    Clean, compassionate staff; comfortable room

I was pleasantly surprised by this clean, well-coordinated facility. The staff - especially the nurses and therapists - were friendly, compassionate, and extremely helpful; therapy was careful and pain-free. My very large, spotless room fit visiting family, and the food was okay. I would recommend it and choose it again.

    Summary of reviews

Reviews of Pheasant Ridge Nursing and Rehab Center present a mixed picture, with a clear split between direct-care strengths and operational weaknesses. Positive comments emphasize the interpersonal and therapeutic side of the facility: many reviewers described staff as friendly and compassionate, therapists as skilled and patient, rooms as large and visitor-friendly, and housekeeping as thorough. Several families also noted effective pain control and instances of well-coordinated care, which they found reassuring during a stay.

However, clinical and operational concerns appear repeatedly and warrant attention. Nursing-care delivery is described as inconsistent in practice, and family members cite lapses in routine personal care such as bathing and wound management. These gaps are often attributed to limited staffing levels and delays in responding to call bells and resident requests. Reviews also point to weaknesses in how clinical incidents are documented and communicated to families, particularly around falls and follow-up information.

Facility management and oversight emerge as a recurring theme. Multiple reviewers expressed frustration with administrative follow-through and perceived a mismatch between the facilityโ€™s marketing/presentation and the everyday care experience. This combination of managerial shortcomings and frontline staffing constraints appears to drive many of the negative observations about continuity of care and responsiveness.

On balance, Pheasant Ridge demonstrates clear strengths in therapy services, housekeeping, and positive staffโ€“resident interactions in many cases. Prospective residents and families should weigh those strengths against the operational concerns: inconsistent personal-care practices, staffing and responsiveness issues, and reported gaps in incident reporting and leadership follow-through. When evaluating the facility in person, consider asking targeted questions about staffing ratios, wound-care protocols, call-bell response times, incident reporting procedures, and how interdisciplinary teams coordinate care to ensure that written promises align with day-to-day practice.

About Pheasant Ridge Nursing and Rehab Center

Pheasant Ridge Nursing and Rehab Center sits at 4355 Pheasant Ridge Road, SW in Roanoke, VA, with a modern building that feels comfortable but isn't flashy, and they mostly focus on nursing home care and rehab services, so you'll find 101 beds here, both private and semi-private, where people get help for things like stroke, dementia, and even harder to manage medical problems like pulmonary or pain issues, and you can find staff around at all hours since they never close, and from the way everyone talks, the staff tend to treat residents and their families with a lot of care, sometimes like their own. The place is run by Mr. Nathan Libassi, and it's owned by Consulate Management Company, which calls it a proprietary facility, and they seem to make sure the rooms and community spaces, like the big family parlor, stay clean and comfortable. So, if someone needs skilled nursing, after-surgery care (post-acute), or help getting stronger again with physical, occupational, or speech therapy, they'll get it here, and they even have a strong discharge planning program to help you move back home if that's possible.

You get a sense they take on the tough cases, so people dealing with complex health problems, wounds, or memory loss from things like Alzheimer's can get a lot of attention, and the care plans they make fit each person, with input from full-time social services staff helping families deal with life changes or paperwork. There's a code alert security system and basics like cable TV in the rooms, and extras like a beauty and barber shop. Residents can join in daily recreation and planned activities, and meals come with options for special diets, and the dining room tries for a fine dining look but keeps it practical, with both Medicaid and Medicare helping cover costs for those eligible. You'll see more than just basic help, since their teams handle everything from wound care to pulmonary and pain management, and the place is considered part of the long-term care and skilled nursing facility family, similar to nearby places like The Harmony Collection and TerraBella Pheasant Ridge.

Rates are clear-$455 daily for private rooms, $322 for semi-private rooms, and they show this up front if anyone asks or wants a tour, which is easy to schedule by phone, email, or their website. In the past year, they provided almost 33,000 patient days, and maintain 61 staff on full-time equivalent, so there's always someone there. You find them in directories and facility locators, and they're Medicare certified, with a reputation for real, hands-on care and keeping a strong focus on compassion, safety, and personal attention, and while nobody says the place is perfect, a lot of folks do say the staff try hard to make life better for each resident who comes through the doors.
