    Excellent rehab stay, attentive staff

I stayed here for rehab and had a very good experience - kind, attentive staff who kept me informed and helped with concerns. The rehab team was excellent, meals were delicious, and my room and the whole facility were spotless. Activities, a sunny courtyard and cozy common areas kept me busy, and the place felt family-friendly with private or shared rooms available. I'm very satisfied and would recommend it.

    Summary of reviews

The reviews of Lonoke Nursing & Rehab Center present a mixed but instructive picture. Many commenters praise the facility’s interpersonal strengths: staff are frequently described as friendly, attentive, and helpful, and several families highlighted strong short-term rehabilitation outcomes. The physical environment and amenities also receive consistent positive remarks — reviewers note large rooms with private bathrooms, clean common areas (including the dining room), organized laundry services, and outdoor spaces such as a courtyard and patio. Dining and activities are frequently cited as strengths: meals are called enjoyable, the dining area is clean and welcoming, and a variety of activities and games are offered that engage residents and sometimes include small incentives or prizes.

Staff performance and direct care are clearly a focal point in the feedback. Numerous accounts describe compassionate, well-informed caregivers who communicate with families and provide attentive support during rehab. At the same time, there is a countervailing pattern of concerns: some reviewers describe markedly different experiences, citing delays or lapses in care, inconsistent professionalism, and poor phone or administrative responsiveness. There are also serious, individual-level allegations of staff misconduct, including claims related to substance misuse and financial irregularities; reviewers indicate these matters were escalated but, in some instances, perceived management response was inadequate. These accounts suggest unevenness in staff behavior and supervision rather than a uniform operational profile.

Operational and management themes emerge from the contrast between positive and negative comments. Strengths include a pleasant, home-like atmosphere in common areas, structured activities that keep residents engaged, and generally positive short-term rehab outcomes. Conversely, recurring operational weaknesses appear to include inconsistent care quality across shifts or units, gaps in oversight of staff behavior, and breakdowns in communication channels (for example, slow or unsatisfactory phone contact and variable responsiveness to family concerns). Reviewers also referenced workforce issues such as compensation and retention pressures, which can contribute to variability in staffing and continuity of care. A few reviewers raised concerns about documentation and compliance practices; given the seriousness of those claims, prospective families may want to review inspection records and ask management about quality-assurance processes.

For prospective residents and families, the pattern suggests that Lonoke Nursing & Rehab Center can offer strong rehabilitation services, a clean environment, engaging activities, and a caring team in many cases. At the same time, variability in experience and the presence of isolated but serious allegations point to the importance of direct due diligence: arrange an in-person tour, ask specific questions about staffing levels and supervisory procedures, review state inspection and complaint history, and clarify the facility’s processes for reporting and addressing conduct or documentation concerns. Doing so will help families weigh the facility’s evident strengths against the operational risks identified by some reviewers.

About Lonoke Nursing & Rehab Center

Lonoke Nursing & Rehab Center sits at 1501 Lincoln Street in Lonoke, Arkansas, and holds 77 certified beds for people who need inpatient or long-term care, and even though it doesn't have published details about its history or extra programs, it operates as a rest home and skilled nursing facility under the management of Ashley Thomley since December 2024, and it connects to groups like Catlett Care and Central Arkansas Nursing Centers. About 50 to 99 employees work there, including certified nurses, nursing assistants, physical therapists, and licensed medical staff, who all help people with round-the-clock supervision, medication help, personal care, and mobility needs. The nurse turnover rate is 48.2%, but there's a higher number of nurse hours per resident each day than the average, coming in at 4.38, which usually means more direct attention for each resident, and they still aim to help everyone get better, stay independent, and maybe return home when they're ready.

Residents have access to long-term and rehabilitation care, respite care for those recovering from illness or surgery, and special support for anyone with medical disabilities or needs for higher security-like protected areas that help keep folks safe if they're likely to wander. The facility also provides regular meals and snacks, though inspections have shown some problems with making sure food meets every person's dietary needs and stays warm and palatable, which rated as a level E deficiency, and the most recent inspection in July 2024 found 15 problems, while recent reviews marked 24 issues in total, including 2 related to infections.

There are amenities such as flat screen TVs, free Wi-Fi, activity programs, salon and barber services, and restaurant-style meals, all to keep folks comfortable and help their mental and physical health. Help with transportation is available, and you've got people nearby at all hours to help with daily activities and keep medication managed. The Administrator, Mr. Ashley Thomley, is the principal contact, and while there's no information posted about Better Business Bureau accreditation, the staff work together to give people in their care attention based on each person's unique needs, trying to maintain dignity and well-being for all residents.
