    Skilled rehab, attentive staff, welcoming

I had a very good experience - friendly, professional staff and a skilled rehab team that helped my mom walk again. Private rooms, attentive nurses/CNAs, and caring leadership (Director of Nursing Ashley and Director of Rehabilitation Robert) made us feel like family. Clean, welcoming facility with helpful administration; I'd recommend it for rehab or long-term care.

    Summary of reviews

Reviewer feedback for this facility is strongly polarized: many families highlight effective rehabilitation services, a number of caring and professional staff members, and a welcoming community environment, while other accounts raise substantive operational and safety concerns. Overall impressions cluster around two themes โ€” a core of positive, rehab-focused care and staff who engage residents warmly, and recurring operational weaknesses that produce inconsistent experiences.

Care quality: Rehabilitation services receive repeated praise โ€” therapy teams are described as skilled and successful in restoring function for some residents. At the same time, clinical-care concerns appear across reviews and include delayed response to assistance requests, lapses in incontinence-related care processes, equipment and alarm reliability problems, and inconsistencies in medication and procedural adherence (for example, glove use and dressing of IV sites). A small number of accounts raise serious post-discharge and end-of-life concerns; these are described as individual incidents and have prompted questions about clinical oversight and family communication.

Staff and conduct: Many reviewers describe compassionate, patient-centered caregivers, specific staff members who go above and beyond, and effective leadership that is visible and responsive. Conversely, others report instances of unprofessional or disrespectful staff behavior, concerns about discriminatory conduct, and at least a few allegations of personal-property issues. These contrasts suggest variable staff performance and inconsistent application of professional standards across shifts or units.

Dining and activities: Comments about dining are mixed โ€” some families find the food acceptable and the kitchen cooperative, while others note cold meals, limited menu variety, and pest-control concerns that affected meal experience. Activity programming receives both praise (engaged activity staff, meaningful games and social interaction) and criticism (limited outings and sparse offerings for some residents), indicating uneven activity availability depending on timing or unit.

Facilities and environment: Several reviewers praise clean, well-maintained common areas and private rehab rooms; others describe specific maintenance problems such as peeling paint, broken fixtures, inadequate room furnishings, and low room temperatures. Sanitation and linen-handling issues are recurring operational themes; these have direct implications for resident comfort and dignity. Pest-control concerns appear in relation to both resident rooms and dining.

Management and communication: Administration is frequently named as a strength by families who experienced positive outcomes and clear communication, with some leaders and departments singled out for praise. Yet other reviewers cite weak management oversight, unanswered phone calls, poor responsiveness to family concerns, and inadequate communication during infection-control events. These divergent accounts point to variability in managerial follow-through and unit-level supervision.

Notable patterns and guidance for families: The overall pattern is one of significant variability โ€” excellent care and strong rehabilitation for some residents coexists with operational lapses that have safety and dignity implications for others. Prospective families should plan an in-person visit, observe staff interaction and call-light responsiveness, review recent state inspection results, ask about staffing ratios and night coverage, inquire about infection-control and pest-management procedures, and verify procedures for medication administration and incident reporting. Speaking directly with the therapy team, nursing leadership, and activity staff during a tour can help assess whether the facilityโ€™s strengths align with an individual residentโ€™s needs.

    About Diversicare - Montgomery

Diversicare - Montgomery is a large, for-profit senior care community at 2020 North Country Club Drive in Montgomery, Alabama, with 138 beds and staff available 24 hours a day, every day of the week, including Sundays, and they always seem to have someone around to answer questions or help out if something happens, which people have said makes the place feel welcoming and safe. The building has studio room layouts for residents, and the entire facility stays clean and well taken care of, with maintenance and cleaning teams keeping everything in neat shape, almost spotless, which you'll notice right away when visiting, and there's a friendly feel, too, as residents often say hello to newcomers in the halls.

This place offers a range of care, serving both short-term rehab patients and those needing long-term skilled nursing, hospice care, memory care for people with dementia, and assisted living for seniors who want a little extra help. Therapy services are strong here, including occupational therapy, physical therapy, and respiratory therapy, with trained medical and therapy staff making sure people get individualized plans to support recovery and ongoing health. There are specialized treatments available, like IV therapy, wound management, and dialysis care, and people point out that both the Director of Nursing and the Director of Rehabilitation are caring and professional, with a full team working together to handle complex medical needs and daily care tasks.

People appreciate the resident and family councils, which help families voice concerns and keep everyone involved in care planning, and there's a big focus on helping each person reach their best level of independence, whether they need help with medicines, health programs for wellness, or regular support from staff for daily routines, and you'll find options like a barber and hair salon, so people can maintain routines they've always enjoyed. Residents benefit from a community that hosts a mix of group outings, scheduled picnics, and entertainment options to keep things lively, which families often say helps their loved ones feel more at home and stay connected with others.

Diversicare - Montgomery accepts Medicare and Medicaid, making it easier for families looking for financial support, and it holds a license as a Skilled Nursing Facility (license #N5106). The facility earned high marks during health inspections with no severe deficiencies, has an overall Grade A rating, and it carries a BBB rating of A+, which gives some peace of mind, though its community rating of 4.5 out of 10 (23rd in Montgomery) and a 3-star rating from the Centers for Medicare & Medicaid Services shows there's room to grow, as with most places. The Executive Director, Mr. Elmore Patterson, oversees the team and is known for being approachable and helpful, alongside the business office staff who handle questions promptly. Pet-friendly assisted living is offered for those who want to keep furry companions, and private rooms are set up for comfort and safety.

Residents and their families say the staff treats people with kindness and dignity, aiming to help each person regain abilities after an illness or injury, and making it a goal to help everyone feel respected and supported. The facility partners with the Alabama Department of Public Health and follows all necessary guidelines, always working towards providing a safe, professional place for seniors to live and recover. All in all, Diversicare - Montgomery stands out for its strong therapy services, responsive staff, friendly environment, and steady attention to cleanliness and comfort, with a team committed to doing right by the people they serve.
