    Compassionate care helped my mom

I'm very pleased with Birchwood - compassionate, attentive nurses, doctors and therapy staff (special shout-outs to Bruce, Terrance, Javier and the PT team) helped my mom recover and regain independence. The facility is clean, quiet, with great food, engaging activities and strong care coordination; administration and dietary went above and beyond. Overall very good care and clear improvement under leadership - I'd recommend a tour.

    Summary of reviews

The reviews present a mixed but evolving picture of Birchwood Health and Rehab. Many families and visitors praised the facility's caregiving staff as compassionate and attentive, and they frequently singled out the therapy and rehabilitation teams for producing effective, motivating outcomes that helped residents return home. Clinical services such as wound care, pain management, and on-site specialty services (vision, dental, podiatry) were described positively; several accounts emphasize coordinated appointment scheduling and discharge planning as strengths.

Care quality appears uneven. Positive experiences with accommodating nurses and effective wound care coexist with reports of inconsistent nursing performance across units and shifts. Night-time attentiveness and staffing adequacy emerged as recurring operational concerns. Reviewers also raised issues about medication handling and consent practices, including one or more serious financial or medication-related claims; these have contributed to family anxiety and calls for clearer documentation and tighter medication controls.

Dining, activities, and therapy were commonly cited as strong aspects of the resident experience. The dietary team received praise for variety and responsiveness, and activity offerings were described as engaging. Physical and occupational therapy were repeatedly noted as standout services, with specific praise for staff who motivated residents and supported successful rehabilitative outcomes.

Facility conditions are described as generally clean in public areas, but an aging building and spot-specific sanitation or mold concerns were mentioned. Maintenance responsiveness appears inconsistent: while many reviewers commented on well-kept common spaces, others noted delays in addressing in-room repairs and amenities. Families considering placement should verify maintenance turnaround times and inspect specific rooms.

Management comments indicate a trajectory of change. Several reviews highlight improvements under current leadership, describing a hands-on administrator and stronger operational direction; conversely, other reviews raised substantive concerns about billing, insurance handling (including unauthorized insurance changes in at least one account), delayed payments to contractors, and contractor communication. These administrative issues have affected perceptions of overall reliability and require operational attention.

Notable patterns: strong rehabilitation and therapy services; a caring frontline staff and accessible administration in many cases; counterbalanced by inconsistent nursing quality, medication and consent-process vulnerabilities, staffing shortfalls at night, and administrative/financial control weaknesses. Prospective residents and families should weigh the facility's evident clinical and rehabilitative strengths against these operational concerns, tour the building, ask about current staffing levels and medication-safety protocols, and request documentation of recent administrative or corrective actions before deciding.

    About Birchwood Health and Rehab

Birchwood Health and Rehab is a for-profit nursing facility with 87 licensed beds, run by Birchwood Rehab Holdings LLC and managed by Mr. Peter Lewis, and it's been inspected by the Florida Agency for Health Care Administration, with a license number 12640961, and holds joint commission accreditation, and you'll find a mix of skilled nursing, long-term care, rehabilitation services, assisted living, memory care, adult day services, home care, and hospice care there, so people can get post-acute rehab, therapy like physical, occupational, and speech, wound care, pain management, and 24/7 nursing care all under one roof, and they've got private or semi-private rooms, cable TV, internet, and telephone, with home-like surroundings and modern touches, plus social activities and support services for daily needs. People paying for care will find the monthly cost changes based on the type of room and level of care needed, and the place accepts Medicaid and Medicare. Their CMS inspections show mixed results, with quality measures above average, staffing about average, but health inspections and the overall rating slightly below average, and the nurse staffing rate-at 3.55 nurse hours per resident per day-is below the state average, nurse turnover's pretty high at 51.9%, which is above what's seen elsewhere, and the place often has around 76 people living there. Birchwood Health and Rehab has had a number of state inspection deficiencies, with 21 cited in 2023 and several specific issues like food safety, resident rights, pressure ulcer care, and following doctor's orders, plus a total of $49,680 in fines, so that's probably why it's listed on the Florida Nursing Home Watch List and sometimes faces license sanctions or appeals, and you'll find it's had some trouble with staffing levels and ensuring a homelike environment, but it does keep a focus on offering a range of care and specialized programs for different needs, aiming to support both safety and well-being for the people living there.
