    Attentive staff, excellent rehab care

I had a very positive experience - staff were consistently attentive and compassionate, nurses and therapists provided high-quality care and rehab, and case manager Courtney kept communication clear and helpful. The facility was clean, meals enjoyable with attentive dining staff, and social activities made my stay pleasant; I felt confident trusting them with ongoing rehab needs.

    Summary of reviews

Feedback for Centre Ave Health & Rehab is mixed, with clear strengths in therapy, patient engagement, and many aspects of daily care, alongside operational concerns that prospective families should evaluate during a visit. Numerous comments highlight compassionate direct-care staff, active rehabilitation services, and visible efforts by case managers and social-work teams to coordinate care. At the same time, other accounts describe lapses in timeliness and communication that have had significant consequences for families.

Care quality and staff performance are a prominent positive theme. Reviewers frequently praised nursing and therapy teams for being attentive, encouraging mobility, and delivering individualized rehabilitation that produced measurable progress. Physician communication and family conference calls were described as helpful in coordinating plans of care, and specific staff members (including case management) were noted for strong advocacy and thorough updates. These strengths appear to be consistent with good short-term rehab outcomes and supportive day-to-day caregiving for many residents.

Conversely, a pattern of operational weaknesses emerges in the areas of clinical responsiveness, medication/equipment handling, and safety oversight. Several accounts describe delayed responses to clinical needs, missed or delayed medication actions, and problems locating or managing respiratory equipment. These incidents suggest gaps in incident response protocols, medication administration controls, and some aspects of transfer/room safety. Additionally, slow response times to call-button alerts were mentioned and may reflect staffing levels or workflow constraints.

Dining, activities, and facilities receive generally positive remarks but with variability. Many residents and families praised meals, attentive dining staff, and social programs (including music and group activities). The building and rooms were often described as clean and comfortable with natural light. However, others noted a decline in meal quality and service continuity following an ownership transition, indicating the potential for operational disruption to affect resident experience.

Management and operational trends warrant attention. Positive feedback singled out proactive case managers and engaged social-work staff; however, several reviews raised concerns about understaffing and its effect on capacity and response times. Communication with families during critical events was identified as inconsistent in some instances, including late notifications. For prospective residents and families, recommended questions include: how the facility handles call-button response metrics, medication and equipment protocols, staffing ratios on applicable units, procedures for incident notification to families, and recent changes in ownership or management that could affect services.

Overall, Centre Ave Health & Rehab demonstrates clear capabilities in rehabilitation, compassionate caregiving, and resident engagement, but also shows operational gaps that have led to serious concerns for some families. A focused conversation with facility leadership and an on-site observation of staffing, response times, and dining/cleanliness routines will help families assess whether the facility’s strengths align with their needs and whether the identified weaknesses have been addressed.

About Centre Ave Health & Rehab

Centre Ave Health & Rehab sits right on the Fort Collins Campus of Columbine Health Systems at 815 Centre Ave in Fort Collins, and the people there try to make the place feel warm and comfortable for folks who need help with their health, whether that's for a short time or if somebody needs a place for longer care, and you see, they have all private rooms already furnished so everyone can have some privacy and comfort, and they put in cozy furniture to help people feel at home. The staff speaks English, and they offer skilled nursing for folks with complex health needs, plus short-term rehabilitation programs for those needing therapy after surgery or illness, and they've got three separate therapy gyms so people can work on getting stronger with physical, occupational, or speech-language therapy, and their doctors run things as a personal care facility to make sure care is centered on each individual, with memory care that focuses on supporting those with Alzheimer's and dementia.

The place is set up to be pet-friendly, which means residents who have pets can keep that comfort with them, and there are garages available if that's something a person wants for convenience, while transportation services help folks get where they need to go outside the building. They focus on safety using emergency response systems in every room, and medication management and safety monitoring are part of their routine. To help people feel better beyond just physical health, the staff tackles pain management and pays attention to emotional well-being, offering support in different areas like ostomy care, urological supplies, and mobility devices, plus they have a medical equipment department that handles things like CPAP and BIPAP machines, hospital beds, respiratory therapy, incontinence help, and compression fittings.

Their memory care support is tailored to meet the needs of people with Alzheimer's and other memory conditions, using programs that aim to engage residents with social activities and fitness, keeping people active mentally and physically, while personalized care plans make sure services suit each resident's needs. Chef-prepared meals are served daily, and if someone wants to look their best, beauty and barber services are right there on-site. The building has walking paths and quiet courtyards for relaxing and spending time outside without going far, and because this is part of Columbine Health's full network, residents who need to move on to different levels of care can do that more smoothly.

Care levels here include both skilled nursing and mid-level support, plus long-term and short-term stays, whether someone's coming in from the hospital, from home, or from a doctor's office. There's an internal pharmacy, Centre Pharmacy, which keeps medication needs close at hand, and if in-home care is needed, they also offer help with daily living, home health, therapy, safe living solutions, and dementia support for those who want to stay at home instead of moving in. All told, Centre Avenue Health and Rehab Facility, LLC provides comprehensive nursing care, rehab therapies, personalized support, and a focus on comfort for seniors with a variety of health and memory care needs. Currently, they're not accepting new patients, and their office hours aren't specified.
