    Summary of reviews

The reviews indicate a facility with mixed strengths and notable operational variability. Clinical rehabilitation services, particularly physical therapy, are described positively, and individual caregivers are frequently cited as friendly or accommodating. Several accounts emphasize staff who engage residents socially, assist with in-room needs, and provide one-to-one support that residents appreciated. The building’s interior and some units are described as clean and reasonably well maintained.

However, there are recurring concerns about consistency of care and responsiveness. Reviewers describe long waits for assistance, slow responses to call bells, and uneven support for activities of daily living, including bathroom assistance. Staffing levels and scheduling appear to be variable; this is frequently mentioned as the context for delayed attention and lapses in routine care. Mobility- and transfer-related safety practices are an important area of concern: at least one account references a transfer-related injury and lack of appropriate transfer equipment use, indicating a need to review transfer protocols and staff training.

Dining experiences are inconsistent across accounts. Some residents and families praised meal options and food they enjoyed, while others described missed meal deliveries, trays left unattended, and unsatisfactory meals. Housekeeping and laundry processes also show mixed performance: while parts of the facility are described as clean, there are specific sanitation concerns tied to bed linens and room turnover practices. Additionally, several reviews note problems with personal-property management, including missing clothing and mobility aids, suggesting room-move and inventory controls could be strengthened.

Administrative and communication shortcomings are a recurring theme. Families reported difficulties getting timely responses from administration, unresolved billing issues, and delays in activating legal-authority documents such as power of attorney. There are also mentions of limited or inconsistent incident reporting and family notification after adverse events, which compounds concerns about transparency and follow-up.

For prospective residents and families, the pattern suggests the facility can offer strong rehabilitation services and has caring individuals among the staff, but also shows operational inconsistencies that may affect day-to-day reliability. Important areas to assess during a visit include staffing ratios at different shifts, transfer-safety protocols and training (including use of gait belts and two-person transfers), call-bell response times, housekeeping and laundry turnaround procedures, property-inventory practices, meal-service continuity, and the facility’s process for incident reporting, legal-authority activation, and billing resolution.

About Heritage Square Health Care Center

Heritage Square Health Care Center sits in a quiet area of Greendale, Wisconsin, with lots of plants and walking paths outside, and it gives seniors many types of care, including nursing home care, assisted living, memory care, independent living, skilled nursing, and home care services, so people can move between care levels as their needs change, and it works as part of a larger Continuing Care Retirement Community, so you don't have to leave if your health needs shift. The nursing home has a 3-star rating from the Centers for Medicare & Medicaid Services and is Medicare certified, and Medicaid assistance is available. Heritage Square focuses a lot on short-term rehabilitation and helping people get back on their feet after hospital stays with things like physical therapy, psychosocial therapy, wound care, and palliative care, and it has respite care for families who need a break. The staff includes registered nurses and nurse aides 24 hours a day, along with infection prevention nurses, diabetic wound licensed nurses, and on-call doctors and specialists such as an on-site dentist, radiologist, and optometrist. Private, furnished studio rooms come with bathrooms, cable TV, kitchenettes, Wi-Fi, telephones, closets, and space for visitors. People who need help with daily things like bathing, dressing, moving around, and taking medicine will find constant supervision and a 24-hour call system to make sure residents stay safe, and staff has special training for non-ambulatory care and memory needs. Amenities like a fitness program, spa, library, movie theater, hair salon, arts room, game room, and community events give many choices for both leisure and therapy, and there are garden walks, scheduled daily activities, social events, and mental and physical therapy sessions led by trained staff. Heritage Square even uses robots for meal services to keep things running smoothly and help with safety. Residents and their families can join councils to speak up about concerns or share ideas about care, and staff at Heritage Square can answer questions about how often doctors visit and how the center works with medical groups. With Medicaid and Medicare accepted, many people find it easier to get care, and the facility puts a lot of focus on helping folks transition back home after hospital stays, using a team that works hard to create personalized care plans and specialized health programs for every individual.
