    Attentive compassionate 24/7 ventilator care

I placed my loved one here long-term and was very pleased with the attentive, compassionate staff and the spotless, peaceful home. They provided skilled 24/7 care (including ventilator/trach needs), owner involvement, home-cooked meals, frequent updates, flexibility to visit with pets, and calm, respectful support through end-of-life. I highly recommend this caring facility.

    About Canyon Creek AFH

Canyon Creek Adult Family Home sits in Brush Prairie, WA, and offers care for adults who need help with daily life and medical needs, and you'll find it's owned and managed by Darren Vierck, a registered nurse with a lot of experience working with seniors and people with complex medical problems, and this place keeps just six residents at a time, which means you see a higher staff-to-resident ratio and there's round-the-clock care from registered and licensed nurses. The home is licensed under Washington license #456900 and follows rules set by local and state authorities, so you know standards get met for safety and care, and the facility operates as a 6-bed adult family home (AFH) with both private and semi-private rooms, including studio suites, shared apartments, and one-bedroom units, each fully furnished and comfortable.

Residents get regular health monitoring, help with things like bathing, dressing, medication management, and special treatments such as wound care, tube feeding, and ventilator support, and there are tailored care plans for everyone, especially for those with Alzheimer's, dementia, or other memory problems. The staff helps with managing chronic illnesses like diabetes or hypertension, and there's hospice and respite care for folks who need short-term or end-of-life support, and the nurses can help with things like getting in and out of the shower, eating, toileting, and making sure everyone gets the right medication on time.

Meals are home-cooked by chefs and meal planners, and staff prepare special diets for diagnosed health conditions, so folks get healthy, tasty food three times a day with snacks, and you'll see amenities like a bistro, snack shop, chapel services, reading room, community kitchen, landscaped gardens, walking trails, recreation rooms, hot tub, sauna, wellness spaces, barber or salon, and air conditioning for comfort, with water or mountain views for some rooms. Residents can use cable or satellite television, internet access, and guest WiFi that's high speed, so staying connected or watching TV isn't a problem, and each room comes with an emergency call system or buttons for quick help anytime, and there's 24-hour emergency response both in-person and through systems in the rooms.

Community activities include music therapy, art, animal therapy, movie nights, tabletop games, birthday or holiday parties, fitness classes, and outside trips, and the schedule keeps people active, whether that means heading out to shop, visit the doctor, or attend spiritual services, and there's travel help for all those things. The place is wheelchair and walker accessible throughout, and staff help with travel both onsite and in the community. Canyon Creek AFH focuses on helping people with dementia or developmental disabilities, too, and features systems to reduce confusion or wandering for those residents, so their safety's a top priority, and you'll see disease management and medication monitoring handled by nurses who know what they're doing.

The ownership by a registered nurse gives the facility a strong focus on medical needs, especially for those who are ventilator-dependent, medically fragile, or need physical therapy and regular monitoring, and the home environment feels private and homelike, with plenty of ways for residents to get attention, stay busy, and feel supported. Visiting hours run from 9:00am to 8:00pm, and amenities or care programs can change, so it's a good idea to check with the administrator if you want to confirm every detail. Canyon Creek AFH doesn't accept Medicare unless certified by the proper agencies, and the facility's classification as a residential care home means it aims to help both seniors and their families by offering dependable, simple, and attentive care to every resident.
