    Attentive staff; excellent medical care

I moved my mom here and have been very pleased. The staff are loving, attentive and family-like-courteous, compassionate caregivers with doctors on staff and a PA on call five days a week; the medical care has even exceeded what we experienced at the hospital. The place is clean and welcoming with single-occupancy apartments, enjoyable activities and decent dining; overall very satisfied, with just a few small improvements that would make it perfect.

    Summary of reviews

Mountain Meadows Nursing & Rehab Center presents a mixed profile: several reviewers described substantive clinical strengths alongside operational weaknesses. The facility maintains on-site medical coverage, including physicians and a physician assistant who is available during the week, and offers rehabilitation services. Many families and visitors described the staff as courteous, compassionate, and welcoming, contributing to a family-like atmosphere that puts relatives at ease. The buildingโ€™s exterior appears modern and the interior is generally reported as clean, and private single-occupancy apartments and dedicated dining and exercise areas are available.

Clinical care is a noted strong point in some accounts: the presence of on-site providers and an organized rehab team led some families to say medical attention was better than what they experienced in hospital settings. At the same time, multiple operational patterns raise concern. Reviews indicate gaps in medication-management practices and documentation, inconsistent adherence to prescribed diets and fluid orders, and incontinence-care issues. These items suggest opportunities for stronger pharmacy controls, clearer medication-administration protocols, and improved recordkeeping.

Staffing and responsiveness present another area of contrast. While many reviewers praised individual staff members as attentive and caring, others described inconsistent staff performance and periods when nurses appeared overwhelmed. Understaffing was linked to delays answering call lights and slower responses to resident needs. These patterns point to variability in staffing levels, training, or supervision rather than uniformly poor or excellent performance.

Dining and activities also show uneven experiences. The facility offers a formal dining area and generally acceptable food, but reviewers flagged high-sodium menu options and cases where dietary or fluid restrictions were not consistently followed. Activity programming exists โ€” including a dementia-specialized unit and a small activity/relaxation room โ€” but the offerings were characterized as limited in scope and space by several families.

Physical plant observations are similarly mixed. Many accounts emphasize cleanliness and recent improvements, but reviewers also mention aged furnishings, worn equipment, and uncomfortable mattresses in resident rooms. Communication with families emerged as a recurrent theme: some families experience helpful, transparent interaction, while others reported breakdowns in information-sharing and expectations, including perceptions that services did not fully match the facilityโ€™s promotional materials.

For prospective residents and their families, the pattern suggests that Mountain Meadows may offer strong interpersonal care, on-site clinical resources, and a comfortable communal environment in many respects, but also has recurring operational issues that warrant direct inquiry. Recommended pre-admission steps include touring actual resident rooms, asking for written medication-management and staffing protocols, reviewing how dietary and fluid orders are implemented, inspecting bedding and equipment, and clarifying activity schedules and dementia programming. These steps can help determine whether the facilityโ€™s strengths align with an individualโ€™s care priorities and whether identified operational weaknesses are being actively addressed.

About Mountain Meadows Nursing & Rehab Center

Mountain Meadows Nursing & Rehab Center has several options for people looking for skilled nursing, rehab, memory care, independent living, assisted living, and respite care, and with 110 certified beds, most days about 98 people live there, and staff say they aim to provide a respectful and caring place for those who need long-term care or short-term rehab, and it seems they try to get families involved in care planning, though inspection reports show the facility does have some noteworthy issues, because state inspectors found a total of 20 deficiencies, including several related to pharmacy services like a higher medication error rate and some problems meeting nutrition and dietary standards, such as not always serving food at proper temperatures or getting it from approved sources, and there's at least one infection-related deficiency and other violations of infection prevention rules, so that's important for families to know when considering what kind of care might be right for their loved one. Nurse staffing hours here stand at about 3.02 per resident per day, which sits lower than the state average of 4.1 hours, and while the facility has features found in most nursing homes, like inpatient physical, speech, and occupational therapies, and it's certified for both Medicare and Medicaid, the oversight comes from Tonya Moser, who's managed since 2016, and John Scribner as of August 2024, with indirect owners Anthony Adams and Bryan Adams, both through Ovation Health Systems, Inc. Mountain Meadows includes a memory care program, and its activity calendar looks full, with things like art classes, movies, live music, bingo, church services, and birthday parties, and the dining program tries to offer choices for different tastes and allergies, serving southern favorites and lighter options with assisted meal service for those who need help. The facility strives to feel home-like, promising a warm and safe environment, and folks in short-term rehab, long-term care, memory care, or respite care get access to a range of specialized care and support for both daily living and health needs, and the place is set up to help residents stay active with group activities and encourage social connections. Staff say they focus on treating everyone with dignity, aiming to support both the physical and emotional well-being of people living there, and while it offers many services for older adults and their families, families might want to carefully look at inspection reports and current staffing before making decisions.
