    Patient caring staff, clean facility

I'm very pleased overall - my husband adjusted and my mother is happy; I feel they're in good hands. Staff are patient, caring and friendly, nurses/CNAs keep me updated, the facility is clean with no smell, and limited visitation has been fantastic; I highly recommend.

    Summary of reviews

The reviews present a mixed picture of Meadowview Nursing Center with notable strengths alongside several operational weaknesses. Positive comments emphasize an attentive nursing and CNA presence, instances of caring, communicative staff who keep families updated, and a generally clean, odor-free environment that some residents and family members found reassuring. Several accounts described successful adjustment after admission and satisfaction with everyday interactions, indicating that the facility is capable of providing supportive, person-centered care in some situations.

However, a number of reviews point to substantive variability in care quality. Concerns include inconsistent execution of basic personal-care routines such as bathing and assistance, intermittent monitoring of meals and hydration, and broader gaps in clinical oversight. These operational patterns manifest as delayed or inadequate attention to residents' basic needs for some families. There are also reports of problematic discharge and transition practices that suggest weaknesses in discharge planning and care coordination at the time of transfer or discharge.

Staffing and management issues emerge as a recurring theme. While many reviewers praise individual nurses and aides, there are also descriptions consistent with insufficient staff training, supervision, and stability. Some reviews describe abrupt personnel changes and disciplinary actions that appear to have affected continuity of care. Concerns about staff conduct and communication toneโ€”ranging from insufficient responsiveness to problematic interpersonal interactionsโ€”were raised alongside praise for staff who are patient and communicative.

Facility and security observations are similarly mixed. The facility's cleanliness and lack of strong odors were noted positively, but there are serious operational concerns about property security and asset management, including allegations of theft and missing personal items. These accounts suggest a need to strengthen inventory controls and secure storage processes. Information about dining quality and activities is limited in the available summaries; however, the reported gaps in oversight of basic needs imply that meal provision and supervision may be inconsistent in some cases.

Management and administrative responsiveness are areas of concern. Multiple comments indicate perceived mismanagement or insufficient administrative follow-through on care plans and family communications. Given the mixture of positive and negative experiences, a pattern emerges of variable performance across shifts, units, or staff teams. Prospective residents and families would likely benefit from asking the facility for specifics about care-plan documentation, staffing ratios and turnover, discharge procedures, security and property controls, and sample activity and dining schedules to better gauge consistency of service.

    About Meadowview Nursing Center

Meadowview Nursing Center sits as a smaller, for-profit home with 59 certified beds and most days sees about 55 residents around, and you'll find that skilled nursing care is present with nursing staff on hand for 12 to 16 hours and a call system that answers at any hour, so everyone can get the help they need with things like bathing, getting dressed, taking medicine, transferring between bed and chair, and getting through the regular day-to-day stuff, which is important, especially for people who have trouble walking or need help because they can't get around on their own. The Center covers medical care along with therapy, rehabilitation, dementia care, and even hospice care when it's needed, and the staff includes registered nurses, licensed practical nurses, and certified nurse aides who handle a fair share of the work, putting in about 3.67 nursing hours per resident each day, though that's a little less than the state's average, and nurse turnover sits quite high, a touch over half leaving every year. Folks living here get access to a private room if they want, each one set up with its own bathroom, cable TV, kitchenette, phone, air-conditioning, Wi-Fi, and high-speed internet, which helps a person stay comfortable and a bit more independent, and the main building also has rooms for art, games, and books along with space to keep fit, enjoy wellness activities, or step outside for planned events on walking paths or relax in the common rooms, or even catch a movie or listen to music with the group. The Center coordinates meals with different diets in mind, prepares and serves all food, and provides full housekeeping, concierge help, furnished rooms, and even plans out daily activities, which gets everyone moving and keeps things interesting most days, though there have been some concerns flagged, like an infection-related deficiency that's out of line with federal rules and a number of inspection deficiencies-nine in recent reports-despite their policies to prevent neglect or theft. Meadowview Nursing Center accepts both Medicare and Medicaid; it's run by Bradley Long and Mark Sims, who each own half, and has been under Trinity Management, Inc. since late 2004. The Center carries a 4-star CMS rating, which is higher than both the state and national average. Clinical lab and physical therapy services are in place as well. Overall, Meadowview Nursing Center offers most care and senior living supports you might expect from a small, certified nursing home, though inspection history and staffing challenges are part of the facts to keep in mind.
