    Attentive staff, clean facility, recommended

I've been very pleased with the caring, professional staff - nurses and CNAs are attentive, kind and punctual - and the facility is consistently very clean and well kept. Residents enjoy freedom of movement, plentiful social programs and effective therapy; I feel well cared for and would recommend this community.

    Summary of reviews

The reviews present a polarized picture of Avalon Villa Care Center: a consistent cluster of positive comments praising individual caregivers and therapy services sits alongside a substantial set of operational and quality-of-care concerns. Strengths most frequently cited include attentive, compassionate CNAs and nurses (with several staff named positively), an effective physical-therapy program, helpful social services, and in some accounts a clean, organized environment with active social programming. Families who had positive experiences commonly emphasize prompt, personalized attention and a community-feel in the unit.

Conversely, a number of reviews describe systemic care-quality issues. The most recurrent operational concerns are inconsistent staffing (including reports of short staffing and high turnover), delays in attending to resident personal-care needs, and lapses in medication-administration oversight. These patterns are linked in multiple accounts to long wait times for assistance, missed or unobserved medication events, and inconsistent bathing/incontinence-care practices. Such issues suggest gaps in daily care processes and supervision rather than isolated scheduling inconveniences.

Facility and amenities feedback is mixed. Several reviewers praised cleanliness, secure access, shared activities, and an environment that allows residents to move about; others described variable cleanliness, persistent odor concerns in some areas, small multi-bed rooms with limited storage and privacy, and inconsistent meal quality that in some reports contributed to poor intake and weight loss. Dining received both positive notes (described as above-average for an institutional setting by some) and negative comments about meal quality and appetite support by others.

Staff professionalism and management practices are another area of divergence. Positive reviews highlight friendly, professional front-line staff and proactive administration. However, other reviews raise concerns about staff conduct, unprofessional appearance, inappropriate use of personal devices, and inadequate supervision of clinical competencies (including allegations of unlicensed staff on schedules). There are also reports of poor communication with families — unanswered calls, abrupt phone interactions, and unclear information during discharge events — and serious allegations about late-night discharges and financial or regulatory problems, including mention of a substantial fine; these items indicate potential weaknesses in policy enforcement and regulatory compliance that warrant direct verification.

For prospective residents and families: the facility demonstrates clear strengths in rehabilitation and in the performance of individual caregivers, but the pattern of operational concerns suggests the need for careful due diligence before placement. Recommended steps include an on-site visit during a shift change to observe staffing levels and responsiveness, asking for written policies on medication administration, incontinence and hygiene care, and discharge/transfer procedures, reviewing current licensure and any recent inspection or sanction records, and meeting the unit manager and therapy team. Observing meal service, activity participation, room arrangements, and how staff respond to a call bell can help assess whether this particular unit’s performance aligns more with the positive or negative accounts in these reviews.

    About Avalon Villa Care Center

Avalon Villa Care Center operates as a for-profit nursing home owned by Harry Jacobs since June 2015, with 131 Medicare-certified beds and typically about 113 residents each day, and even though the place is fully licensed and accredited, the records show a history of 103 total deficiencies from inspection reports, including areas like resident rights, assessment, care planning, infection control, and nurse training, plus complaint reports in February and April 2025 which together identified seven specific deficiencies. The center's nurse turnover rate stands at 51.5%, which is above the state's average, and the nurse staffing level is 3.99 hours per resident per day, falling short of the state average of 4.5 hours, so families might want to keep that in mind when looking at staff attention.

Avalon Villa Care Center doesn't provide much public detail about its services or amenities, but there's a range of activities, like adult education, cooking, musicals, movies, religious services, evening socials, holiday celebrations, and activities for socializing, which might help keep folks busy and connected, and the building has features like an enclosed patio, a reception lobby, and a television viewing room, as well as semi-private rooms, hallways, an activity area with equipment, and a sunlit room for residents to spend time in. Avalon offers care for several health issues such as stroke, dementia, brain injury, amputation, pneumonia, dehydration, and more, and has programs for rehabilitation, ventilator care, wound management, psychiatry, dysphasia, respite care, psychosocial supports, and resident care planning.

There's a healthcare and support team that takes an interdisciplinary approach to care, and the facility accepts referrals or admissions at any hour, day or night, which might be important for families needing flexibility, and they'll work with most insurance plans and HMOs. Avalon provides social services to address requests, concerns, and discharge planning, and activities that aim to help residents stay active. There's an individualized rehabilitation focus meant to promote as much independence as possible, with both short-term and long-term skilled nursing and rehabilitation care available. The staff's dedicated to providing personalized care, and the environment aims to support comfort and well-being, but the recent deficiencies and low staffing hours are things families should weigh before choosing Avalon Villa Care Center.
