    Timely, caring facility; minor improvements

I placed my loved one here and was impressed by the timely responses, thorough initial assessment, smooth admission and quick room assignment. The staff are knowledgeable, friendly and competent - they coordinated with the doctor, provided wheelchair assistance, and created a family-friendly atmosphere with an on-site salon, chapel, cafeteria and regular holiday events. Overall I'm very satisfied with the communication and care, with just a little room for improvement.

    Summary of reviews

Overall impression: The facility presents as a well-appointed post-acute and long-term care setting with strengths in admissions, initial assessment, and resident-focused amenities. Multiple reviewers highlighted a smooth intake process, prompt room assignment, thorough initial clinical evaluations, and active coordination with attending physicians. Operational supports such as wheelchair assistance, an on-site salon, chapel services, cafeteria dining, and planned social events contribute to a family-friendly environment and a sense of normalcy for residents.

Care and staffing: Clinical and support staff are generally described as knowledgeable and personable, and several families praised the tone of administrative communication. These positive accounts suggest competent baseline care, particularly during admission and physician coordination. However, a subset of feedback points to variability in day-to-day caregiving responsiveness and inconsistencies in clinical follow-up. Specific concerns imply that pressure-injury prevention and routine wound care are areas where practices may not be consistently applied, and that incident-response procedures could be more robust.

Dining, activities, and facilities: The facility's physical amenities and programming are clear assets. Cafeteria-style dining, holiday programming, invitations to family events, and accessible services such as the salon and chapel support social engagement and convenience. The physical environment was described as pleasant by several reviewers, which aligns with those positive service elements.

Management and quality patterns: Administrative communication is often cited as a strength, with families noting helpful coordination during admission and physician interactions. At the same time, the feedback indicates gaps in quality-consistency controls and clinical oversight that can lead to uneven resident outcomes. There are also concerns regarding end-of-life coordination and communication, including at least one serious post-acute outcome that prompted additional scrutiny by a family. Those items point to opportunities for improved protocols around incident escalation, hospice collaboration, and family outreach during critical events.

Guidance for families: Prospective residents and families should weigh the facility's clear strengths in admissions, amenities, and day-programming against the reported variability in clinical consistency. Recommended pre-admission questions include inquiries about pressure-injury prevention protocols, staffing levels and shift coverage, incident-response workflows, and how the facility coordinates with hospice or palliative services. Observing direct care interactions and requesting recent quality-assurance or incident-response policies during a tour can help assess whether this facility's operational practices meet a family's specific expectations.

About Senior Rehabilitation & Skilled Nursing Center

Senior Rehabilitation & Skilled Nursing Center is in Nederland, Texas, with other sites in Port Arthur and Liberty, and it serves seniors in need of skilled nursing care and short-term rehabilitation after illness, injury, or surgery, and you'll find that the place offers both large private and semi-private rooms, including some with private bathrooms, kitchenettes, cable TV, phones, and Wi-Fi-all designed to make recovery and daily living a bit easier. The center admits residents for both long-term care and short rehabilitative stays, accepts Medicare, Medicaid, and Veterans benefits, and is government (county) owned, with 128 certified beds as of June 2025; the staff includes nurses on duty by physicians, available 24 hours a day, and support with medication, bathing, dressing, dining, and transfers is part of daily life for those who need it.

There's a full suite of therapies offered-physical, occupational, and speech therapy-plus specialized care for tracheostomy, wound, IV therapy, diabetes, and different kinds of cardiac care for those with conditions like CHF or COPD, and folks can get care after orthopedic surgeries too, like knee or hip replacements. Rehabilitation programs focus on building strength, improving mobility, and helping residents get as independent as their situation allows, and there are individualized programs that set goals and check progress along the way.

Meals come from a professional chef with restaurant-style dining, and special diets for allergies or diabetes are no problem, and residents can join in on the community's movie nights, arts and crafts, music groups, and all sorts of activities that happen every day, either indoors or outdoors since there are walking paths, gardens, a patio, and other outdoor spots for getting some fresh air or socializing. Some people like the fitness room or library, others prefer the spa or wellness room, and there's also a dedicated memory care unit for those who need extra support due to memory loss. Housekeeping, laundry, dry cleaning, and even move-in coordination are provided, so residents don't have to worry about those details.

The business side is run by an Owner, Director of Nursing, and Director of Business Development, and the community belongs to a network called Gulf Healthcare Centers, which means there's shared experience and support across different sites, including Port Arthur and Galveston. There's organized transportation and parking, too. Community events, resident-run programs, and group wellness activities keep people busy and give everyone a chance to connect. The facility is not a CCRC, it's focused on skilled nursing and senior rehabilitation rather than retirement living, and while it has a 1-star rating from the Centers for Medicare & Medicaid Services as of now, the community rating from residents or families is 8.4 out of 10. The building doesn't have Better Business Bureau accreditation, but the rating stands at A+. The care here covers a wide range, with skilled nursing, respite care for short-term stays, memory care, and all sorts of therapies aimed at keeping seniors as healthy and comfortable as possible in a home-like environment, and there's always staff around to help, day or night.
