    Caring, prompt staff; clean, engaging

I've been very pleased with the caring, friendly staff-Mary Pixley, Tina Miller, Stephanie, Julonda, Nai'Kiera, Kayla, Trena and Hope really stand out. Nursing, wound care and therapy are professional and prompt; medications are given on time. Admissions was fast and easy, the building is clean and homey, meals are plentiful and activities (bingo, crafts, Sunday school) keep residents engaged. Staff consistently go above and beyond and feel truly compassionate-I'm grateful and would recommend this close-to-home option.

    Summary of reviews

The reviews for Azalea Health Center present a mixed picture with clear strengths in frontline care and activity programming alongside operational and management concerns that families should weigh carefully. Many comments emphasize caring direct caregivers — CNAs are frequently described as compassionate, attentive, and willing to go above and beyond. Nursing staff receive praise in areas such as wound care, and therapy teams (OT/therapy assistants) and admissions personnel are highlighted for speed and helpfulness. The facility offers a broad activities program including arts and crafts, games, and faith-based classes, and several families describe the environment as homey and conveniently located.

At the same time, a number of recurring operational issues appear across reviews. Staffing levels and turnover are a common theme, with families describing heavy CNA workloads and under-staffing that contribute to missed tasks and scheduling errors. Several reviews point to inconsistent clinical coordination, including medication-administration problems and missed scheduled appointments, which in at least one case contributed to rehospitalization. There are also repeated mentions of gaps in supervision and monitoring, and of inconsistent personal-care practices such as bathing and incontinence care.

Facility and service-level concerns include sanitation issues in some bathrooms and common areas, laundry disruptions, inconsistent hot water, and an overall worn facility appearance in places. Meal service receives mixed feedback: some families praise plentiful, preference-tailored meals, while others describe inconsistent quality and poor preparation. Communication and responsiveness from administration and some staff are important pain points — families cite difficulty getting information, curt or rude interactions, and staff who are unreachable by phone. A small number of reviews raise serious concerns about staff conduct and unprofessional behavior; these comments, including isolated allegations of misconduct, suggest the need for investigation and close family oversight.

In summary, Azalea Health Center shows strengths in hands-on caregiving, wound care expertise, therapy services, and an active activities program, which may make it a good fit for families prioritizing those areas. However, prospective residents and families should be attentive to operational risks: staffing stability, medication and clinical coordination, sanitation and facility maintenance, and the quality of communication from management. Regular family visits, clear expectations with staff and leadership, and confirmation of care plans (medication schedules, bathing/incontinence plans, wound-care follow-up) are advisable steps for those considering this facility.

    About Azalea Health Center

Azalea Health and Rehabilitation Center sits at 1600 Anthony Rd in Augusta, Georgia, in a spot close to major medical centers like the Charlie Norwood VA Medical Center and Summerville Internists, which can give families some peace of mind knowing help's nearby if there's ever an emergency or someone needs a specialist. The center has 99 certified beds and provides a full range of skilled nursing and rehabilitation services, including memory care, on-site dialysis, cardiac management, and both palliative and respite care for those who need a short stay. Staff are available 24 hours a day, and you'll see in-house physical, occupational, and speech therapists who stick with the residents as employees rather than outside contractors, which can help with consistency in care. There's also access to onsite doctors and nurse practitioners five days a week and after-hours on-call healthcare every day.

Residents get personalized assistance with daily personal tasks like bathing, dressing, transferring, and medication management, though the facility has some documented deficiencies when it comes to medication error rates and care planning. There have also been reports of issues with assessment processes and hands-on care for daily living, which shows that, like many other places, there's room for improvement. The nurse turnover rate is fairly high at 56.5%, compared to the state's average, but the staff spend above-average time with residents, at 3.63 hours per day.

Azalea's known for being a faith-based facility, and people there can include prayer in their care plans if they like. The center holds a Bronze Quality Award from the American Health Care Association, and it's earned The Gold Seal of Approval from The Joint Commission, which speaks to efforts in quality and safety, even though inspections have still found areas to fix. The place is for-profit and run by a Limited Liability Company, with an affiliation to Harborview Health Systems.

Amenities include spacious, furnished rooms with private bathrooms, cable TV, kitchenettes, air conditioning, phones, and Wi-Fi. Residents can use the 24-hour call system for any needs. The dining area offers restaurant-style meals, prepared by professional chefs, and can handle allergies and special diets if needed. For socializing and recreation, Azalea provides community activities, fitness classes, movie nights, and music programs, along with walking paths, gardens, outdoor common spaces, a game room, a library, arts and activity rooms, and spa services like those at Esteem Salon and a wellness room.

The center also offers parking, transportation, move-in help, housekeeping, emergency alert systems, and a care planning process that includes residents, their families, and a full team of professionals. The environment encourages connections with both caregivers and residents, aiming to boost quality of life through attention to everyone's needs. Azalea takes Medicare and Medicaid as payment and is part of a broader senior living community that may include assisted living and memory care. Leadership emphasizes strong relationships and positive morale, working to provide support for caregivers as well as those living at the center.

Azalea Health and Rehabilitation Center isn't currently accepting new residents, but still has a focus on skilled nursing and rehabilitation care, working to support both short-term and long-term stays with a range of healthcare and daily living services. The facility's online profile has detailed maps and lets people share or print the listing, and families can claim the listing if they want to add photos or descriptions. Staff speak English, and the center keeps a place in the community as a nursing and rehab option under the Harborview name.
