    Excellent care, friendly staff, recommended

I had a very positive stay - the staff were friendly, professional, and genuinely caring, with supervisors who took personal interest and management that communicated proactively. The facility was clean, well-maintained, and conveniently located, with modern equipment, excellent therapy and nursing, and tasty, accommodating dining. I felt like my loved one was in good hands, the team treated everyone like family, and I would recommend this place for rehabilitation.

    Summary of reviews

Poudre Canyon Health and Rehabilitation elicits a mixed but distinct pattern of strengths and operational weaknesses. The facility receives consistent praise for its therapy programs and many individual caregivers, while also drawing repeated concerns about staffing, facility condition, communication, and certain aspects of clinical continuity.

Care and staff: Many families and patients highlight knowledgeable occupational and physical therapists who pursue goal-oriented recovery and demonstrated commitment. Reviewers frequently describe nurses and CNAs as compassionate and personally engaged; supervisory staff are sometimes described as taking a direct interest in resident progress. At the same time, there are recurrent operational concerns about inconsistent staffing levels, low staff-to-resident ratios, and uneven clinical competency. These staffing patterns are linked in reviews to delays in routine care tasks, gaps in medication continuity, and variability in the timeliness of clinical response.

Therapy, dining, and activities: Therapy services are a clear strength, with multiple references to therapists who go "above and beyond," support functional gains, and maintain a family-like approach. Dining staff are noted for accommodating nutritional requirements and providing appealing meals. Recreational programming is described as active and creative, including traditional activities and entertainment that engage residents and celebrate accomplishments.

Facilities and sanitation: Accounts of the physical environment are mixed. Some reviewers describe clean, well-maintained areas and modern equipment, while others describe an older building with inconsistent maintenance, sanitation concerns, and localized odor issues. There are also mentions of occasional infrastructure problems affecting sanitation systems. Prospective residents should expect variability by unit and consider an in-person tour to assess current conditions and recent corrective actions.

Management, communication, and transitions: Reviews show a contrast between reports of responsive, engaged leadershipโ€”especially under more recent managementโ€”and reports of inconsistent communication, poor discharge coordination, and front-desk or phone responsiveness gaps. Several reviews raise serious concerns about handling of personal property during transitions and, in a few cases, about family notification following a resident's death; these point to weaknesses in postโ€‘mortem protocols and family communication that warrant direct inquiry when evaluating the facility.

Billing, safety, and oversight: Financial and administrative issues appear as a recurring theme. Reviewers describe billing or insurance coordination unpredictability, unexpected charges, and difficulty resolving disputes. Safety supervision and resident oversight are another concern in some accounts, with reviewers noting situations that suggest gaps in supervision and monitoring. These items suggest the importance of clarifying staffing ratios, safety protocols, and billing practices before admission.

Bottom line: The facility demonstrates clear strengthsโ€”especially in therapy, certain caregiving staff, meal accommodations, and recreational programmingโ€”but also shows persistent operational weaknesses in staffing consistency, facility maintenance, communication, and administrative processes. Families considering this facility should prioritize direct assessment of current staffing levels, medication continuity practices, property-handling protocols, billing procedures, and recent management actions to address sanitation and infrastructure issues.

About Poudre Canyon Health and Rehablitation

Poudre Canyon Health and Rehabilitation Center LLC sits at 1000 S LeMay Ave in Fort Collins, Colorado, and serves as a skilled nursing facility with 81 beds focused on helping seniors and people recovering from injuries or illness. This place offers 24-hour skilled nursing care and features a wide range of services, like physical, occupational, and speech therapy, plus wound care, dialysis, trach care, wound vac, bariatric beds, and intermediate care when needed, so you'll find a lot of different kinds of help under one roof. There are both private and semi-private rooms, and they do their best to keep the look and feel homelike instead of like a hospital, which can make things easier for those staying there a while or just for a bit.

The facility employs a team of healthcare professionals, including a Director of Nursing and a Business Office Manager, adding to the sense of order and support, and you'll notice physical therapists and occupational therapists are on staff to create personalized treatment plans for every patient, which helps folks regain mobility and keep their independence as much as they can. The Nurses, therapists, and other staff are trained to provide five-star nursing and medical care, and the team at Poudre Canyon puts a lot of effort into creating a comfortable environment with modern rooms, inviting common areas, and social spots where residents can take part in different activities and programs to help them recover and stay engaged.

A lot goes on at Poudre Canyon Health and Rehabilitation for patients that need short-term rehab, long-term care, respite stays, or memory care services, and there's specialized support for those with complex medical needs as well. They've got a bunch of supportive features, like Wanderguard technology for safety, scheduled transportation for appointments, meal planning, dietary services, laundry, housekeeping, and plenty of social and recreational activities. The facility accepts several payment options, including private pay, insurance, Medicaid, Medicare, Veterans benefits, PACE, Managed Care Benefits, and Long-Term Care Insurance, making it easier for families to figure out what works best. Staff pay close attention to each resident's needs, working hard so patients get as much help-physical, emotional, or social-as they require. Right now, Poudre Canyon Health and Rehabilitation Center LLC isn't taking new patients, but you can check for updates about openings. The main language spoken by staff is English, and the management group, Recover Care West, oversees the day-to-day running of the facility to keep standards high and care consistent.
