctors, nurses, and therapy specialists who put care plans together based on each person's needs, which means some folks are getting physical, occupational, or speech therapy and others might only need light assistance or help after surgery, and for those needing end-of-life care, the Caris Hospice program brings physical, emotional, and spiritual support right to the resident, while NHC HomeCare gives healthcare services to patients who prefer to stay in their own homes, and if someone's just visiting for a short stay, there's respite care too, so family caregivers can take a break. The building itself is laid out with a variety of studio rooms, and the amenities help make life easier-you've got housekeeping, laundry, cable TV, emergency call systems, and Wi-Fi, plus beauty salon and barber services, and public places like a courtyard, library, sunroom, and residents lounge, and there's an activities director who sets up social events and trips out. For medical needs, skilled care covers things like wound and respiratory care, intravenous therapy, and tube feeding, plus dietary consultations making sure meals meet the residents' needs, and therapy like physical, speech, occupational, respiratory, and nutritional is on-site, too, whether folks need inpatient or outpatient help. NHC HealthCare - Lexington is also part of National Healthcare Corporation, which has a long history since 1971, and the staff follow ethical and nondiscrimination policies, try to involve both families and the community, and provide information and support like the Day By Day Podcast and Family Resources. Payments can be arranged through Medicaid, Medicare, private insurance, managed care, or private pay, and all facility areas have controlled access for security. The nursing home has a 5-star Medicare rating, and in the local community, it sits as the ninth highest-rated with a 6.6 out of 10 score. Residents can enjoy green outdoor spaces, patios, and options to join in social gatherings, or just relax in the comfort of their rooms. The building's designed for elderly people's needs, with social, medical, and daily living support all in the same place, and there are plenty of ways for folks to stay as independent as possible while getting the help they need.
